I undertake to notify Panorama Football Club in writing of any change regarding name or address, failing which I undertake to pay all tracing charges incurred by Panorama Sports Club or their Attorneys. I undertake to pay all costs disbursed by Panorama Football Club to its Attorneys in securing my compliance with the provisions of this Agreement on the Attorney and Client scale which shall include costs of all attendances, tracings and collection commission charged whether action has been instituted or not. Code of Conduct Panorama Sports Club, with input from its coaches and members have drawn up a code of conduct, which we expect to be followed by everyone attending our grounds, and our teams and members when here or visiting other grounds as representatives of Panorama Sports Club. Please sign the section and return a copy to the Club. FOR PLAYERS Play the game because you want to and because you enjoy it. Play the game by the rules, which govern the sport. Learn the rules governing the game. Abide by the referee s rules, not your own or fellow players and spectators. Respect your opposition and play to win, fairly. Control your temper and language on the field and on the side lines. Follow the instructions of the coach and not the spectators/parents. Attend all matches and practices where you are requested to do so at the proper times. Contact the coach in advance if you cant attend a match or training session. Proudly represent your Club, irrespective of which level selected. Do not criticise or abuse fellow players - remember, you re on the same side. Abide by the decisions of the coach without descent. Support the Club to the best of your ability. Help to maintain and look after the Club s facilities at all times. Respect other Clubs both their rules and regulations. Be proud to play for PANORAMA SPORTS CLUB. Make sure all fees and obligations are up to date. This code applies to all those involved in the Panorama Sports Club, football section under the auspices of The Football Association. Football is the national game. All those involved with the game at every level and whether as a player, match official, coach, owner, of administrator, have a responsibility, above and beyond compliance with the law, to at according to the highest standards of integrity, and to ensure that the reputation of the game is, and remains, high. FOR COACHES: To teach and instruct your sport to the best of your ability. To instil amongst your players fair play. To teach the rules that governs the game. To teach your players to play within the laws of the game. To promote the fun aspect of playing the game. Never shout, argue or swear at your team or the opposition. Refrain from sarcasm and obscenities at the officials and visiting teams. Be prompt when attending practice and games. Be unbiased when selecting or coaching your players. Help other coaches to further the instruction of the sport. Keep the players and parents informed of your plans and of the players/team progress. Coach within the guidelines laid down by the Club. Abide by the rules and regulations of other Clubs. Encourage your team to respect the opposition whether you win or lose. Community Football, at all levels, is a vital part of a community. Football will take into account community feeling when making decisions. FOR PARENTS: Never force your child to play a sport. Endeavour to praise effort and performance more than the result. Praise good behaviour quickly to show that you value it. Assist the coach with discipline when concerning your child. Help your child to understand the rules and how to play by them. Control your temper and language on the side lines. Show respect for the opposition as well as your own team applaud both for good play. Refrain from verbal or physical abuse of the match officials and the opposition. STAY OFF THE PITCH - during games and practices. Do not coach from the sidelines or shout at players if they make mistakes. Do not criticise the coach - rather speak to him/her direct or put it in writing. Enjoy watching the game regardless of the result. Support the Club whenever possible with all its functions. Help the players to arrive on time for all practices and games. Abide by the decisions of the coach and the Club concerning the different sports. Help to maintain and improve the Club and its facilities. Make sure all fees and obligations are up to date.

A Section of Panorama Sports Club INDEMNITY I being the member / parent / guardian of hereby request/give permission for him/her to participate in Football and Extra Curricular activities of Panorama Sports Club (hereafter referred to as "PSC"). I accept that all reasonable precautions will be taken to ensure the safety and welfare of myself and/or my child and my/his/her belongings. I hereby undertake not to make any claims against PSC, its officials or agents and hereby indemnify all of the foregoing against and absolve them absolutely from any claims, costs and expenses of whatsoever nature I may have in respect of any loss, damage, injury or death to persons and to property arising out of or in connection with the above mentioned activities. I hereby accept that I shall be responsible for payment of all medical and/or hospital accounts, where applicable, should injury be sustained by my child/myself and hereby cede all powers as parent/guardian/member to the Chairman of PSC or her/his representative should medical treatment/surgery be deemed necessary for my child/myself out of activities of PSC.
