Signed Application 2. Signed Expectation Statement & Code of Conduct 3. Signed Rules 4. Waiver of Liability 5. 8x10 color photo (headshot-shoulders up and only you in the photo in western attire) 6. 8x10 color photo (on horseback in western attire) 7. Brief biography, between 150 and 175 words, typed. Bio will be used for press releases, programs and other publicity. Please include, background, family, education and goals, interests and hobbies. 8. Copy of Driver s license or birth certificate Important Information: Date of all competition will be on 03/10/2018 There will be a horsemanship pattern and interviews. Location: Outlaws in Cave Creek The horse you compete on should be the same horse you use throughout the year. There will be a dinner/coronation to showcase your speech, modeling and extemporaneous questions. Our Queen s reign is from March 24, 2018, to the end of the first performance of the CCRD Rodeo Days Rodeo, 2019. All CCRD Queen Rules and protocols should be adhered at all times time. Qualifications: 1. Queen contestant must be 17 years of age and not older than 23. 2. Contestant must not be or ever have been married, had a marriage annulled, or is now or ever have been pregnant (getting married or becoming pregnant during reign forfeits the title). 3. Shall be an active student or high school graduate or equivalent.

4. Shall not be a titleholder of any other pageant during your reign and may not run for any other title with the exception of Miss Rodeo Arizona, Miss Teen Rodeo Arizona, or Miss Turquoise Circuit until completion of your reign. 5. If chosen, a previously held rodeo queen title which has not expired prior to March 1, 2018, will be forfeited. 6. Shall not have been convicted of a felony crime. 7. Cave Creek Rodeo Days Royalty Expectation Statement & Code of Conduct I understand that if I am awarded the title of Cave Creek Rodeo Days Queen, I will be a representative of Cave Creek Rodeo Days (CCRD) which is produced by the Cave Creek Rodeo Days Organization (CCRD). I am responsible to represent the sport of rodeo in a professional manner. I cannot be the queen for any other organization or entity once I accept this title. I will give up all other titles that have not ended prior to February 1 of the reign period. I will strive to represent unparalleled sportsmanship with high moral and ethical character. This title recognizes a number of attributes: sportsmanship, personality, public speaking and appearance. Therefore, it is my responsibility, as Cave Creek Rodeo Days representative to act, speak, and dress according to the presence my title requires any time I am in the public eye. Riding and horsemanship skills are essential to the promotion of the sport of rodeo. Therefore, I will strive to improve my riding and horsemanship skills in order to better promote the sport. I understand that I am responsible to provide my own horse that is capable of performing queen activities during rodeo activities and events. If I become the Cave Creek Rodeo Days Queen, I Will: Wear my crown and sash any time I am acting in the capacity as Cave Creek Rodeo Days Queen and only with the approval of Queen Committee Chair. Make myself available for all opportunities to appear as Cave Creek Rodeo Days Queen to the public. Attend specific events as requested by the rodeo committee (Exhibit A shall have mandatory appearances I agree to be available for). Queen Committee Chair must approve all official appearances. Understand it is my financial responsibility to attend all required appearances. Cave Creek Rodeo Days will reimburse fuel for travel outside the Phoenix metropolitan area for approved events. Be on time. No one should have to wait on you. Prepare for the next event so you are ready. Do my homework on directions, layout, schedules and requirements for each event. Know what to bring, wear and pack. Know where to check in, park, overnight stay, etc. Follow through on my responsibilities through the end of my reign. Not drink, smoke, use profanity, or chew gum in public while acting in the capacity as the Cave Creek Rodeo Days Queen. Not hang out in bars and/or parties at a rodeo or event while acting in the capacity as the Cave Creek Rodeo Days Queen. Never publicly complain - while visiting I will remain positive and professional. Work with the rodeo committees on whatever promotions they may desire. Not make deals with vendors or potential vendors for your personal use, benefit, or for your relatives. Not use cell phones in public, check messages, or keep phones in pockets or public view while wearing the crown and banner. Remember the last thing you do is what they will remember. First impressions are important and last impressions are forever. At no time during the term of my contract shall I post suggestive photos or use profanity on social media (no photos or discussions about boyfriends, no photos or discussions about parties or alcohol). I will never appear inappropriate on social media (Facebook, Twitter, Snapchat, Instagram or Other) whether it be a personal page or account or the intellectual property of Cave Creek Rodeo Days.

At all times keep posts professional in verbiage, with no language or images that is unbecoming to, or in any way compromise the professional image of Cave Creek Rodeo Days Queen or the professional brand of the Cave Creek Rodeo Days Organization. Dress Code Requirements: Remember, you re invited to bring color and sparkle to an event. Cave Creek Rodeo Days Queen must adhere to PRCA arena dress codes in the rodeo arena. Dress or skirt no shorter than 2 above knees. No visible tattoos or facial piercings. No sleeveless or tank tops. No cleavage, no braless attire, no open back or bare midriff. At no time should bra straps be visible. Clothing should reflect a professional western image. No evening gowns or cocktail type dresses. No burn out, transparent material, or see through lace on body portion of top. Hair is to be styled fashionably. Pony tails and or braids may be periodic acceptable alternatives, however, hair must still be styled, (some curl wings ). Hats: keep a brush for each color. Keep them clean and well-shaped. Boots: keep clean and free of mud. Banners: clean and pressed. Carry pins for the banner at all times. Jeans: clean and preferably ironed. Shirt: Flashy for the arena. You need to be set apart from the rest. Always wear the shirt color for the rodeo theme of the performance when applicable (e.g., pink for Pink Night). Pay attention to your fingernails and makeup, especially lipstick. After changing into non-rodeo attire continue to follow professional, conservative clothing and attire. Other: Your vehicle, trailer, horse, etc. need to be clean and well-kept. They represent you and you represent our PRCA rodeo. If you stay at someone s home or board your horse there for an event, leave it as you found it (stalls cleaned). They are not your maids and we want to be welcomed back. Send thank you notes after visiting to those who have helped you during your stay. (Rodeo Committee persons whom you have worked with directly (gate/flag persons, stock contractor, hostess when you stay with someone or who have bought a meal or any courtesy extended to you.) Understand any costs associated with overnight stays if you are not self-contained. Cave Creek Rodeo Days does not reimburse hotel rooms, only fuel. Signature Date

Cave Creek Queen Rules 1) Applicant, if chosen as Queen, must sign a contract between herself and Cave Creek Rodeo Days, waiver of liability and expectation statement. 2) Applicant/Queen understands this is a volunteer, unpaid position with the Cave Creek Rodeo Days Organization. 3) Applicant/Queen must conduct herself in a professional manner; portray high moral standards, and good sportsmanship. 4) Applicant/Queen is expected to be supportive and respectful of one another. You are being watched at all times. 5) Applicant/Queen will be responsible to ensure the ability and health of horse to travel rodeo road and for the safety/care of equipment. 6) Applicant/Queen will provide their own transportation to and from all activities. 7) Applicant/Queen is required to have a suitable horse for rodeos and parades. 8) Applicant/Queen must be aware of and conform to PRCA dress code (long-sleeved shirts, western jeans, hat, and boots). 9) Applicant/Queen is not permitted at any time to drink alcoholic beverages, smoke, use drugs, or use foul language. 10) Applicant/Queen is required to dress in western attire when representing Cave Creek Rodeo Days in any capacity. 11) Any problem that may arise should be quickly directed to the Queen Committee Chair or the Board of Directors. 12) Parents, friends, and relatives are welcome to attend all events. Appropriate language and respect of others will be displayed at all times throughout the reign. 13) If you are a reigning queen for another organization, you must relinquish that title prior to February 1. If not, you will forfeit your Cave Creek Rodeo Days Queen crown and all benefits derived thereof. 14) The Cave Creek Rodeo Days chaps and crown are property of the Cave Creek Rodeo Days and must be returned at the end of or termination of service. These and all other related items are to be kept in good condition as it will be passed down to future royalty. Any damage to Cave Creek Rodeo Days property or equipment may require reimbursement by Queen other than normal wear and tear. If the representative fails to return the Cave Creek Rodeo Days chaps or crown, she agrees to pay $1,250.00 to the Cave Creek Rodeo Days organization. 15) While traveling to events, curfew is midnight in order to be presentable for activities. 16) Royalty s boyfriend/significant other (or what appears to be) is not permissible to take royalty to required events or interact with Royalty at events even when not in crown and banner, people still recognize you. 17) Any possible advertising or sponsorship opportunities must be reviewed and approved by the Cave Creek Rodeo Days Committee Chair of Marketing in order to prevent any conflicts with existing Cave Creek Rodeo Days or PRCA sponsors. 18) Unauthorized use of any Cave Creek Rodeo Days or Cave Creek Rodeo Days logos, trademarks or trade names may result in dismissal. Please be prepared for additional media /PR dates (radio, TV, etc.) Mandatory appearances leading up to the 2018 Cave Creek Rodeo Days also require distribution of marketing materials such as event schedule flyers. It is your responsibility to promote Cave Creek Rodeo Days. Fuel reimbursement (Google maps mileage report required) and day expense of $40.00* for meals with required receipts and completed expense report as follows: Yuma, Payson (August), Flagstaff, Prescott, Kingman & TCF. Fuel shall not exceed the federally approved standard 2017 mileage rate of 53.5 cents per mile.

Waiver of Liability In consideration of my application being accepted for participation in the Miss Cave Creek Rodeo Days Queen contest, I hereby agree to abide by the rules and regulations of the Cave Creek Rodeo Days Organization and the Miss Cave Creek Rodeo Days Queen committee and understand that any violation of the rules and regulations may disqualify me from the contest. I further agree to hold harmless the Miss Cave Creek Rodeo Days Queen contest committee, the Cave Creek Rodeo Days Organization and its officer s agents, members, employees, volunteers for any claims resulting from the loss, accident or injury, received by me in connection with my participation in any event, performance, parade or other activities. Also, I hereby release the right, title and claims of any text, images or photographs that may be taken and/or used relating to the Cave Creek Rodeo Days and the queen contest for their use in promotion and publicity. I certify that I have read the rules and regulation of the Miss Cave Creek Rodeo Days Queen contest. I agree to fulfill my obligation(s) as Miss Cave Creek Rodeo Days conducting myself in the proper manner and/or way of life by remaining unmarried and free of any questionable acts involving an issue of morality. I agree to return any and all items received in relationship to the title of Miss Cave Creek Rodeo Days including sash, crown, chaps, gifts and belt buckle if at any time it is determined I cannot fulfill my duties and obligations as Miss Cave Creek Rodeo Days.
