We are currently working on the erosion remediation plans for permitting and will distribute when available. Required permitting: 1.) EPC - Submittal of Miscellaneous Activity Permit. 2.) Following coordination with Rick Cabrera, 6/16/16, a permit will not be required through Development Services for this work in the wetland. 3.) Following phone call with Russell Martin, 6/22/16, this remediation work may be done as an asbuilt deviation to the approved SWFWMD permit. Contractor will stabilize the areas outside of wetland limits; regrade washout areas and sod. 2.) Per phone conversation dated 6/10/16, Bill Vorstadt met with EPC Director and coordinated with Russell Martin (SWFWMD Environmental Reviewer) and confirmed that the proposed work in the wetland conservation areas to stabilize the creek can be permitted under an EPC 5

Miscellaneous Activities Permit ($150 review fee). Bill confirmed that the proposed permanent riprap in the creek area will not require mitigation. 3.) The washout area near the pipe discharge from Pond 2 to creek is of more concern with future washouts since this area has a sharp curve in the creek. As discussed in the field, we are proposing to grade 20 ft from existing tree to remove a delta area where fill has settled and reshape the creek to provide a milder curve then provide riprap on both sides of creek around curve to stabilize. A retaining wall may be added to the lot side of creek to provide further protection in this area. 4.) The washout areas upstream and downstream of the culvert crossing will be cleaned out and restabilized with sod and rip rap. Each corner of the culvert crossing headwall from ROW will be reinforced with geoweb to prevent future erosion in this area. 5.) The creek area downstream of culvert crossing will be regraded to remove washout material and reinforced with riprap from the creek bottom to +/- 8 ft up the bank. 6.) As noted at the field meeting, the pre-development conditions for this project, prior to purchase by our Client was documented with field visit and photos with coordination through EPC and SWFWMD to permit re-grading of the creek to stabilize the creek and avoid future washouts with the development. 7.) We will coordinate with County Development Services and SWFWMD to confirm what additional actions are needed to permit the work in the wetland limits. As seen from the pictures (attached PowerPoint), the site has impacted Bullfrog Creek and lacks the proper installation of BMPs on site. I would appreciate if you could escalate this within developmental services for failure to comply. I spoke with Danny Brooks (site contractor) yesterday who said that silt fence will be installed within 2 days, however due to the current site condition, that will be insufficient. The requests for the contractor/ owner are to: Remove failed / damaged BMPs along the creek that are buried under the sediment. Properly install BMPs as to prevent further washouts (See PowerPoint for reference) o Silt fence not trenched 7

o Inadequate single row silt fence along Bullfrog Creek is unable to contain sheet flow from northwest quadrant. o Gully repair o Areas where silt fence is undercut Remove bulk sediment deposited within NW ditch and Bullfrog Creek, as well as the construction material located at Wetland Impact A-4.
