MEDICAL TREATMENT AUTHORISATION & LIABILITY RELEASE I [the undersigned] hereby agree for my son/daughter to participate in Inspire Allstars (IA) cheerleading squad / classes and all associated activities. I understand that this activity involves risk to the participant and acknowledge that, due to the nature of this activity, there is a possibility my son/daughter may sustain physical injury or illness in connection with their participation. In the event of an injury, your child will be attended by a qualified First Aider. Where an injury is more serious and requires medical treatment by a paramedic, an ambulance will be called. In order that your son/daughter may receive the necessary medical treatment in the event of sustaining injury or illness during participating in this activity, I hereby authorise the cheerleading coaches or other supervising staff to obtain medical treatment on their behalf for such an injury or illness. I hereby hold IA, its officials, and other associated venues and its representatives, harmless in the exercise of authority. All choreography, cheers and concepts used belong to IA. Choreography learnt with IA will not be used on any occasion or for any purpose other than IA performances, unless written permission from the Head Coach has been obtained prior to such an occasion or purpose. All information relating to upcoming performances; including venues, potential sponsors and any fees paid are strictly confidential and under no circumstances must any communication to an unrelated third party occur. Squad performances and appearances may by visited by members of the media, including radio, television, newspaper, and magazines. I understand that I am not to speak to the media without the presence and direction of the Programme Director/s. I grant my permission to be involved in approved media activities without remuneration. The IA name and logo will not be used for the purpose of promotion and sponsorship except by the Programme Director/s, or by those specifically instructed by that person and for the specific purpose as instructed. All uniforms remain the property of IA until squad members remunerate the full cost. It is each squad member s responsibility to ensure that the good condition of all uniforms is maintained. I will not change the appearance/style of the uniforms. All styling and concepts used with regard to appearance will be used only for performances relating to IA. ATHLETE CODE OF CONDUCT As a cheer athlete you are not only an ambassador for Inspire Allstars but also the sport of cheerleading. You are expected to conduct yourself (both in and out of your uniform) in a manner that upholds the reputation of both. Please read the information below carefully before signing and returning the slip underneath. GENERAL RULES Always be punctual Attend every training session Always have a good warm up Participate to the best of you ability Take care of your appearance - always be neat for practice and competition Always dress properly for practice No sweets, gum or snacks during training No swearing - behave appropriately Maintain a positive attitude at all times - SMILE!! Treat each other with respect - this includes other squads!!! Be kind to others - do not gossip Talk to your coach if you have any problems And most importantly, ALWAYS HAVE FUN!! UNIFORM AND PRACTICE WEAR Hair must be tied back, and no hats must be worn during practice For safety reasons YOU MUST NOT ANY WEAR JEWELLERY You must have a uniform in order to compete. Uniforms must be purchased as advised Cheerleaders in uniform or wearing any item of squad or team identification must always conduct themselves in a manner becoming to a Inspire Allstars cheerleader. CONDUCT Squad members are required to maintain and uphold the reputation of Inspire Allstars through their own conduct and squad spirit. Members are required to be courteous, polite, friendly, and try to have a smile for everybody. All cheers and chants shall be of a positive and sportsmanlike manner. Members will make every attempt to ignore or dissuade negative responses at events and not become involved in such action themselves. ANTI BULLYING POLICY Every squad member has the right to be free from intimidation, both in the squad and in the surrounding community. Inspire Allstars will not accept and will question unkind actions or remarks, even if these were not intended to hurt. Any wounding action or comment will be called bullying and will be dealt with seriously. We are a listening squad. We all have a duty to report bullying. Bullying is too important to ignore. FUNDRAISING All cheerleaders will participate in as many fund raising projects as they can. The money raised will be used to fund additional agreed expenses during the year such as extra items of kit or
equipment. Donations to, and fund raising for, squad team funds or charity projects may be conducted as agreed by the squad and coaches. ELIGABILITY Athletes must be physically fit, neatly presented and if under 18yrs old provide written parental permission to train and compete. A candidate may be male or female and a minimum age of 7yrs Acceptance of an athlete on to an Inspire Allstars squad shall be on the basis of ability, Coaches agreement and acceptance of the squad rules. It shall be without prejudice of gender, race (including ethnic origin, colour, nationality and national origin), disability, sexual orientation, religion or belief. MEMBERSHIP This is open to anyone who meets the eligibility criteria above. Who attends regularly and participates fully in the squads activities and pays their subscriptions on time. The squad will be divided into appropriate groups depending on age as specified by The International All Star Federation and ability. Insurance is mandatory and should be paid to British Gymnastics through NDGA. There are no Exemptions. ATTENDANCE REQUIREMENT Once a cheerleader agrees to participate in the competition all practices must be attended (including extra ones). Other Performance From time to time the squad will be asked to take part in fetes, parades and other charitable events. All members are expected to participate in such events, unless holiday or illness prevents them doing so. Social Events From time to time the squad will arrange social events. All members are encouraged, but not required to attend. Non-attendance notification is required. The Coaches will give reasonable notice of all upcoming events, through engagements lists, word of mouth and separate letters (if necessary). DISCIPLINARY ACTION It shall be squad policy that disciplinary action will only be taken as a last resort, and that every reasonable attempt will be made to avoid situations likely to lead to such action through squad discussion and positive encouragement, by example and by finding ways and means to correct negative trends. A squad member may be suspended from performances/competitions for the following reasons:- Unexcused absence, excessive absence, failure to cooperate with coaches or failure to abide by the squad rules and regulations. A squad member may be dismissed from the squad for the following reasons: - Excessive and Irreconcilable disruptive influence on the squad, excessive absence, bringing the squad into disrepute. PARENT CODE OF CONDUCT At Inspire Allstars our vision is very clear: The child comes first and the sport comes second. We are committed to supporting the positive development of young people through the activity of cheerleading. Good sportsmanship occurs when team mates, opponents, coaches and officials treat each other with respect. Children learn the basics of sportsmanship from the adults in their lives, especially their parents and coaches. Children who see adults behaving in a sportsmanlike way gradually come to understand that the real winners in sports are those who know how to persevere and to behave with dignity - no matter where they place. Parents and guardians can help their children understand that good sportsmanship includes both small gestures and heroic efforts. It starts with something as simple as saying hello or waving at another team before a competition and includes acknowledging the talents of other teams and accepting bad calls gracefully. Displaying good sportsmanship isn t always easy: it can be tough to congratulate the apposing team when you haven t placed where you thought you should have, but children who learn how to do it will benefit in many ways. A child who practices good sportsmanship is likely to carry the respect and appreciation of others into every other aspect of life. Without constant reminders and good examples children may forget what behaviours are appropriate at competitions and in the gym. Adults who emphasise good sportsmanship see winning as just one of the goals they d like their children to achieve; they help athletes take pride in their accomplishments and improving their skills so that the children see themselves as winners, even if the score sheets do not show numbers going in their favour. Your behaviour and actions and practice and competition will influence them more than any pep talk you give. Here are some guidelines that we expect Inspire Allstars parents to follow in order to help us do our job with the athletes: Unless you are coaching the team, please remember that you are a parent. Shout words of encouragement rather than directions! Keep comments positive; don t bad mouth coaches, athletes or competition officials. If you have a serious concern about the way practices or competitions are being conducted or you are upset about another parent/coach/official s behaviour please discuss this privately with the Head Coach. At competitions applaud good routines, no matter who they are. Set a good example with your courteous behaviour towards the parents and athletes on other teams congratulating their children when they win. Keep it in perspective; it is just Cheer and Dance. These are activities that your child choses to participate in because they are fun and enjoy the activity. Look for good examples of sportsmanship in professional athletes and point them out to your children. Talk about bad examples too and why they upset you.
