2017-2018 Deer Hunting Permit Packet This packet contains all information and paperwork necessary to apply for a hunting permit within the City of Mentor. In this packet, you will find: Hunting application Permission to hunt form(s) City of Mentor hunting agreement form and regulations Archery proficiency test form Deer harvest form Complete the following forms, and return them to the City of Mentor Department of Parks, Recreation and Public Facilities 8500 Civic Center Blvd. Mentor, OH 44060: Hunting application Permission to hunt form(s) City of Mentor hunting agreement form Archery proficiency test form Upon return please bring your driver s license as well as your State of Ohio hunting license. At this time the City of Mentor will begin processing your application, confirming property owner permission, lot size and safe hunting areas on the proposed property or properties. If your application is approved you will be contacted by a representative of the City of Mentor who will issue a 2017-2018 hunting permit. If you are approved you will receive parking tags to be displayed in your vehicle while actively hunting as well as a City of Mentor hunting permit number, which is required to be displayed on your tree stand as well as all arrows/bolts that will be on your person while actively hunting. While you are hunting it is your duty to know The State of Ohio hunting rules and regulations as well as the regulations in the City of Mentor hunting agreement form. If you harvest a deer it is your duty to abide by the State of Ohio regulations on game checking as well as return the City of Mentor deer harvest form to the City of Mentor Police Department within 24 hours or report via the website. Please keep in mind that the Chief of Police has the right at anytime to revoke hunting privileges for any reason. Safety is the City of Mentor s main concern, and any improper or illegal behavior will result in relinquishment of your permission to hunt in the City of Mentor. Hunting Agreement THIS AGREEMENT, entered into this Day of 20, by and between 1. 2. 3. (Hereinafter referred to as PROPERTY OWNER ) and (Hereinafter referred to as HUNTER ). WHEREAS, the PROPERTY OWNER wishes to permit the HUNTER to hunt on the belowreferenced property in accordance with the rules of the City of Mentor; and WHEREAS, said real property upon which said hunting shall be permitted is as follows: A. PROPERTY OWNER S ADDRESS/ADDRESSES AND PHONE NUMBERS 1. 2. 3. B. Size of hunting area Acres (no more than 5 of the total number of adjoining parcels must equal or exceed 3 huntable acres, without permission from the Chief of Police) C. A hunting area must be approved by the City of Mentor prior to any hunting. WHEREAS, the PROPERTY OWNER and HUNTER wish to set forth herein the terms and conditions under which the hunting shall take place. The PROPERTY OWNER and HUNTER agree to the following: 1. PROPERTY OWNER hereby grants permission to the HUNTER to hunt the abovereference PROPERTY OWNER S land during the State Ohio Division of Wildlife Bow Season, or the timeframe as set by the City of Mentor, provided that the terms and conditions set forth herein below are satisfied. A. The HUNTER shall have a valid hunting license and appropriate deer tags as required by the State of Ohio Division of Wildlife regulations for the 2017 hunting season. B. Hunting is not permitted in any hunting area on Sundays. Sunday hunting will result in the loss of the hunting permit. C. The HUNTER uses no weapon except a bow and arrow, longbow, compound bow or crossbow during the hunting season. 5

D. The HUNTER shall provide proof of passing an archery proficiency test as set by the City of Mentor. The HUNTER must use the type weapon that they qualify with to hunt (longbow/compound/crossbow). E. The HUNTER shall have in his/her possession at all times while hunting: a city hunting permit, a valid photo I.D, and a functioning cell phone assigned with the same number listed on the permit application. F. The HUNTER agrees to hunt only within the HUNTING AREA designated by the City of Mentor. G. The HUNTER agrees to park his/her motor vehicle in areas designated by the PROPERTY OWNER and place the parking permit/tag in a place clearly visible through the front windshield of the vehicle while hunting. H. The HUNTER agrees to conduct him or herself in a legal and responsible manner as a guest on the PROPERTY OWNER S land. The HUNTER agrees to respect the PROPERTY OWNER S land and agrees to leave the property in the condition the HUNTER found it. I. The HUNTER shall not hunt less than 100 feet from any property line, public or private street, or public or private drive. All shots taken by the hunter must be directed on to the property for which they have permission to hunt. J. The HUNTER must harvest one (1) antlerless deer before harvesting an antlered deer. K. The HUNTER agrees to follow all State of Ohio Division of Wildlife rules and regulations for deer hunting. L. The HUNTER agrees to refrain from recording any video footage or photographic imaging of any kind with any devices, including via the use of cellular or smart phones, pertaining to any hunting activity within the City of Mentor. M. HUNTERS must be eighteen (18) years of age or older in order to apply for a hunting permit. No minors will be allowed to hunt within the city. 2. Additionally, the PROPERTY OWNER and HUNTER agree to the following provisions: A. The HUNTER agrees to hunt from a tree stand, which shall be a minimum of eight (8) feet off the ground. B. The HUNTER agrees not to place any hunting equipment (tree stands, ladders, bait, weapons, etc) within the hunting area any earlier than two (2) weeks prior to permissible city hunting. The HUNTER also agrees to remove all hunting equipment from the hunting area no later than two (2) weeks after the close of permissible city hunting. 6

C. The PROPERTY OWNER and the HUNTER acknowledge that all required forms must be submitted and approved by the City of Mentor prior to each hunting season. D. The HUNTER shall immediately remove carcass and entrails from the OWNER S PROPERTY, unless the PROPERTY OWNER agrees to burial on the PROPERTY. No entrails will be left in the open or in plain view. E. The PROPERTY OWNER/OWNERS and the HUNTER shall preview the property boundaries prior to hunting. All hunting locations within the hunting area are subject to city approval prior to hunting. F. The PROPERTY OWNER agrees that the City of Mentor shall determine the number of hunters allowed per hunting area. G. All harvested deer must be reported by the HUNTER by returning a harvest form to the City of Mentor Police Department within 24 hours of the harvest or by reporting the same through the city s website within the same time period. H. The HUNTER shall immediately notify the City of Mentor Police Department any time the HUNTER wounds a deer and loses sight of it. I. The HUNTER shall immediately notify the City of Mentor Police Department if a wounded deer leaves the approved hunting area. J. The HUNTER may track a wounded deer on public right of ways and on public property, however, he/she is not allowed to carry his/her weapon onto public right of ways or public property. The HUNTER shall immediately notify the City of Mentor Police Department if a wounded deer is tracked onto a public right of way or public property and requires further measures for harvesting. K. The HUNTER shall clearly mark his/her arrows and tree stands with their city permit number for hunter identification. L. The Chief of Police may revoke anyone s permission to hunt in the city at any time if he determines it is in the best interest of the public. M. The Chief of Police may deem a property unsuitable or any portion thereof for hunting at any time if he determines it is in the best interest of the public. N. A city representative or police designee has the right to enter any property where permission has been given to hunt if it pertains to hunting and/or public safety. 7

Archery Proficiency Test In order to qualify to hunt in the City of Mentor a proficiency test is required. The test consists of taking five shots at a NFAA 40 cm target at a distance of 20 yards. No shots are to exceed the fourth ring, a minimum of 22 out of a possible 25 is required and there will be no practice shots prior to testing. Deer Harvest Form If you harvest a deer it is your duty to abide by the State of Ohio regulations on game checking as well as return the City of Mentor deer harvest form to the City of Mentor Police Department within 24 hours or report the same through the city s website (http://cityofmentor.com/departments/parksrecreation/natural-resources/) within 24 hours.
