List any changes in Pet Shop - Retail Sales Business Name, Address, Owner, etc.? If your business was sold or transferred last year, you must supply this office with an Annual Report including all information regarding the disposition of all regulated wildlife. New business or pet shop owners and/or any new business locations must obtain individual commercial permits. N.J.A.C. 7:25-4.11(a) states "Any person who transfers possession, as distinguished from ownership, or location of any animal for which a permit has been issued, shall, within 48 hours, report to the Division of Fish and Wildlife exactly which animals are transferred and the name and address of the person to whom the animals were transferred." IMPORTANT NOTE: Immediately upon "Transfer" or sale of ownership to a new owner, the new owner is required to obtain their own Pet Shop - Retail Sales Permit. The Wildlife Permits Unit does not transfer your Pet Shop - Retail Sales permit. If the mailing address or actual location of the store changes, you must report updated information to the Wildlife Permits Unit within 48 hours of any change. Attach Original White Copies of the EXOTIC and NONGAME WILDLIFE SALES RECEIPTS - TEMPORARY 20-DAY PERMITS completed for each regulated wildlife sale made during the past year to this renewal application. IMPORTANT NOTE: Pet Shop - Retail Sales must submit these copies annually with your renewal application. These receipts must be completely filled out, legible with the buyer's full name, complete address, city, state and zip code or the Pet Shop - Retail Sales owner will be subject to penalties, fines and other law enforcement ramifications. On this Renewal - Annual Report - List the common names and total numbers of each regulated Exotic and Nongame Species you bought or sold during the past year. List the source of the animals, including the complete Name and Mailing Address of each Wholesaler and Consignment seller. It is your responsibility to obtain regulated wildlife from legal Sources of Acquisition or you are in violation of New Jersey regulations and are subject to penalties or fines. (Attach additional pages as necessary). List Regulated wildlife species and total number of each that were returned to dealers, returned to consignment sellers, donated, stolen, escaped died, etc. Specify what happened to each animal. (Attach additional pages as necessary). LIST SPECIFIC SPECIES OF REGULATED WILDLIFE YOU WANT TO HAVE ADDED OR DELETED ON THIS YEARS PERMIT. Example: BIRDS, REPTILES, AMPHIBIANS. FOR MAMMALS SPECIFY TYPE: CHINCHILLAS, FERRETS, ETC. DO NOT LIST EXEMPT SPECIES. PLEASE SPECIFY ADDITION OR DELETION. If you want to add regulated wildlife to your permit, please provide the following information: Source of Acquisition including name and address, species, diet, experience with species and caging facilities for each type of animal. Please note that the USDA requires permits for the sales of "POCKET PETS" including but not limited to the following: HEDGEHOGS, DEGUS, SUGAR GLIDERS and JERBOAS. It is your responsibility to contact the USDA to obtain the correct permit for the sales of "Pocket Pets" For more information contact their office at (919) 855-7100. If you sell any "Pocket Pets" without the correct USDA permit you are in violation of Federal regulations. All New Jersey Exotic and Nongame regulated wildlife possession permits expire December 31 of the year of issue (unless otherwise noted on your permit). It is your responsibility to renew your permit for sales purposes in New Jersey. If you do not need a permit for the current year, you must still complete the Renewal Application - Annual Report and return to the Wildlife Permits Unit. If you do not need a permit for the current year, DO NOT SUBMIT A FEE. 2

This RENEWAL APPLICATION - ANNUAL REPORT, MUST BE COMPLETED AND RETURNED to our office with the correct fee by January 31. Do not submit this renewal - annual report to our office via fax or e-mail. Our office must receive the correct renewal fee with the renewal application - annual report to process. Attach additional sheets as necessary. Your permit cannot be issued until your application is received with correct fee and reviewed by our staff. Failure to return your Renewal Application - Annual Report by the deadline - January 31 is in direct violation of the New Jersey Division of Fish and Wildlife, Wildlife Permits Unit Regulations, which could result in fines up to $5,000.00 per violation, penalties, possible non-renewal of your permit and other law enforcement ramifications. The Renewal Application - Annual Report MUST be correct and complete. All regulated wildlife in your possession for the previous year must be accounted for. You must also supply any other requested information necessary under state regulations. (Attach additional pages as necessary). N.J.A.C. 7:25-4.6(e) regulation is as follows: "Pet Shops and Animal Dealers shall submit to the Division an annual inventory of acquisitions, sales and exchanges upon the expiration or renewal of their permits". It is your responsibility as a New Jersey commercial permittee to obtain regulated wildlife species from legal sources. If your source of acquisition is "illegal" you are in violation of N.J.A.C. 7:25-4.7 (a3). You may be subject to penalty or fines if you violate this regulation. It is illegal to sell regulated wildlife species under an Individual Hobby Exotic and Nongame possession permit. NO Pet Shop or Animal Dealer may possess or sell ANY species of regulated wildlife that is NOT listed on your permit. All regulated wildlife permit amendment requests must be in writing and contain sufficient information (source of acquisition, species, caging facilities, diet, experience with species, etc.) to allow our staff to evaluate the request. POSSESSION OF ANY WILDLIFE on NEW JERSEY'S POTENTIALLY DANGEROUS and/or ENDANGERED SPECIES LISTS IS STRICTLY PROHIBITED Potentially Dangerous Species as listed in N.J.A.C. 7:25-4.8 may not be possessed or sold by authority of this permit. Potentially dangerous exotic birds include, but are not limited to the following Parrots: Alexandrine; Blossom-Headed; Blyth's; Derbyan; Emerald-Collared; Intermediate; Long-Tailed; Malabar; Mauritius; Monk (aka: Quaker & Grey Breasted); Moustached; Newton's; Plum-Headed; Ring-Necked; Rose-Ringed; Seychelles; Slaty-Headed; and the Patagonian Conure and all others in the Psittacula genus. Endangered Species as defined by N.J.A.C. 7:25-4.13 may not be possessed or sold by authority of this permit, [SEE REGULATIONS]. The only Corn Snake color morphs that are legal to possess and/or sell in New Jersey are the following: Red Albino (Amelanistic), Butter, Blizzard, Candy Cane, Creamsicle, Lavender, Pearl, Snow and Sun Glow. The Center for Disease Control (CDC) has issued a health warning restricting the possession and sales including but not limited to the following species: Dormice, Gambian Giant Pouched Rats, Pygmy Mice, Spiny Mice and Zebra Mice. Also Note: Due to the possibility of Monkey Pox a health warning restriction has been announced for the following species: Prairie Dog; and six types of African Rodents: Tree Squirrels, Rope Squirrels, Dormice, Gambian Giant Pouched Rats, Brush-Tailed Porcupines and Striped Mice are not allowed to be possessed or sold. Effective immediately under the authority of the New Jersey Division of Fish and Wildlife: Captive Game and Exotic and Nongame regulations, THERE IS A BAN UNTIL FURTHER NOTICE ON THE IMPORTATION OF WHITE-TAILED DEER, BLACK-TAILED DEER, MULE DEER, RED DEER, SIKA DEER, MUNTJAC, REINDEER, ELK AND MOOSE INTO THE STATE OF NEW JERSEY. This is due to the growing concern of the spread of CHRONIC WASTING DISEASE into wild and farmed herds of animals in the deer family throughout the United States. It is the responsibility of the permittee to act in accordance with state, federal, worldwide health organization and other regulatory agencies which mandates any current applicable circumstance, restriction, quarantine, warning, announcement, etc. Ban of regulated wildlife species for possession or sale are imposed due to the possibility of health risk. 3

Bird band and/or microchip numbers must also be listed under your current inventory. INVENTORY CONTINUED List below the INVENTORY of REGULATED EXOTIC and NONGAME SPECIES in your store as of December 31 FOR THE PREVIOUS YEAR ISSUED PERMIT. For regulated birds, the band number and/or microchip number MUST BE INCLUDED. (Attach additional pages as necessary) 9

IMPORTANT: Enclose copies of all of your Exotic and Nongame previous year issued invoices - receipts for your regulated wildlife. Regulated bird invoices - receipts must document that they are either CAPTIVE - DOMESTICALLY bred or were legally in captivity prior to the Wild Bird Act, December 10, 1991. If they do not, contact your wholesaler - source of acquisition for a statement on their letterhead or equivalent. This documentation must be submitted to the Wildlife Permits Unit with this renewal application - annual report. If your application is received without this important information, it WILL NOT be processed until these items are received. PET SHOP - RETAIL SALES IMPORTANT INFORMATION NO Pet Shop or Animal Dealer may possess or sell ANY species of animal that is NOT listed on their permit. Pet Shop and Animal Dealer Permits DO NOT authorize the sale, possession, or distribution of BATS. Any violation of possession or sales of the following could result in fines, penalties and/or revocation of your permit. The BENGAL MONITOR (Varanus bengalensis) is listed as an ENDANGERED SPECIES under the FEDERAL ENDANGERED SPECIES ACT, Department of the Interior and CANNOT BE SOLD, POSSESSED, DISTRIBUTED, ETC. THE FOLLOWING IS A PARTIAL LIST OF VENOMOUS, REAR-FANGED SPECIES WHICH MAY NOT BE POSSESSED UNDER A PERMIT SPECIFYING NONVENOMOUS REPTILES : Mangrove Snake Parrot Snake Asiatic Dogtooth Cat Snake Boomslang Indian Cat Snake African Krait Snake VENOMOID (DEVENOMIZED) SNAKES are considered POTENTIALLY DANGEROUS SPECIES [N.J.A.C. 7:25-4.8(a)], and are not allowed to be possessed or sold in New Jersey. ONLY BOA CONSTRICTORS WITH THE GENUS AND SPECIES (Boa constrictor constrictor) ARE EXEMPT, BOA CONSTRICTORS WITH THE GENUS AND SPECIES (Boa constrictor imperitor) or (Boa constrictor ortonii) ARE REGULATED AND REQUIRE A PERMIT. GREEN & YELLOW ANACONDAS have been determined to be a potentially dangerous species, N.J.A.C. 7:25-4.8(a), and are no longer allowed to be possessed or sold in New Jersey. The New Jersey Department of Health mandates: Turtles and Tortoises CAN NOT be sold in New Jersey. Important note: if the turtles and tortoises are not on any New Jersey or Federal Endangered Species List and if they Source of Acquisition is Legally Acquired (ie: not "captured or taken from the wild), they may be possessed in New Jersey with the proper Exotic and Nongame Possession Permit. Due to difficulties in determining morphological differences among several species and the fact that these difficulties have already produced problems, including but not limited to the following species CANNOT BE SOLD OR POSSESSED in New Jersey: Gray Tree Frogs (all species) Dino Dogs Larval Salamanders Mud Puppies Water Dogs Axolotls It is your responsibility as a commercial permittee to ascertain that the Sources of Acquisition for regulated wildlife is legal. For the sale and distribution of regulated New Jersey Exotic and Nongame wildlife species, a permit is required. Request a copy of the New Jersey Exotic and Nongame permit from your Sources of Acquisition or contact our office for more information. For regulations regarding BOARDING and/or CONSIGNMENT SALES of Regulated Exotic and Nongame birds, mammals, reptiles and amphibians, contact the Wildlife Permits Unit. The Wildlife Permits Unit must be informed a minimum of two weeks prior to any New Jersey OFF- SITE regulated wildlife sales activities including Trade Shows, Wildlife Expos, Auctions, etc. INFORMATION MUST INCLUDE DATES, TIMES, and LOCATIONS of EVENTS. Failure of notification could result in fines. IMPORTANT NOTE: If any amendment is requested (other than any New Jersey Staff clerical error) after an Exotic & Nongame permit is issued for the current year, a $5.00 amendment processing fee will be required per amendment. IMPORTANT NOTICE: In our experience, it has become clear that many commercial permittees do not know or understand our policy regarding the display of regulated exotic and nongame wildlife, particularly exotic birds, in your place of business. Rather than waiting for an inspector to inform or re-inform each place of business, the required information is supplied below. Read it carefully, as you will be required to comply. All regulated exotic and nongame wildlife species must be housed or caged in a manner that prevents any possibility of liberation, accidental injury to the animal and/or to the public, and any possibility of public contact. All regulated exotic and nongame wildlife species exhibited outside of cages must be behind a barrier or other obstruction to prevent public access. Out of cage birds must be wing-clipped, prevented from any possibility of escape and caged at night. If a potential customer wants to handle a regulated animal for sales purposes, it must be done ONLY under the direct supervision of an employee and out of direct contact with other customers. All regulated animal cages must be covered or otherwise inaccessible to the public, there can be no open tops. Remember, that it is your obligation to conduct your business operations according to the requirements of the Wildlife Permits Unit Exotic and Nongame Wildlife Permits Regulations and Policies.
