PREMIER ROLLER HOCKEY at HOT SKATE 14 MERRICK ROAD LYNBROOK 11563 516-593-1424 INTERNET VERSION 4/18 TEAM REGISTRATION INFORMATION GUIDE Coach: Presently the only age division we off league play is Adult. Generally the minimum age to play in the Adult League is 17 ( a parents written permission/signed waiver) must be provided. A younger age may be accepted with league approval. The enclosed team registration packet includes the following items: Team Registration Information Guide' (this cover letter) Team Registration Form Team Roster Form Hockey Waiver & Acknowledgment (make duplicate copies for each player) 'General League Rules' / 'Facility Rules and Conduct Rosters may not exceed 15 players and rosters are frozen prior to the drop of the puck on the third game. During the regular season, if your goalie is unable to attend your game another goalie currently playing in the league may substitute or you may suit up a player from your team. During the playoffs only a rostered goalie/player on your team may play goalie. Generally there are no coach s in the adult division (besides player/coach). If one is wanted by team, that coach must be over age 21, on the team roster and have a waiver on file. League supplied Game jerseys will be given out by your first game. Choice of style, numbering or color is not possible. You may however, supply your own jerseys. All jerseys must have 'large' numbers on the back for identification purposes. Players must keep the same number throughout the season. No duplicate numbers. To fully register your team you must submit a fully completed 'Team Registration Form', 'Team Roster Form', 'Hockey Waiver & Acknowledgment' Forms for each player on the roster and the Team Registration Fee of $2,600 (Teams re-registering from the prior season pay $2,200). There are Waivers for each player to read & sign. Full payment must accompany the completed forms. Payment may be made by cash or check. Checks must be made out to PREMIER ROLLER HOCKEY at HOT SKATES. Registration is on a first come first serve basis. Space is limited. The enclosed 'Facility Rules & Conduct' / 'General League Rules' sheets briefly highlight rink/league policies and acceptable conduct. Please give each rostered player a copy (make copies) of these sheets so they are aware of these rules. The league bases it s rules according to the Official Rules of Premier Roller Hockey at Hot Skates. These rules are posted conspicuously in the league office and given to each coach upon registration. We made every effort to address both the 'common' and 'not so common' issues that may arise during hockey activities. If unaddressed issues emerge we will make every effort to address those situations in a 'fair' manner. We are looking forward to a successful season. GOOD LUCK!

SUMMER 2018 TEAM REGISTRATION TIMELINE NO REGISTRATIONS OR ROSTERS WILL BE ACCEPTED VIA EMAIL (no exceptions). REGISTRATION MAY ONLY BE MADE IN PERSON, OR VIA MAIL. SUCH REGISTRATION MUST BE PAID IN ACCORDANCE WITH THE TIMELINE & TERMS SHOWN BELOW. WEDNESDAY MAY 16 TH 2018 Last Date for on-time Registration fee price of $2,200 for renewals and $2,600 non-renewals. This payment may be made either by cash or checks or a combination of both. A team is considered a renewal if they played in the SPRING 2018 league. Their roster would have to be at least 75% the same as the SPRING 2018 seasons. Must be fully paid with the $2,200/$2,600 fee by this date to receive that pricing. After this date fees increase to $2,300/$2,700 respectively. WEDNESDAY MAY 23 rd 2018 Last and final date for LATE REGISTRATION for SUMMER 2018 league. TEAMS MUST BE FULLY PAID BY THIS DATE. IF A TEAM HAD NOT BEEN FULLY PAID AS OF 5/16/18 THEN THE $100 LATE FEE PAYMENT IS IN EFFECT WITH FINAL PAYMENT DUE 5/23/18. NO EXCEPTIONS! We expect the schedule to be posted on Wednesday evening May 30 th and the season to begin on Monday June 4 th. If teams are providing their own jerseys each player must have their own unique number (not zero or blank) and the same color. Otherwise team must use Hot Skates provided jerseys. PARTIAL TEAM REGISTRATION If you cannot commit to a team registration and the financial commitments required, then your option is sign-up as a group of individuals who are requesting to be on the same team. Each player would be considered an individual registrant and the deadline for that registration is WEDNESDAY MAY 23 RD 2018. INDIVIDUAL REGISTRATION FEE IS $230. We will honor your request as best we can to be placed together. In this scenario any grouping of individuals will be subject to the league forming the team around that core group. The league will have control of how many players on the roster, what division and at the league discretion will honor (or not honor) and scheduling requests by the group. Unless guaranteed by the management upon registration, any request of schedule preferences should be assumed not to be honored, with the hopes of the accommodation.
