In order to participate in the 2014-2015 season, all swimmers are required to complete the forms included in this packet. Please return registration packets to the JCCSF, attention Regina Aguilar. You are only required to register once per school year. o o o JCCSF Swim Team Registration Form JCCSF Activities Consent & Release of Liability Permission to Enter Pool Unsupervised (for swimmers 10 years old and older) All forms must be filled out and returned by your child s first day of practice. Due to safety and liability reasons, swimmers will not be permitted to participate until these forms have been submitted. Also included in this packet is a short bio about Head Coach Rick Mulvihill and a document articulating expectations of the program. They detail what you can expect of the JCCSF and the Head Swim Coach as well as what the JCCSF will expect of all swimmers and families. By specifically listing these criteria, we feel the team can cohesively move toward a successful experience for all involved. We are proud to announce that the Great White Sharks Swim Team will now be led by our Aquatics Program Manager, Cliff Merritt. Cliff has been with us for eight years and has assumed oversight of the swim team. His office is located in the Aquatics Center, so please feel free to stop by and introduce yourself the next time you re at the pool. Credit/Debit Card Number: Expiration Date: Parent of Participant s Name: Signature: Date: PAYMENT and REFUND POLICY o Swim Team Payments are processed monthly. o Participants will be charged automatically to a credit or debit card on file with the JCCSF. o Payments for swim team will be processed on the 3rd of the month. At this point fees are non-refundable. o In order to withdraw from the upcoming month of lessons (without penalty) participants must provide written notice to the Swim Team Manager no later then the 18th of the preceding month (Example: to withdraw from July you must provide written notice by June 18th). Acceptable forms of written notice are email to: raguilar@jccsf.org (notification emailed to coaches or verbally communicated will NOT be honored or processed). o Participants that withdraw after the withdrawal date but before the processing date will be charged a Cancel Fee equal to 50% of the monthly team fee. o We do not offer refunds or credits for missed practices nor do we pro-rate early withdrawals. o Should a significant emergency/ medical condition occur which precludes the swimmer from participating in a practice for which they are registered; a pro-rated refund may be issued after the receipt of appropriate documentation at the discretion of the JCCSF. Please contact the Swim Team Manager for more information. o All class participants must complete a JCCSF Youth Medical Consent and Liability form before participating in their first practice. This form is available in the Youth Athletics Office, JCCSF Front Desk or at www.jccsf.org/youthsports. Failure to do so can result in enrollment cancellation and forfeit of fees. JCCSF Great White Sharks Swim Team Expectations of JCCSF, Head Coach, Swimmers & Parents JCCSF Swim Team Mission Statement The JCCSF Great White Shark Swim Team is designed to provide a developmental approach to the competitive sport of swimming. The experience falls within the Jewish Community Center of San Francisco s philosophy with a focus on engaging young swimmers. This will include swim skills development, challenging appropriate training methods, a focused work ethic, commitment, goal setting, teamwork, sportsmanship, learning how to be competitive, and establishing an environment that fosters growth and self-improvement. Our coaches will support these goals by providing enlightened guidance, strong direction, excellent instruction, and consistent positive reinforcement so everyone involved will have the best experience possible. Youth Sports Mission Statement The Jewish Community Center of San Francisco offers a wider range of recreational and competitive sports programs for children. Our program includes sports experiences for all levels of skill. At the JCCSF, we view participation in sports as an opportunity to engage children in sports-specific skills as well as life skills, such as personal effort and teamwork. We gauge our success not by the final results but by the improvement of our participants, the effort put forward and the sense of teamwork we achieve. Expectations of JCCSF Management Staff Provide clear and consistent communication of Swim Team goals and information. Open door policy where the JCCSF professional staff is available to discuss any aspect of the team including constructive feedback from swimmers, parents and coaches. Supervise all team coaches in all aspects of the team experience including communication, planning and coaching. Ensure an environment where swimmers can improve their skills thereby leading to increased confidence and a positive experience. Foster a team experience where all participants (swimmers, parents and coaches) can be an active, informed and engaged member of the team. Ensure the team participates in 7 9 meets per year. Ensure all team goals and events are being planned and achieved. Show respect for all players, parents, coaches, referees, league staff and spectators. Expectations of the Head Coach Be the leader of the JCCSF Great White Sharks Team for all swimmers and parents. Be a strong and active presence/advocate for the Swim Team. Attend all team functions including practices, meets and social events. Communicate clearly with swimmers and parents regarding expectations, team information, swimmer progress and/or anything pertaining to the team/individual experience. Including addressing parents directly regarding any swimmer issues that require special attention. Keep the Program Manager informed of all team information and needs. Be prepared for each team function and provide a structured, disciplined and engaging practice environment which focuses on fundamental skill development and relationship building with all participants. Maintain consistent and clear communication with team members and their families via email and other mediums throughout the season. Be receptive to constructive feedback from JCCSF professional staff and parents.

Arrive to pool at least 15 minutes prior to practice and meet time. Be prepared with meet registration information, guide swimmers to time and location for each race and provide active direction during meet. Provide supervision, direction and guidance of swimmers during actual practice time. Provide direction and guidance during swim meets. Maintain a professional demeanor and appearance during practices and meets. Schedule, coordinate and (as necessary) manage 7 10 swim meets per year. Assist in planning and execution of team social events Cultivate relationships with local swim clubs, teams and programs. Identify appropriate parent volunteer opportunities. Schedule and lead parent volunteer training sessions as necessary. Show respect for all players, parents, coaches, referees, league staff and spectators. Expectations of Swimmers - What does it mean to participate? Regularly attend practices and meets. Communicate to the coach when he/she will not attend. Swimmers are expected to attend at least 4 swim meets per year. Swimmers are excepted to attend team social events. Be a team player. Be supportive of other swimmers on the team showing high levels of sportsmanship. Ask questions when you don t understand a coaching point, concept, or instruction. Actively cooperate with coaches and teammates. Come to practice and meets on-time, ready to swim. Listen to coaches during team meetings, drills and other team activities, doing their best to focus during games and meets. Always try your best, both physically and emotionally. Rewards do not come just with improved times/technique; they also come with maximum effort. Help coaches clean up equipment after practices and meets. Show respect for all players, parents, coaches, referees, league staff and spectators. Expectations of Parents Escort players 12 and under to and from the JCCSF Pool. Players younger than 13 years are not permitted to walk to the pool by themselves. Parents and caregivers must park and walk into the pool to ensure proper supervision and safety. o Swim Team members age 10 years to 12 years do have the option to walk themselves into the pool area if their parent chooses to complete a supplementary permission agreement. This form is available by contacting Regina Aguilar. Supervise swimmers during away meets when they are not competing. Coaches will be at meets to provide logistical guidance and coaching. Coaches are not responsible to supervise swimmers when they are not competing. Parents are expected to be available to transport swimmers to/from meet and practices. Coaches are not permitted to transport swimmers. Enable coaches the opportunity to establish themselves with each player via practice and meet interaction. The relationship between player, coach and team does not happen instantaneously but over the course of time. This sometimes takes 2-3 weeks so we ask that parents give coaches the time to connect with the team. Support the goals and strategies of the coach. In the event that you disagree with a particular goal, strategy or philosophy of a coach, we ask that you take the time to discuss the differing opinion with the coach in a constructive manner. The best times for these discussions are generally away from the pool and we encourage parents not to engage their coach immediately following a meet as emotions are often not best suited for productive discussion.

Attend parent meeting in September prior to the start of practices. This is an opportunity to meet the coach, learn about team expectations, and policies and program philosophies. Ensure players are on time to practices and games. Communicate directly to the coach when your swimmer will not attend a practice. Respond to meet information in a timely manner, meeting set deadlines, so coaches can plan accordingly and register the appropriate swimmers according to meet deadlines and procedures. Volunteer during meets and occasionally at practice to help shuttle kids to the pool and get them on deck in time for practice. The Head Coach will define more specific volunteer roles as the season progresses. Communicate team and player issues directly to the coach of the team as well as JCCSF professional staff. To exhibit good sportsmanship during meets, cheering appropriately for all participants. Parents are not permitted to coach from the sidelines during practices or meets. Many voices, particularly those of their own parent, can cause a swimmer to be confused, thereby creating indecision and a lack in confidence. It also undermines the coach, taking away his/her ability to direct a player or team successfully. Show respect for all players, parents, coaches, officials, league staff and spectators. As such, we are always available to talk about what s working for your swimmer and how the JCCSF can continue to offer the very best swim team experience.

JCCSF Swim Team Great White Sharks Permission for Swimmer to Enter Pool Unsupervised ONLY For Swim Team Members age 10 years old and older The general JCCSF Fitness Center policy is that children 12 years of age and younger must be accompanied by an adult. This policy is in effect for the safety and well-being of your child and includes access to the JCCSF Pool. We realize that this presents difficulty in having your child attend swim team practice so we are adjusting the policy in an effort to make drop-off and pick-up more convenient. It s important to emphasize that this is a test policy that will be continued or suspended based on your cooperation and this policy s overall success. I hereby give permission to my child to enter the JCCSF Pool without adult supervision. I further understand that my child is to go directly to the pool after using the Youth Changing facility. My child is not permitted to use the Men s or Women s Locker Rooms, enter the JCCSF Fitness Center (workout area) or any other area of the JCCSF Fitness Facility. Swimmers with the appropriate permission are required to show a special card to the Front Desk staff in order to gain entry to the pool without adult supervision. Without this card, swimmers will not be permitted to enter without a parent or caregiver. Swimmers will receive this card upon submitting this signed permission form. Card will be provided by the JCCSF within 5 business days of receipt of this signed form. Proper behavior is expected by all children. Loud voices, horseplay and other inappropriate behavior will not be tolerated and may result in unsupervised permission being rescinded. Any violation of these rules may result in this special permission being rescinded and would therefore require the child to enter the facility under the supervision of a parent or caregiver. The JCCSF has created this policy for the convenience of JCCSF Swim Team members and the JCCSF is the sole decider of the application of this policy and can make the decision to revoke or change the policy at any time. A refund is not merited if unsupervised permission being revoked thereby makes it difficult for your child to attend their assigned practice time. This is a trial policy. The JCCSF will evaluate the success of this policy to determine if it will continue. FAQ s How long is the season? The season begins in September and ends in July. No swim practice or meets in August How much does it cost? The fees are based on the number days that the swimmer attends practice AND their membership status. Fees are charged monthly. Please refer to registration form for details. What if our schedule changes? Provide written notice by the 18 th to raguilar@jccsf.org of the month prior of the expected change. We try to accommodate family and sports schedule throughout the season. What if my swimmer wants to drop from the swim team? If you advise by the 18 th of the month, the following month will not be charged. My swimmer is 7 years old and wants to try-out for team? The swim team is for swimmers 8 years old and older. We would encourage you to consider our pre-swim class, Tiger Sharks, one hour in duration, to build endurance and strength. How does my swimmer participate on the team? Swimmers must try-out to be considered for the team. Please contact Regina Aguilar at raguilar@jccsf.org to express your interest. If there is space available, she will advise you of the next tryout opportunity, typically the 3 rd Thursday of the month. How old are swimmers on the swim team? Swimmers are 8 to 17 years old. When is practice? Our younger swimmers (grades 3 rd - 6 th ) typically swim at 4pm and our junior and senior high school students swim at 5pm. We have practices Monday through Thursdays. There are no drop-in or make-ups. Swimmers are assigned swim days based on availability and preference. Swimmers must swim at least 2 days a week with options of 3 and 4 available.

How many swimmers participate? For positive instructional purposes and safety, 14 swimmers are in the pool between 2 lanes during a practice session. The team can be as large as 35-40 swimmers depending on their collective schedules. Do you participate in swim meets? The Great White Sharks participates in about five swim meets with local JCCs in the Bay Area, typically on Sundays. The Great White Sharks hosts a meet at USF/Koret Center. What type of meets? Our meets are considered recreational and follow USA Swimming rules. How often are the swim meets? Do I have to participate? Meets are held during the Spring/Summer months approximately once a month. Swimmers will be advised in advance when registration opens for specific meets. Registration typically closes about ten days later or when participation reaches capacity. According to the Expectations of Swimmers, it is requested that they participate in at least two meets. Participation completes the experience. How does the team communicate? As a member of the Great White Sharks, swimmers families will have access to the JCCSF Scoreboard. This is a secured sight for the team to receive newsletters from the Coaching Staff, meet information, results, important upcoming dates and more. Members are able to access it at any time as well as a portal for team information to be dispersed efficiently. Parents are always welcomed to contact Regina Aguilar at 415.276.1514 or raguilar@jccsf.org My swimmer is sick, can t make her schedule practice day. Can I come for a make-up? Please send an email to both Coach Rick and Regina if your swimmer is not going to attend practice. This is especially important if there are multiple or extended absences. Because our practices are at capacity, and for safety reasons we are not able to accommodate drop-ins or make-ups.
