SCVA JUNIOR GIRLS MANUAL 205 August 204 To Team andor Club Representatives, Welcome to the 205 SCVA Junior Girls Volleyball Season! We will begin our season with the Qualifying tournaments in January. Please look over the tournament registration forms included in the packet for tournament dates and registration deadline dates. Please read over the manual carefully. There are recent changes regarding the registration process, including the commitment letter, tryout dates, and Application to Change Clubs procedure. There are also new divisions for the 205 USAV Junior Olympics. We also ask for your continued support in helping keep the facilities clean during tournaments. It is your responsibility as club representatives to make sure that all staff, coaches, and parents associated with your club are aware of all the rules and regulations. Please inform all parents and all outside parties of the rules specific to the junior girls season. Please discuss, explain, and emphasize the rules specific to eligibility, registration, and facilities. Parent handbooks are available through the SCVA office and website. The USA Volleyball Waiver and Release of Liability Form has been drastically changed over the past few years. There is a new Individual membership form that is formatted to use for all membership types; i.e. junior player, adult player, coaches, chaperones, officials, etc. Please indicate on the form which membership type is being submitted. This is the only form that will be accepted no old forms will be accepted! All registration material is available on our website under the blue link Registration across the top of our home page. The Parent Handbooks are available this year in pdf format only. Please provide your club parents with the link to the document under the blue link Registration across the top of our home page. I. GETTING STARTED A. Registration SCVA JUNIOR GIRLS MANUAL 205 All members (coaches, players, chaperones, team representatives, and club directors) MUST register with the SCVA in order to participate in any SCVA activity. All adults involved in the club (Including club directors, coaches, team representatives, and chaperones) must pass a USA Volleyball Background Screening test prior to the registration process. The background clearance is good for a two year period. The background screening process will take at least 4 days for processing. Please incorporate this time frame when registering adults. Please refer to the registration packet that has a copy of the waiver and release of liability form for all members, the team registration form, the medical release form, and the consent and waiver form for the background screening of all adults associated with the club. Please refer to the directions in the registration packet for specific instructions regarding online registration and additional registration information. If you have any questions regarding registration call the SCVA office. DUE TO THE USA VOLLEYBALL INSURANCE REGULATIONS, REGISTRATION FOR ALL CLUBS IS DUE TWO WEEKS PRIOR TO THE FIRST TOURNAMENT IN WHICH THEY ARE ENTERED INCLUDING THE COMPLETED BACKGROUND SCREEINIG PROCESS. A 204205 Individual Membership Form for each player, coach, and team representative must be completed. This waiver and release form is two sided and requires signatures on both sides. All sections must be filled out completely and signed on both sides before the registration form will be accepted. Along with the 204205 Individual Membership Form a completed Letter of Commitment must be submitted for each player. All clubs must collect a copy of the Medical Release Form from every player. It is mandatory that every coach carries a copy of the Medical Release Form for each player in case of an injury. Do not submit these forms to the SCVA office. They are to remain on file with your club. As part of registration, it will be mandatory for each coach to sign a CODE OF ETHICS and return the signed code along with their registration form and consent and waiver form in order to be eligible to coach in any SCVA event. A TEAM REGISTRATION FORM must be filled out for each team and submitted with the individual registration forms. The team registration form is not mandatory to use. You may elect to use a form of your own choosing (i.e., off of your computer). However, if you do use your own form, the information must be in the same format and in the same order as the attached SCVA form. Online individual registration, the completed team registration form, the completed and signed individual registration forms, and the appropriate money must be received in the SCVA office before registration is complete. Birthdates must be submitted for each registered member. Please check the forms carefully and make sure they are complete. All clubs must collect a copy of the Medical Release Form from every player. It is mandatory that every coach carries a copy of the Medical Release Form for each player in case of an injury. Do not submit these forms to the SCVA office. They are to remain on file with your club. The registration fees for the season are as follows $60.00 for each junior player, $60.00 for each adult, plus an additional $20.00 for background screening. The fees have been increased due to the increase of USAV registration fees. For each junior team registered the fee is $5.00. Club registration is $50.00 which includes registration for both girls and boys clubs using the same name as well as for facility insurance. Note No club with an outstanding balance will be eligible to register with the SCVA until the balance is paid in full. Registration forms filled out and signed at club tryouts are not considered commitment to play by the players. 4

SCVA JUNIOR GIRLS MANUAL 205 You may register new members throughout the season as long as ) the player is registered online, 2) the completed paperwork and appropriate money is received in the SCVA office ONE WEEK prior to participation, and 3) all eligibility requirements are met. Please note that a form to add new players to your club is enclosed in the registration packet. Registration of new players will not be accepted unless the add form is enclosed along with the waiver and release form. No USAV registration cards will be distributed this season. Each club has the ability within their online database to keep track of every player and staff and their USAV #. Upon request we can provide a detailed description of how this is done. NO REGISTRATION (EITHER PAPERWORK OR MONEY) WILL BE ACCEPTED AT ANY TOURNAMENT SITE. It must be sent to the SCVA office. Please keep copies of ALL submitted registration forms (both sides), and have your coaches carry copies of the medical release forms. B. Insurance. Individual coverage Once registered with the SCVA, members are covered by the USA Volleyball s sport accident policy. Please note that the liability insurance as provided by the SCVA will not cover the club andor its administrators during practice, tournaments, or tryouts unless the club andor the SCVA has on file the signed waiver and release forms. If an injury occurs during a tournament an Incident Report Form must be completed (see Supplements) and returned to the SCVA office. Coaches must carry a copy of the medical release forms with them at all times! 2. Certificates of Insurance C. Entries Certificate of Insurance, naming facilities as additional insured, are available upon request from the SCVA office. The certificates will be issued within a three to four week period upon request. Request should be made in writing listing the club, specific facilities required along with their addresses, and sent by mail to the SCVA office. Please direct any questions regarding insurance to Ann at the SCVA office. The 205 schedule is enclosed. The completed entry form and appropriate entry fee must be in the SCVA office by the indicated deadline date before acceptance into any tournament. The entry deadline will be FRIDAY, three weeks PRIOR to the tournament date. Phone andor fax entries will be accepted, only if there is available room. The entry fee will be $250.00 per team for each day of each tournament, unless otherwise specified. One entry form can be used to enter any one or all of the tournaments you wish. One entry form per club is enclosed please make copies. The exceptions to this entry form are the Las Vegas Classic, the Junior National Qualifiers, and the Summer Soiree, and a tournament to be determined at a later date. These tournaments will have their own separate entry forms. No entry will be accepted unless accompanied by the entry fee. If a team is unable to participate, the SCVA office must be notified by 200 PM of the Friday preceding the tournament date. If there is a problem after 200 PM the Friday preceding the tournament date, you must leave a message at the SCVA office at (74) 97-3595. There will be no refunds issued if a team drops from a tournament 0 days or less prior to the date of the tournament. Confirmation of the site and time will be posted on our website at www.scvavolleyball.org the week prior to the tournament date. Please double check the playing sites on the Friday before the tournament date to make sure there were no last minute changes. Please take the time to inform your coaches and parents of the site locations for your teams, as it is your responsibility. 5

Age Divisions USA VOLLEYBALL JUNIOR PLAYER AGE DEFINITION For use during the 204-205 Season Once a player participates (including but not limited to practicing, training, attending workouts andor competition) in a club or varsity program for any university, college, community college, or junior college, heshe is ineligible to take part in any regional and national programming, which includes but is not limited to national JNC qualifying and championship events. 8 and Under Division Players who were born on or after September, 996 or Players who were born on or after September, 995 and a high school student in the twelfth (2th) grade or below during some part of the current academic year 7 and Under Division Players who were born on or after September, 997 6 and Under Division Players who were born on or after September, 998 5 and Under Division Players who were born on or after September, 999 4 and Under Division Players who were born on or after September, 2000 3 and Under Division Players who were born on or after September, 200 2 and Under Division Players who were born on or after September, 2002 and Under Division Players who were born on or after September, 2003 Note These age definitions are used for domestic club programs and are not the same as the age definitions for the International andor Domestic High Performance programs for 205. No waivers will be granted for the 205 season. Please note that a junior athlete or team may NOT play in two age divisions during the same season. An individual player may play in an older age division but must remain to play in that division for the entire season (exception optional tournaments). If you cannot field a team for a mandatory tournament, you must consult the SCVA office in order to obtain a one-time waiver for the particular tournament. If a waiver is not obtained, the team involved will forfeit all games for that day and will not be allowed to participate in the next scheduled mandatory tournament. If more than one waiver is granted during the course of the season due to a shortage of players, a different player must be utilized each time. II. ELIGIBILITY A. Beginning of Season Junior athletes are eligible to play for any club or team up until the beginning of the sanctioned season. At the time of her first court appearance during the sanctioned season, the athlete is to remain a registered member of that specific club throughout the season. Please note, that for a club to be eligible to participate in regional events, or to be eligible for entry into the Las Vegas Classic, the club must abide by the tryout and commitment dates set forth by the SCVA. However, abiding by the tryout and commitment dates does not guarantee entry into these events. 6

Tryouts, Commitment Dates, and Transfer Policy For the 204205 season, all junior girls clubs are eligible to host tryouts for the various age divisions on the following dates. Girls 4 and Under and 2 and Under Divisions - October 9th, 204 2. Girls 8 and Under through 5 and Under Divisions - October 2nd, 204 SCVA Tryout Procedures should be posted at all club tryouts. SCVA Tryout Procedures for San Diego Areas should also be posted at all tryouts. For the 204205 season, no junior playerparentguardian may commit to a club prior to the following commitment dates 400PM on Monday, October 6th for 8 and under through 5 and under division, and 400PM on Tuesday, October 4th for 4 and under and 2 and under divisions. No deposits may be given during the tryout period. Once the letter of commitment is signed and turned into one club, the player is then committed to that club. Please note, the SCVA recognizes that San Diego area high school players are bound to the CIF rules. The SCVA asks that all San Diego clubs and players abide by those rules, so as to not jeopardize the eligibility of girls during their high school season. Thus the following tryout dates and commitment dates will be used for those clubs in the San Diego areas. Girls 4 and Under - tryout date of October 8, 204 with a commitment date of October 27, 204. 2. Girls 6 and Under and 5 and Under - tryout date of November 8, 204 with a commitment date of November 7, 204. 3. Girls 8 and Under - tryout date of November 5, 204 with a commitment date of November 24, 204. A Letter of Commitment must be signed by all participants. After the tryouts and Letter of Commitment are signed and turned in, all teams in division 5 and under and higher do not begin practices until December, 204 or after. The Transfer Period will be from December 3th, 204 through February 20th, 205. No transfer between clubs will be allowed prior to or after the transfer period. In order to transfer between clubs during the transfer period, one must complete an application for a change of clubs, and then wait for the SCVA to approve the transfer. If a player does not follow the tryout procedures, commitment date, andor the transfer procedures, she may be suspended for the 204205 season. C. Coaches Impact Certification It is a requirement for all coaches be Impact Certified. There are webinars available through the USA Volleyball website www.usavolleyball.org under Resources. Or look at the information regarding Impact clinics on the SCVA website www.scvavolleyball.org. D. Inter-Regional Transfers A player shall represent no more than one region, club, or team in sanctioned competition during the same season. A family relocation due to a job or military transfer may receive special consideration. 7

SCVA JUNIOR GIRLS MANUAL 205 E. New Players Players may be registered at any time throughout the year as long as the proper paperwork and monies is received prior to participation. F. Team Changes Within Clubs During the season, an athlete may change teams within her age division within her respective club. However, rosters must remain the same for the Qualifying, Regional, or Special Event tournaments. Please note that it is your responsibility as coaches to make sure that the players as well as the parents are well informed of these eligibility rules. There is a section that the parents must sign on the registration form which acknowledges that they have been informed of these rules. They are to consult their coach or Club Director for further details. Any questions regarding eligibility please call Ann Davenport at the SCVA office. Please take the time to inform the parents of the involved athletes prior to the beginning of the season as this will help alleviate misunderstandings. G. Clearinghouse Procedures College Bound Athletes Prospective student-athletes intending to participate in Division I or II athletics must register and be certified by the NCAA Initial-Eligibility Clearinghouse. Please inform your athletes and their parents to obtain registration materials from their high school guidance counselors. III. NON-SANCTIONED COMPETITION Any team or club desiring to conduct a volleyball event or local, regional, or zone competition must receive a sanctionapproval from the SCVA. SCVA-registered players and teams competing in non-sanctioned competition may jeopardize their eligibility. AT THIS TIME, the SCVA will only sanction the following events tryouts, scrimmages, or friendship tournaments (where no entry fees are charged). USAVSCVA insurance is not in force during non-sanctioned events. Please refer to the USAV Official Rulebook andor Guidebook for specific policies. IV. RECRUITING POLICY Although the SCVA is aware of certain illicit recruiting actions, we have an inability to enforce such a policy because recruiting violations come to us in a he said, she said manner and are very difficult to ascertain fault. Therefore, we are asking the cooperation of all coaches and directors in this matter. The following are guidelines that we ask all clubs to abide by Refrain from making negative comments concerning other clubs andor their coaches. Allow no coach or club representative to make initial contact with an athlete or her parents. We ask that these guidelines be followed in order to foster a positive experience for the players in the pursuit of their volleyball goals. V. FORMAT The tournament format is designed to give maximum competition to each athlete in our program and to allow them to achieve a healthy and satisfactory attitude toward themselves, their teammates and the sport of volleyball. Following the qualifying tournaments, each team will be placed into their level of competition (Divisions). There will be movement of teams following each Mandatory Tournament. Points will be 8

SCVA JUNIOR GIRLS MANUAL 205 awarded to teams based on their order of finish in their respective divisions. No points will be given for optional or special event tournaments. Please refer to the enclosed point system for the specific point breakdown. A. Seeding The only information available for seeding teams is what is submitted by the club with their tournament entry form. Please include the following information Team Name Expected Division what seed you expect; Division 7-8 would be equal to Silver Division, Seed 3 or 4. Last year s finish for the team Strength comparison to last year s team. Comments helpful to understanding the team s strength and weakness. B. Qualifying Tournaments Requests for morning and afternoon playing times for Day will be considered only if noted on the Qualifying entry form and received by the entry deadline. The Qualifying Tournaments will determine the starting Division (and position within that Division) for each participating team in the SCVA Mandatory Tournaments. Any non-participating team that wishes to enter future tournaments will be placed in the appropriate position (excluding Divisions I, II, III, and IV). The Qualifying Tournaments have their own separate entry forms that MUST be used to enter the teams. Teams must enter ALL dates of the Qualifying Tournaments. A complete and detailed description for all the age divisions will be issued when the number of teams in each division is known. Please refer to the accompanied description. Remember, this is to be used as an example only; the exact format will be issued later. Any non-qualifying teams wishing to enter future tournaments will be seeded at the Program Director s discretion. 8 & Under Division 3 Days (Estimated 275 teams) Teams will be expected to identify themselves as Gold Division (top 64), Silver Division (next 64), Bronze Division (next 64), Copper Division (next 64), and Brass Division (all remaining teams) on the Qualifying entry. All teams wanting consideration for the Gold Division must submit a separate, written statement to justify this consideration. Without this statement, seeding will be done at the discretion of the Program Director. Teams will then be placed into the divisions based on their own rankings and the number of total entries. All teams will compete on Day, Day 2, and Day 3 in pool play and the final starting positions for the first Mandatory Tournament will be determined. 6 & Under Division 3 Days (Estimated 275 teams) Teams will be expected to identify themselves as Gold Division (top 64), Silver Division (next 64), Bronze Division (next 64), Copper Division (next 64), and Brass Division (all remaining teams) on the Qualifying entry. All teams wanting consideration for the Gold Division must submit a separate, written statement to justify this consideration. SCVA JUNIOR GIRLS MANUAL 205 4 & Under Division 3 Days (Estimated 240 teams) Teams will be expected to identify themselves as Gold Division (top 64), Silver Division (upper middle 64), Bronze Division (lower middle 64), and Copper Division ( next 64 teams), and Flight (all remaining teams)on the Qualifying entry. All teams wanting consideration for the Gold Division must submit a separate, written statement to justify this consideration. Without this statement, seeding will be done at the discretion of the Program Director. Teams will then be placed into the divisions based on their own rankings and the number of total entries. All teams will compete on Day, Day 2, and Day 3 in pool play and the final starting positions for the first Mandatory Tournament will be determined. 3 & Under Division 3 Days (Estimated 60 teams) Teams will be expected to identify themselves as Gold Division (top 32), Silver Division (next 32), Bronze Division (next 32), and Copper Division (all remaining teams) on the Qualifying entry. All teams wanting consideration for the Gold Division must submit a separate, written statement to justify this consideration. NO POINTS WILL BE GIVEN FOR THE QUALIFYING TOURNAMENTS. C. Mandatory Tournaments Each team positioned in all Divisions must play in each Mandatory Tournament. The 8 and under division will have 4 Mandatory Tournaments. All other divisions will have 5 Mandatory Tournaments. If a team does not participate, the team will automatically drop a Division, but points will be given for last place of that Division. If a team chooses not to participate in a SCVA Mandatory Tournament in order to attend another tournament elsewhere, that team could drop a division. All Divisions will consist of 6 teams. Points will be given to each team participating in the Mandatory Tournaments. Division I - IV The 6 teams will be seeded into 4 four team pools with match play. The top two teams from each pool will play-off in a one game to 25 bracket format which will be played all the way through. All four teams in each Division (with the exception of Division ) will advance one Division for the following Mandatory Tournament. The 3 th, 4 th, 5 th, and 6 th place teams in each Division will move down one Division for the following Mandatory Tournament. See below for the playoff summary.. Girls 3s 8s a. Playoffs for Divisions 2 ) Upper and Lower Brackets first round is 2 out of 3 games 2) Following rounds are game to 25 b. Playoffs for Divisions 3 4 ) Upper Brackets -- First round is 2 out of 3 2) Lower Brackets One round of playoffs 2 out of 3 games to determine which team drops a division. 2. Girls 2 and Under a. Playoffs for divisions and 2 only b. All playoffs are game to 25 0

SCVA JUNIOR GIRLS MANUAL 205 Division V thru remaining Divisions The teams will be seeded into four pools of four with match play. The winner of each pool will move up one Division, and the 4 th place team in each pool will move down one Division for the next Mandatory Tournament. Play will be divided into Am and PM sessions for Divisions V thru remaining Divisions. Starting times will alternate each Mandatory to ensure fairness. Please note if it becomes necessary to have a three team pool, match format will be the best 3 of 5 games. Five team pools will play 2 out of 3 games on 2 courts, if possible. D. Regional Tournament The Regional Championships will be a one-day event for the 6 and under and 8 and under age divisions. It will be a two-day event for all remaining divisions with pool play on Saturday and pool play followed by single-elimination brackets on Sunday. The top 32 teams based on the overall point totals will play in the Gold Division. If there is a tie on points for Regionals, the tie will be broken by using the most current finish from the last Mandatory tournament. The remaining teams will be divided into groups of 32, 48, or 64 to comprise the Silver, Bronze, Copper, and so forth. Double points will be awarded to all participating teams. E. Points A point system based on a percentage multiplier will be implemented. It is under review and will be distributed once the multiplier has been established. F. Inter Regional and Post Season Competition. The Girls Junior National Championships also offers three club divisions, the National, USA and the American divisions. For the National division, the SCVA regional bids will be awarded to teams based on the current regional rankings. There are 24 total USA bids all to be awarded at Junior National Qualifiers. The 8, 7, 6, 5, 4, 3, and 2 and under divisions will be awarded at least one American bid at each of the Junior National Qualifiers, providing that the division meets the required number of teams per division. The American bid will also give one bid per division to the SCVA region which will be awarded based on current standings. The Patriot division is a division for which anyone can apply for entry. It is filled on a first come first serve basis. The SCVA highly recommends that if a club andor team is interested in the Junior National Volleyball Championships to send all necessary paperwork and monies by the stated USA Volleyball deadlines. For further information regarding the Junior National Championships, Junior National Qualifiers, andor regional bids please contact Ann Davenport in the SCVA office. Once a team earns and accepts a bid in a Qualifier or applies for a bid in an at large process, the roster of that team is frozen. All players listed on the roster of a team qualified for the Junior National Championships may not transfer to any other club andor team. In addition, players on a frozen roster may not transfer to any other team even within the same club. 4. Summer Soiree Separate entry through www.advancedeventsystems.com. June 26 th June 29 th Anaheim, California 8, 6, 5, 3, 4 and 2 and Under Age Divisions. VI. TOURNAMENT REGULATIONS A. General Rules. A maximum of 5 players and 5 coachstaff personnel are allowed. NOTE Will be enforced this season. 2. Net Serve If the ball hits the net on a serve, the ball is alive (playable). 3. Substitutions In addition to the unlimited Libero substitutions, each team will have 2 team substitutions. 4. All matches are 2 out of 3 games, rally scoring to 25 points in games and 2 with no cap. Game 3 will be rally scoring to 5 points with no cap. Because of the rally point scoring method, teams have three options at the coin toss to serve, to receive serve, or to take choice of court. Please note that one game play-off will be one game to 25, rally scoring with no cap. 5. The service zone has been extended to the entire width of the end lines. 6. Ceilings andor obstructions are now in play. 7. The ball can be played with any part of the body. 8. Each player receives only one toss per service attempt. Two tosses are allowed for 4 and 2 and under divisions only. 9. If a back row player plays the ball above the height of the net while on or in front of the 3-meter line, the next player to touch the ball must be a teammate. If the ball enters the vertical plane of the net and is blocked by the opposing team, it is an illegal attack by the back row player. 0. Rules specific to the 2 and 0 and Under Divisions Net height will be 7 feet The Mikasa Ultra Lite Volleyball will be used (8 ounces) Two serving tosses will be allowed All other USA Volleyball rules will prevail. C. Tie-Breaking Procedure (for Qualifying, Mandatory, and Regional) In the case of a tie, pool position will be determined by the following formula 2 way tie ) match record, 2) head to head 3 way tie ) match record 2) game record 3) point ratio Play off seeding (Divisions I-IV only) Seeding will be based on ) pool finish 2) match record 3) game record 4) point ratio. Teams will be seeded to avoid any pool conflicts until the finals. The Tournament Director will make the final decision if there are any further complications. D. Pool Play Schedule Listed below is the schedule for both four and five team pools (please note that there are certain circumstances, which may change the listed order of the matches). Uniforms (Rule 5, Article Official USA Volleyball Rules). The playing uniform shall consist of a jersey, shorts, socks, and light shoes (rubber or leather soles without heels). 2. It is forbidden for Junior National age players to wear jewelry, including rope bracelets. 3. It is now required that all uniform numbers be centered on the chest and back. It will also be required that all the numbers be 6 in height on the chest and 8 in height on the back. Numbers must be in a color contrasting to that of the jersey and must be visibly located. Shirts may be numbered from through 99 inclusive. (0 or 00 is not allowed.) 4. Uniforms (jerseys and shorts) must be of the same color, style, and trim. If T-shirts, boxer shorts, tights, leotards, body suits, bicycle shorts, etc are worn in such a manner that they are exposed, they will be considered part of the uniform and must be worn by all the team members and must be identical. For the purpose of identical uniforms, shoes, socks, and kneepads are not considered part of the uniform and are not required to be identical for team members. During cold weather, it is permissible for teams to wear identical training suits provided they are numbered in accordance with the specifications of #3 above and are of the same color, style, cut, and trim. 5. Uniform regulations will be strictly enforced. The Tournament Director will be responsible for determining infractions and may make exceptions to this rule if circumstances warrant. 3

SCVA JUNIOR GIRLS MANUAL 205 F. Officiating Each team must provide a complete officiating team when scheduled. If the officiating team is late for their assignment, that team will be responsible for officiating the remainder of that match and the next non-pre-assigned match in that pool. Failure to officiate for an entire assigned match will result in the forfeit of a game win from that team's overall record. A game win will become a game loss. If a team is late for the last match of the day or misses the assignment, the penalty as described above will carry over to the next tournament. The officiating team will consist of SIX people. 2 nd Official 2. 2 Linepersons 3. Official Scorekeeper 4. Official Libero Scorekeeper 5. Visible Scoreboard Keeper A team that fails to have a complete officiating crew available for an officiating assignment - including a rostered adult (preferably a team coach) for that team - at the time the court official is ready to proceed can be penalized by losing points in the first set of its next match. For every minute an officiating team is late to fulfill its assignment, one point will be awarded to that team s next opponent in the first set of the next match, up to a maximum of 25 points. Penalty points begin to accumulate at the start of the receiving team s warm-up. An adult listed on the event roster must be present at the scorer s table and easily identifiable while any members of the team are officiating. Performing duties as the R2 on the match the team is assigned to officiate fulfills this requirement. A team may forfeit its entire next match for failing, a second time, to have an officiating crew available on time for an officiating assignment. Make sure your team members are familiar with the score sheet and can use it correctly. If you have any problems with an official or need a rule clarification, please call the office at 74-97-3595. G. Supervision A registered member of USA Volleyball is REQUIRED to supervise ALL team members during any SCVA competition. This person, or another registered adult, is required to remain on site until all team members have left the campus. This pertains to ALL age levels. Failure to follow this requirement will result in a $50.00 penalty fee, due before any future participation by the entire club. If the designated supervisor is someone other than a coach, the Tournament Director must be notified. H. Warm-ups A 0-minute warm-up (including ball-handling, hitting, and serving) will be allowed before your first match of the day ONLY--2 minutes shared ball handling and 4 minutes per team for hitting and serving. A 4-minute per team warm-up will be enforced throughout the remainder of pool play. I. Line-up Sheets 4

Prior to your first match of the day you will need to complete and turn in this form to the official on your court. This must be done at EACH TOURNAMENT. List only the players (First and Last Names) that will be participating on that given day. You will also need to provide the official with a separate line-up sheet for each game played during the tournament. The line-up sheet has on the reverse side the appropriate number of line-up forms for use during the entire day. No verbal line-ups will be accepted. J. Protest Procedure A protest must be filed at the time of the incident no later. Only the floor captain may file the protest (the coach may intervene in 4,3, and 2 and under competition). The protest must be written on the score sheet by the scorer or official and must be signed by both the floor captain and the referee. The protested game will be completed but before the next game begins the protest must be ruled upon by the site director. A protest cannot be filed after the completion of the match. JUDGEMENT CALLS MAY NOT BE PROTESTED. VII. FACILITY REGULATIONS This season ALL facilities will follow the same rules. Please note that these are facility rules. A facility encompasses the entire building hallways, foyers, gym proper, and any rooms (unless otherwise specified). The Tournament Director will make exceptions in case of inclement weather. The SCVA asks your help in informing all parents, your players, and interested parties regarding these facility rules. If these rules are not adhered to, we will lose the use of the gyms. (Please emphasize that NO food, drink, or beach chairs are allowed.) A. Chairs NO chairs of any kind will be allowed INSIDE any facility. They may not be placed on blankets, towels, or newspapers. The use of beach chairs is prohibited by campus administration. B. Coolers NO coolers will be allowed inside any facility unless the Site Director sets a specific area aside. C. Plastic squeeze water bottles Plastic squeeze bottles with pushpull tops filled only with water will be allowed courtside. Glass Containers of any kind are strictly prohibited. D. Food and drink NO food or drink will be allowed inside any facility unless the Site Director sets a specified area aside. Any athlete SEEN with food or drink will not be allowed to participate in the next game. If any person associated with a club (i.e., parent, coach, andor other interested party) is SEEN with food or drink in the gym all team andor club members will remain until the conclusion of the tournament on the same day of the infraction to thoroughly clean the facility. If a team, their members, or supporters introduces andor consumes alcoholic beverages in or about the playing site, the penalty will be immediate disqualification. E. Animals No animals are allowed in any facility or any school campus, with the exception of service dogs. F. Skateboards and Rollerblades There is NO skateboarding or rollerblading in any facility or on any school campus, including parking lots. G. Barbecues NO barbecues that use briquettes, gas, or propane will be allowed on any campus. ** All water bottles must be removed from the court at the conclusion of your match. Water bottles left after a match may be cause for your team s loss of privilege to have water courtside. The gym and outside eating and lounging areas must remain clean and picked up throughout the tournament. Trash bags will be provided if necessary. Failure to comply with this will result in your teams cleaning the facility. 5

COMMUNICABLE DISEASE PRECAUTIONS when a player is bleeding, has an open wound or has blood on hisher uniform, the game shall be stopped immediately. The player must leave the court and shall not return until the proper procedure is followed. The competition area and equipment must be completely cleaned and disinfected before play resumes.. Routine use of glove or other precautions to prevent skin an mucousmembrane exposure when contact with blood or other body fluids is anticipated. 2. Immediately wash hands and other skin surfaces if contaminated (in contact) with blood or other body fluids. Wash hands immediately after removing gloves. 3. The bloodied portion of the uniform must be properly disinfected or the uniform changed before the athlete may continue participation. 4. Clean all blood contaminated surfaces and equipment with a solution made from a 0 dilution of household bleach or other disinfectant before competition resumes. 5. It is designed to respond specifically to the inherent hazards of the sport. Offered as part of the sanctioning program, it is one of the most cost effective policies available. A $,000,000 limit of liability for bodily injury and property damage loss is provided with additional limits available on an excess basis. Coverage is provided for officials, volunteers, coaches, trainers, sponsors and registered participants. The policy covers liability from pre-event setup, the event itself and postevent activities at sanctioned events. The policy will respond to claims from spectators, participants and the public in general. The USA Volleyball registration requirement is a condition of the liability policy and a common practice among sports federations. The general liability policy provides an extension of coverage to include non-owned physical damage to volleyball related sports equipment which is under your care custody and control. A REVIEW OF GENERAL LIABILITY COVERAGE Commercial General Liability insurance provides coverage for claims of bodily injury or property damage made against the insured for which they become legally liable. The insurance company will pay on behalf of USA Volleyball and other named insureds, claims which the Insureds shall become legally obligated to pay as damages because of bodily injury or property damage to which the insurance applies, caused by an occurrence during the policy period, up to the policy limit. The General Liability policy for USA Volleyball is an occurrence policy. A claim under this policy shall be considered as being first made at the earliest of the following times (a) When USA Volleyball first notifies the Insurance Company in writing that a claim has been made; or (b) When USA Volleyball first notifies the Company in writing that a suit has been brought; or (c) When USA Volleyball first notifies the Company in writing of specific circumstances, which may result in a claim being made or suit being brought. All claims for damages because of bodily injury to the same person, including damages claimed by any person or organization for care, loss of service, or death resulting at any time from the bodily injury, will be deemed to have been made at the time the first of those claims is made. (b) Actual, alleged or threatened discharge, dispersal, release or escape of pollutants. (c) Loss due to war, whether or not declared, civil war, insurrection, rebellion or revolution. (d) To any obligation for which USA Volleyball may be held liable under any workers compensation, unemployment compensation or disability benefits law. (e) To bodily injury to any employee of USA Volleyball arising out of and in the course of their employment or to any obligation of USA Volleyball to indemnify another because of damages arising out of such injury. (f) To loss arising out of asbestos (g) To loss arising out of employment related practices (h) Claims or actions brought by one player against another player (i) Intentional Acts Bodily injury or property damage expected or intended from the standpoint of the insured. (j) Bodily injury or property damage for which any insured may be held liable by reason of () causing or contributing to the intoxication of a person (2) the furnishing of alcoholic beverages to a person under the legal drinking age or under the influence of alcohol or any statue, ordinance or regulation relating to the sale, distribution or use of alcohol beverages. BROADENED COVERAGE The General Liability policy has been broadened to include the following coverage (a) Contractual Liability Covers oral and written contracts or agreements relating to the conduct of USA Volleyball s business. (b) Personal Injury and Advertising Injury Liability Covers USA Volleyball s legal obligations for injury to others arising from () False arrest, detention, imprisonment or malicious prosecution (2) Libel, slander, defamation or violation of right of privacy andor (3) Wrongful entry or eviction or invasion of right of private occupancy (c) Incidental Malpractice Liability Covers USA Volleyball, Employees and Volunteers for legal liability arising out of rendering or failure to render certain professional health care services. Medical providers must be volunteers without compensation. This coverage is limited to the terms and conditions of the actual policy. (d) Host Liquor Liability Covers against loss arising out of the giving or serving of alcoholic beverages at functions incidental to USA Volleyball s normal operations. (e) Premises Damage Legal Liability - $,000,000 for property damage to premises insured that USA Volleyball rents from others, or premises temporarily occupied by the named insured. This coverage is excess insurance only over any part of any other insurance that provides coverage for property damage to said premises. (f) Non-owned Watercraft (up to 58 feet) Covers loss arising out of the use of nonowned watercraft by USA Volleyball. (g) Limited worldwide liability coverage for bodily injury, property damage, personal injury and advertising injury liability subject to the terms and conditions of the actual policy. (h) Additional Persons Insured Broadens the Named Insured to include any employees of USA Volleyball while acting within the scope of their duties. (i) Extended Bodily Injury coverage provides coverage for loss resulting from intentional acts resulting in bodily injury if the use of reasonable force is used to protect persons or property. SPORT ACCIDENT EXCESS MEDICAL INSURANCE COVERAGE The Sport Accident Excess Medical insurance program provides participant coverage for loss resulting directly from members competing in an approved or sanctioned event. Coverage does not include loss from pre-existing conditions or competing in non-sanctioned events. The coverage extends from the start, through the completion of the event, including direct designated group travel to and from the event. The Medical policy provides up to $25,000 of excess accidental medical coverage for expenses incurred within one year of the date of the accident. Written proof of loss by the Insured is required within 90 days or as soon as is reasonably possible. Proof must, however, be furnished no later than 2 months from the time it is otherwise required, except in the absence of legal capacity. The policy provides coverage against loss in excess of coverage provided under other valid and collectible medical insurance and is subject to a $250 per claim deductible. If no other collectible medical insurance is available, the loss is subject to a $,000 deductible. If injury to the member athlete requires treatment by a legally qualified physician or confinement in a legally constituted hospital, or employment of a trained nurse, x-ray, or ambulance services, and if the first expense of such treatment is incurred within 90 days of the date of the accident, the insurance company will pay the usual and customary expense incurred up to $25,000, subject to the appropriate deductible and any other collectible insurance. DEFINITION OF PARTICIPANT Athletes, coaches, trainers, volunteers, committee members, officials and any other persons who are functioning on behalf of andor while participating in any event sanctioned or approved by USA Volleyball. DEFINITION OF PARTICIPATING Participating includes pre-event and post event activities which are officially approved or sanctioned events. Pre-event activities can include but are not limited to clinics, event dinners and pre-event practicing. Post event activities can include but are not limited to award banquets, award ceremonies and clinics that occur within one day after the event. Coverage is also included for non-sanctioned volleyball related activities for certified officials who meet extended coverage criteria. ACKNOWLEDGEMENT WAIVER AND RELEASE FROM LIABILITY As with most sports activities, a signed Acknowledgement Waiver and Release from Liability (AWRL) form is required from all participants and from parents or guardians in the case of minors. This requirement exists in virtually every sport. It serves to document that the participants or parents of participating minors have acknowledged the inherent risk and danger associated with participating in sporting events. It is intended to serve as appreciable warning of these risks and the participants by signing the waiver, are giving their informed consent to the acceptance of those risks. It is important to remember that a signed waiver DOES NOT reduce the need for insurance or effective safety practices. A signed waiver is USAV s first line of defense against a cause of action for negligence and is a very effective risk management tool.
