MEMBERSHIP INFORMATION Any billing changes can be accepted at our Welcome Desk and require a 30-day written notice. o Examples of billing changes include: Membership Hold Medical Hold EFT changes Cancellation of membership or program For immediate membership account information updates, we are excited to offer an online portal! You can visit www.myiclubonline.com or click on the link from our website. Here, you can update your contact information, billing information and retrieve your check-in log. All children under the age of 13 must be accompanied by a parent at all times while on the NTC campus. Children age 6 years or older must use gender-assigned locker rooms. Any guest under the age of 18 years old must have a parent or legal guardian present to sign a waiver. There are certain memberships that are eligible to bring up to 4 guests per year, free of charge. Guests must be 18 years of age or older. Please check in your guest upon arrival to update your account. For safety and security, no bags are allowed in the fitness area. Small storage spaces are provided in the fitness center. Lockers are also available for your use in both locker rooms. Please provide your own lock. Fitness Center Guidelines For the safety and consideration of all, members and guests are asked to please: Re-rack all weights after use. Place gym bags in lockers or cubbies. Wipe down equipment after use. Allow others to work-in between multiple sets. Drop weights in designated areas only. Wear appropriate footwear. Use only closed containers for beverages. MEMBER FITNESS SERVICES As part of your membership, you receive complimentary fitness program design and instruction by our qualified, degreed and certified team members. Program Design Initial (60 minutes) - During this appointment a fitness team member will develop and instruct a fitness program based on your goals and current fitness level. The goal of this appointment is to instruct you on proper form and usage of equipment to ensure you are comfortable in initiating your exercise program independently. Program Design Review (60 minutes) During this appointment a fitness team member will review your current exercise program and recommend modifications to further your benefit of the program. For those members looking for additional support, motivation, accountability and goal-specific program design, we offer private and group training. These sessions are an additional cost. FastTrac (4 weeks) - FastTrac is designed for beginner to intermediate fitness levels wanting to build confidence in performing their strength training program. This program will teach you the fundamentals of weight training, proper progression and safe technique. Cost $96.00. Personal Training (60 minutes) Personal Trainers provide expertise, motivation and accountability. Whether you are training for sport or health, we have a team dedicated to helping you reach your fitness goals. Includes Private Yoga and Pilates Reformer training. Cost $45-$55 per session. Buddy Training (60 minutes) Personal Training with a buddy. Cost $60-$80 per session. Small Group Training (45 minutes) People work harder, have more fun, and get better results when they train in a team setting. Sessions are offered two or three times per week and are tailored to the individual including nutrition counseling, strategic monthly measurements and homework. Cost $200-$300 per month. TRX Training (45 minutes) TRX suspension trainers allow for controlled body weight exercise that develops strength, balance, flexibility and core stability simultaneously. Cost $8-$10 per session. Human Performance Lab Services (varies) At the NTC we offer numerous human performance services including basic and sport performance nutrition consulting, running analysis, periodized plans, lactate threshold analysis and more. KIDS CLUB Rules and Policies REGISTRATION: Kids Club is a child watch service provided for National Training Center members to be utilized while working out inside the facility. For the safety and liability of your children, a notarized authorization form is required if you have given another caregiver permission to pick up and drop off your children. Those attending Outpatient Rehabilitation may use the Kids Club services while in an appointment. Children ages 12 weeks to 12 years are welcome in Kids Club. A registration form must be on file for each child attending Kids Club. Please be aware that the Kids Club attendant has the discretion to limit the number of children allowed at any one time. The time limit is 2 hours per session per day. DROP-OFF AND PICK-UP: The same parent/guardian who drops off the child must also pick up the child unless the Kids Club employee is notified at time of drop-off. Both parents/guardians must be on the Authorization Form. Parents/guardians may not leave the National Training Center, for any reason, while their child is in Kids Club. Violation of this policy will terminate all privileges of using Kids Club. Only registered children are allowed in the Kids Club area. RULES: Children must wear closed toed and closed heeled shoes (or socks for infants) at ALL times. Flip flops are not allowed. Diapers must be changed by the parent/guardian and disposed of outside the facility. Staff members will not change diapers. The parent/guardian will be notified and should respond to diaper needs promptly. There is a 15 minute cry limit for children who cannot be consoled or comforted. Extreme unruliness or other disruptive behavior will not be permitted. If the staff cannot manage the negative behavior, the parent/guardian will be asked to remove the child from Kids Club as soon as possible. Kids Club is for use only during posted hours, while working out inside the facility. Unsupervised children are not permitted in Kids Club or any other areas of the facility.

KIDS CLUB Rules and Policies ILLNESS: Please keep children at home if they are sick or have a fever. Children with a fever or yellow/green nasal discharge will not be allowed in Kids Club. Any child taking antibiotic medications should be on them for 24 hours before attending Kids Club. Personnel are not allowed to administer any over-the-counter or prescription medication(s). BELONGINGS: Child s first and last name should be clearly printed on all belongings brought into Kids Club. Toys from home are not allowed. However if it is a small security item it will be permitted until the child is acclimated in the room and then it will be put away. Electronic devices will be permitted. Use must be appropriate for children under the age of 12, and we reserve the right to take away the device if it causes a problem or if we feel it is being used inappropriately. Our staff is not responsible for personal belongings. FOOD AND DRINKS: No gum allowed or food is permitted in Kids Club. All drinks must be in spill proof containers (drink boxes/pouches are not considered spill proof) and must have the child s first and last name on it. We do not bottle feed, however if your child has a bottle and is able to feed him/herself that is permitted. Thank you in advance for your cooperation. Your child s safety is our number one concern. AQUATICS PROGRAMS ISR Infant Swimming Resource goes beyond traditional swimming instruction by teaching children ISR Self-Rescue techniques designed to help them survive should they reach the water alone. ISR offers individualized swimming instruction for infants and young children ages 6 months 6 years, teaching competence and confidence in the water. For more information or to register, contact Program Instructor Karen Faust at Karen.Faust@orlandohealth.com. Cost: $510 (complete program); $85 per refresher week. SwimAmerica Designed for children and adults, ages 4 and older. SwimAmerica is an exciting stroke progression program teaching water safety, fitness and fun! Participants learn swimming skills based on their existing abilities, from blowing bubbles to butterfly. For more information or to register, contact NTCSwimAmerica@orlandohealth.com. Cost: $80 per session. NTC Lightning Part of National Training Center Aquatics (NTCA), the NTC Lightning Swim Team is comprised of a variety of training groups ranging from the entry-level swimmer to the Senior/National and Olympic Trials qualifier. NTCA s team goals are to develop the character, skills, and desire necessary for each child to realize their full potential. Our swimmers develop physical fitness, confidence, self-discipline, and enhanced self-esteem, enabling them to maximize opportunities, both athletically and personally. Cost: varies per training group. A try-out with NTCA coaching staff is required prior to registration. For more information, contact Head Coach Don Gibb at Donald.Gibb@orlandohealth.com. NTC Masters NTC Masters is the leading US Masters Swimming team in the Lake County and Central Florida area. We offer multiple practice sessions throughout the week at the NTC. Our NTC Masters Head Coach, Mateo De Angulo, is a Level 2 ASCA Coach and also an NTC Aquatics Assistant Coach. For more information, contact Mateo at Mateo.DeAngulo@orlandohealth.com. Cost: $40 per month. Private Lesson (30 minutes) Private lesson with an NTC Aquatics staff member. Semi-Private Lesson (45 minutes) A semi-private lesson includes 2 or 3 participants and one instructor. AQUATIC CENTER INFORMATION Pool Access Pool usage is included in the family monthly membership fee. Children up to 3 years of age may use the pool for free. For children 4-12 years of age that are not included in a family membership, a 10-visit minor pool pass is available for purchase. A parent or legal guardian must be present. Nonmembers may purchase an adult day pass. A POOL-ONLY day pass may also be purchased dependent children. **Child Day Passes are sold ONLY with an Adult Day Pass or proof of membership.** Pool Guidelines No one is allowed in the pool unless a lifeguard is on duty. In case of emergency, notify aquatic staff or call 911, or 9-911 from a facility phone. Pool operational hours are posted. Please shower before entering the pool. Food, drink, glass, gum and animals are prohibited on the pool deck; Food and drink are only permitted under the awning. A Lifeguard s decision is final. Proper swimming attire is required--no cutoffs or street clothes permitted. Children who are not potty-trained must wear swim diapers. Use of pool equipment (i.e. kickboards, pull buoys, paddles, etc.) is restricted to lap swimmers and team training only. Hanging on the lane lines is prohibited. Rafts, pool toys, and water guns are prohibited in the pool area. Arm flotation devices of any kind and flotation devices that are not Coast Guard approved are prohibited. A personal flotation device may be used if it is Coast Guard approved. Pool lanes are intended for use by multiple swimmers, and circle swimming is required. NO DIVING off of starting blocks. Use of blocks is restricted to competitive teams only, under the supervision of a coach. Diving permitted in designated areas only. Children must be accompanied by an adult at all times. Parent must with their child(ren) at all times. Running, horseplay, and flips into the pool are prohibited. Using the bulkhead as a thoroughfare or shortcut is prohibited with the exception of lifeguard staff and coaches. If someone becomes ill or has a fecal accident, please alert the aquatic staff immediately. Persons with open wounds or contagious skin conditions are not permitted in the pool.

AQUATIC CENTER INFORMATION Lane Set Up The typical lane set up for the NTC Pool is as follows: Monday Wednesday: Short Course (25 yards) Thursday Sunday: Long Course (50 meters) During various times of the year, this lane set up noted above may be adjusted. These changes will be communicated via the weekly member e-newsletter. Pool Hours of Operation Adjustments ThorGuard lightning detection system In the event of bad weather, the facility will adhere to the following guidelines at the discretion of the staff: o Thunder/Lightning: The facility will be closed when ThorGuard horn and flashing light are activated. The facility will remain closed until ThorGuard issues the all clear horn. The facility will also be closed at the discretion of staff for unsafe weather, including heavy rain that prohibits a Lifeguard from adequately seeing the bottom of the pool. o Weather Advisory: Pool closed at the discretion of the staff. Pool Closure The pool will periodically close due to inclement weather, scheduled maintenance, swim meets, or events. Members will be notified as soon as possible. Delayed Opening When the outside ambient temperatures are forecasted to be 40 Fahrenheit or below during the time of pool opening, the pool will delay opening. The pool will open for normal business hours when the outside ambient temperature is forecasted to be 41 degrees or higher. Aqua Group Fitness Class Cancellation o For morning classes, if the temperature is 50 degrees or colder one hour prior to class start time, class will be CANCELLED. o For Evening Classes, if the temperature is 55 degrees or colder one hour prior to class start time, class will be CANCELLED. o Temperature will be determined using the Weather Bug App.
