Our mission at North Florida Elite Cheer & Tumble is to provide quality instruction in a fun, safe and positive environment. We are dedicated to providing an atmosphere in which EVERY child can experience the thrill of success, regardless of skill level. Each and every family is important to us and our program. Inspiring the future one flip at a time! Please be prepared to turn in the Financial Commitment/Credit Card Info page, Tryout Form, Member Information Form, Waiver, Team Rep form, and your tryout fee. If you did not turn in a birth certificate during the season you must provide a photocopy of your child s birth certificate. Tryout fee is $20.00 INCOMPLETE PACKETS WILL NOT BE ACCEPTED. FAQ S Q: How much does it cost per year? A: Cost will be between $1,800-$2,000 depending on the team. Included in this is amount is Monthly Tuition, Competition fees (comp fees include entry fee as well as coach travel expenses). Not included: uniform, warm-ups, travel costs, US Finals, The One and/or Summit fee and (extraneous team shirts, bows, etc. are not included as well). Q: Is there payment plans? A: Yes, there will be several payment choices. Q: Will there be fundraisers? A: YES! We average about 1 fundraiser a month. We have had a few families pay for their entire season of tuition just through fundraisers. New for the NFE Booster Club. If you want to cheer we try to make it happen. Q: What competitions do we attend? A: Our Full Year teams will attend competitions in Florida and Georgia. For our Prep teams we will attend mainly Jacksonville competitions. Q: How do you select teams? A: We build teams so to be the most competitive in each level. We base these placements on physical ability (jumps, tumble, motions, etc.), as well as past cheer experience and attitude. Athletes will be placed where the staff feels they will excel the most. Q: Does everyone make a team? A: We do try our best to place every athlete. However, if you have requested a specific level and we cannot accommodate that, you will not be placed on a team. Q: Are team placements set in stone? A: No, We place athletes based evaluations. If it becomes obvious the athlete is either exceling at their current level or is in over their head, team placement may be rearrange up or down. We do what's best for the athlete & the team. 1

Tuition/Fee Information All payments are made to NFE. Tuition: (May 2017 through April 2018) $95 monthly Uniform Due September 1st $365 Competition fees (Approx.): Due November 1 st $700-850 Choreography Camp (includes Music) : Due:June15 $290 Monthly Tuition Includes: 4 hours of practice per week Any extra Saturday practices Monthly Tuition Does Not Include: $30 Registration Fee for new families. Uniform, warm-ups, shoes, make up, bows, etc. Competition Fees (includes coaches fees & expenses) Choreography Fee $35 USASF Fee Fees are due on or before the 5 th of each month and are considered late after that point. On the 6 th of each month any outstanding balance will be charged to your credit card that was provided in your Tryout Packet. NFE accepts cash, checks, money orders and all major credit cards. *All accounts must be paid in full in order to try out for the upcoming season. *Please contact Kyle Nelson with financial questions, your coach will not be able to advise you on this matter. *Cheerleaders may be asked to sit out of team practices if accounts are not current. There will be no refunds made to anyone who is asked to leave the program or that quits a team. 2

PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK In consideration of the services of North Florida Elite Tumbling, LLC., its owners, agents, officers, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as NFE ), I hereby agree to release, discharge, and hold harmless NFE, on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate as follows: 1. I understand and acknowledge that the activities that I or my child engage in while on the premises or under the auspices of NFE pose known and unknown risks which could result in injury, paralysis, death, emotional distress, or damage to me, my child, to property, or to third parties. The following describes some, but not all, of those risks: Cheerleading and gymnastics, including performances of stunts and use of trampolines, entail certain risks that simply cannot be eliminated without jeopardizing the essential qualities of the activity. Without a certain degree of risk, cheerleading students would not improve their skills and the enjoyment of the sport would be diminished. Cheerleading and gymnastics expose participants to the usual risk of cuts and bruises, and other more serious risks as well. Participants often fall, sprain or break wrists and ankles, and can suffer more serious injuries. Traveling to and from shows, meets and exhibitions raises the possibilities of any manner of transportation accidents. In any event, if you or your child is injured, medical assistance may be required which you must pay for yourself. 2. I expressly agree and promise to accept and assume all of the risks, known and unknown, connected with NFErelated activities, including but not limited to performance of stunts and use of trampolines. My participation and that of my child is purely voluntary. No one has forced or coerced me or my child to participate. I elect for myself and my children to participate in such activities in spite of the risks. 3. I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify NFE from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with my child s participation in NFE-related activities. 4. Should NFE be required to incur attorney s fees and costs to enforce this agreement, I agree to indemnify and reimburse them for such fees and costs. 5. I certify that my child has health, accident and liability insurance to cover bodily injury or property damage that may be caused or suffered while participating in this event or activity, or else I agree to bear the costs of such injury or damage to my child. I further certify that I am willing to assume and bear the costs of all risks that may arise or be created, directly or indirectly, through or by any such condition. 6. In the event that I file a lawsuit against NFE, I agree to do so solely in the State of Florida and I further agree that the substantive and procedural laws in that state shall apply in any such action without regard to the conflict of laws rules thereof. I agree that if any portion of this agreement is found void or unenforceable, the remaining portions shall remain in full force and effect. 7. By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation or the participation of any of my children in this activity, I may be found by court of law to have waived my right to maintain a lawsuit against NFE on the basis of any claim from which I have released NFE by signing this Agreement. I have had sufficient opportunity to read this entire document. I have read it and understand it. I agree to be bound by its terms. Program of Conduct If you have any questions or concerns that need immediate attention, please use the following chain of command: 1. Team Rep 2. Head Coach 3. Owner North Florida Elite maintains the right to refuse services at any time. Good sportsmanship, polite manners and a good disposition are mandatory at all practices and competitions. It is your responsibility to wear the appropriate practice wear to every practice. Personal items such as cell phones, school bags, or gym bags must be taken into the gym and stored in the cubbies adjacent to your practice floor. NO items are to be left in the gym hallways and cell phones must be turned off or muted when practice starts. No Gossip about any other teams (all star or school), about a child on your team or another team or about coaches and staff. It is much better to address a problem than to listen to idle gossip. No profanity or abusive language. Please keep in mind the gym is not a babysitting service. We want our All Stars to use the gym as often as possible, but no child should be dropped off at the gym unsupervised. You must arrive at all practices, competitions or any scheduled event on time. Punctuality is a MUST. During practice your opinion does not count. All squad and routine decisions are left to the discretion of the coaches. Please feel free to talk to your coach about anything; remember to do it at the appropriate time. For example, approaching a coach in the middle of a practice would not be an appropriate time. Never post any negative comments on social media. Practices may be changed or added at any time during the season. Anyone threatening to quit or to pull their child from a team will be dismissed from the program immediately. Only cheerleaders and coaches are allowed in the gym. No one is allowed to yell onto the floor or try to make contact during practices or tumble classes. This is extremely distracting to all involved. The coaches reserve the right to close practices at Any time for Any reason. In this event, the team rep will remain at practices. It is the parent s responsibility to know what is going on with your squad. Check your emails, Facebook page, iclass and the website regularly. Withholding a child from a practice or a competition should never be used as a form of punishment. Athletes should be able to handle school work and all star practices, homework are not an acceptable excuse for missing practice. Parents, relatives, friends and cheerleaders are never allowed to speak with competition officials for any reason. Parents are never allowed to represent North Florida Elite under any circumstances concerning accommodations, competitions or any other situation. There will be no arguing or questioning of the coaching staff s decisions at competitions. If a problem arises between you and a teammate or another parent, the problem will be addressed with all parties involved at a meeting with your coach and the All Star director. Anonymous emails/letters/texts/twitters, etc. will be ignored. The All Star Director may change, add or subtract any rule at any time. 8

TRAVEL All Cheerleaders will stay the night before at all out of town competitions. There must be a parent in every room, no exceptions. Each cheerleader must have a chaperone at every competition. It is not your coach, Team Rep or NFE staff s responsibility to be your child s chaperone. It is your responsibility to pay the hotel directly for your room. Proper traveling attire will be discussed with each team prior to an out of town competition. APPAREL All NFE logos and brands are protected. All spirit wear items are to be purchased through the NFE Pro Shop or an approved vendor. If a sponsor wants to buy an item for entire team; that order must go through NFE as well. All spirit items must be approved by Stefanie or Kyle Nelson Bows, t-shirts, bags, blankets, etc. with an identifiable icon (i.e. panther paw, logo, team names) not purchased through NFE or approved Vendor are not permitted. Teams will not be allowed to wear or use as a group. Anyone that attempts to sell or give away items with NFE logos or likenesses will be in violation of our policy. Money cannot be asked of parents to buy spirit items, goody bag items or any other nonmandatory items. Team Reps may collect money for coach gifts, but this is strictly voluntary. NFE has worked very hard to establish a brand and we take this policy very seriously. Please help us protect our good name! Thank you for choosing to become a part of our program. Our hope is that this is a wonderful experience for your entire family. However, we do know life gets in the way sometimes and we want you to know up front some ways that your spot may be in jeopardy on your team: delinquent accounts, loss of skills/abilities, extensive absences/tardiness, poor attitude or injuries. If an injured athlete is replaced in a routine, they may or may not be worked back in the routine; this will be up to the coach. As always, if you have any questions feel free to contact us. All teams will have a very informative meeting on Monday May 22 at 7pm. This meeting will go over the coach s expectations of your new team. You MUST the meeting. If you are unable to attend one of the meetings your athlete will be unable to practice until a parent has a sit-down meeting with Kyle to review the entire packet.
