Set up equipment to be used for operations. 20 Control leak by rolling drum or getting hole in the vapor space FAIL Patch the puncture hole. 10 Properly place drum into overpack drum by accepted method. 20 Upright overpack drum. 10 Locate overpack drum out of spilled material 10 Notify entry team leader that the operation is complete. 10 Exit out to the decontamination sector 10 Page 8 of 19

CHLORINE A-KIT- SIDE PATCH NFPA 472 4-4.3.1, 4-3.2, A-4-3.1, A-4-3.2 Practical Station EIGHT apply an A-kit for a leaking 150lb Chlorine cylinder. ALL actions shall be in accordance with the kits manufacturers specifications/ reccomendations. Set up equipment to be used for operations. 10 Control leak by actuating a valve if possible 15 Prepare kit to be placed on cylinder... 5 Prepare Cylinder for patch... 10 Secure side patch... 10 Ensure leak has been completely stopped... 10 Secure cylinder from tipping 10 Notify entry team leader that the leak was stopped. 10 Exit out to the decontamination sector. 10 Page 9 of 19

DOME COVER CLAMP Practical Station FIFTEEN NFPA 472 4-4.3.8 stop a leak from MC-306 dome cover. Set up equipment to be used for operations. 20 Ground the tank prior to operations. FAIL Safely operate around tank. HAZMAT- VICTIM DECONTAMINATION Practical Station SIXTEEN NFPA 471 7-1 CFD SOG 8.5.4, 9.8 The candidate shall identify the proper type of decontamination system to set up to decontaminate victims, responders, tools, and equipment. SETUP Identifies product(s) involved. FAIL Sets up a decontamination cooridoor for incident FAIL Identify type of decon system to set up. HAZMAT- PPE Practical Station SEVENTEEN NFPA 472 3-3.3 The candidate shall identify the proper type of Personal Protective Equipment (PPE) For a given situation. The canidate shall identify and inspect each type of PPE available. SCBA SCBA Donning & Doffing - Extend all straps before donning 1 - Check for full tank gauge reading full.. FAIL - Donning Did the examinee per manufacturers recommendations: - Turn on tank valve or main valve as necessary 1 - Compare tank gauge with regulator gauge 1 - Acceptable procedure for donning backpack,. 1 - Fasten all harness straps properly. 1 - Extend the coat collar free of straps.. 1 - Don facepiece, pull spider straps back, not sideways.. 1 - Check facepiece seal (pressure test). FAIL - Complete donning within 60 seconds 1 Sub Total (8) APR APR Donning & Doffing Extend all straps before donning.. 2 Identify Proper cartrage and exposure limits... FAIL Donning Did the examinee per manufacturers recommendations: Don facepiece, pull spider straps back, not sideways..... 1 Check facepiece seal (pressure test)..
