Safety harness inspection checklist for equipment supplied for fall arrest purposes, during Scaffolding Operations Note. If the equipment has been used to arrest a fall, it must be returned for periodic inspection by a competent person before being put back into service. This checklist is not a periodic inspection, it is for the use of trained in-house users/examiners. Instructions to person using this checklist These checks should be carried out in good light and must involve detailed examination of all parts of the harness - both visually and through feel. Webbing should be run through the hands slowly, bending the webbing into a U shape examining for all signs of damage - even 1mm cuts from the edge of the webbing are significant. Condition checklist: Is all stitching in good condition, free from obvious weakness? Is the webbing free from fraying, cuts (of 1mm or more), or other damage, however small? Is the webbing free from surface abrasion? Is the webbing free from discoloration or flaking? Is the webbing clean, i.e. free from dirt, grit, paint, and grease? Is the webbing free from areas of softening or hardening of fibres? Is the webbing free from signs of heat damage, e.g. burns, glazing of the surfaces Are all the fittings present and free from defect, e.g. D rings, buckles, rivets? Are all moving parts, functioning correctly, e.g. karabiners, screwlink connectors? Does the webbing appear to be in generally good condition? Is the harness clearly marked to show when it is due for re-examination by a competent person? Is the harness free from mould or other signs that it has been kept in damp conditions? Is the harness free from any alteration, addition or repair unless carried out by manufacturer? Are the following legible on the labels: the safety notices (buckle threading diagrams etc.) the date of manufacture an identification number details of the EN standard(s) the item complies with restrictionsonuse,e.g.safeworkingload. Warning. Equipment must be withdrawn from use immediately should any doubt arise about its condition for safe use and it should not be used again until confirmed in writing by a competent person that it is acceptable to do so. Suitability checklist: Is the harness being used for the purpose intended? Is the harness being used within its rated capacity? Is the harness being used only by trained users? Is the harness compatible with other equipment and clothing to be worn simultaneously? Will it be ensured that there is a suitable anchorage point, including where necessary for the functioning of the equipment, an anchor point above the user? Are there suitable rescue arrangements in case of a fall? Is a shock absorbing lanyard to be used with the harness? Will there be sufficient free space to enable any fall to be arrested before the individual strikes the ground or any other object? Note. This checklist/register is to be retained on file for at least three years.
