Please send a cheque payable to Synchro Canada with the entry form to the National Office. According to CASSA Rule, 5.1.5, fees of $25 per routine are charged for any late, incorrect and/or incomplete entries by the stated deadline(s), payable to Synchro Canada. Late Entry Fees It is the coach s and/or the swimmers responsibility to pay this fine to the appropriate official by one hour before the beginning of the competition for that category. Failure to do so shall result in disqualification. The final deadline for late entries is two weeks prior to the start date of the meet. No entries will be accepted after this date. Email Policy Information Language Policy Figures Draw Snacks during Figures Competition Routines All correspondence as required will be by email. All meet information packages and other important communications will be available on the Synchro Canada website, www.synchro.ca. All official information packages and correspondence will be provided in French and English. All event announcements will be made in French and English. The figures draw will be determined and communicated by Synchro Canada, according to Synchro Canada Rules. Note: All meals are the responsibility of the swimmers and their clubs. All routines must enter the pool as outlined in Pool Layout section below. Results Information Package #1 p. 3

Food on Deck Athletes Watching Routines Drug Testing Athletes Waiver All results will be sent to coaches by email and will be posted on Synchro Canada Website (www.synchro.ca) immediately following each event. No Food Allowed On Deck. Athletes and team members will be asked to watch all events from the athletes seating area (see map). Please refer to Synchro Canada Website regarding drug tests and banned substances. Each athlete is required to complete the Athlete Waivers forms that can be found on page 14 of this information package. Please have the form signed by the athletes or by the parents (if under the age of 19) and drop them off at the registration desk on April 26, 2016. Admission Prices All event tickets price will be announced in the next package Photo and Video A photographer and videographer will be present during the meet. Photos and videos will be offered at a moderate price. Further announcement will be available in the next package. Transportation is the Clubs responsibility. Transportation Please note that all sponsored hotels are less than 10 kilometers from the swimming pool. The Public Transit Web site is at www.calgarytransit.com Food Services The Talisman has limited concessions on site that athletes can purchase food. We are working on providing a place for athletes to have the ability to eat food provided for them by the chaperones, further details will be provided in the next package. Security and Health Services Trained Lifeguards will be on deck at all times during the competition. Social Activities Information for Officials See details in next Information Package. Transportation will be available upon arrival and departure to and from the airport, as well as to and from your hotel and the pool during the meet. MUSIC REQUIREMENTS Digital music will be used at this competition. Instructions and location to send music in advance will be included in information package #2. Music Format There must be one digital file for each registered routine in one of the following formats: Mp3 files with an.mp3 file extension or Wave files with a.wav file extension. Coaches should review each digital file for quality, timing and completeness. It is suggested there is a 3-5 second lead from play until the music starts. Naming the digital music files: Each routine/music file must be named as indicated below. Please make sure not to have any blank spaces or special characters (accents, #, $, &) in your files names. Talisman Centre pool layout The Figures competition will be held in the Dive Tank which is 20mX20m and a depth of 5m. The routines competitions will be held in the South competition pool which is 20mx25m and a depth of 2.6m, the North competition pool will be available for warm up and cool down and measures 20mx25m and a depth of 1.8m. Please note that all routines will commence at the South end of competition pool and their routine exit will be at the North West corner of the competition pool. The Bulkhead at the entry point along width of the pool is a height of 0.33m and a width 0.9m. Information Package #1 p. 6

5940 Blackfoot Trail SE Calgary 403-259-1720 Rate: $139.00 Booking Code - 11730 Distance from Pool 7.1 k ATHLETES SURCHARGE - $75 This surcharge is assessed to each athlete attending each of the national competitions (i.e. Qualifier or Nationals) to offset the costs of having officials at these meets. This surcharge is charged only once per athlete per meet (i.e. if a club has 15 athletes attending Nationals they pay $1125=15 x $75 no matter how many solos, duets or team events they have; and pay again for each athlete who attends Nationals). This surcharge is to be paid along with the entry fees. For those clubs whose province pays this fee, the club is to communicate with their provincial office to obtain the refund by submitting a copy of their roster as verification. # of Athletes @ $75 = $ Information Package #1 p. 9

ANTI-DOPING TESTING INFORMATION COACHES: Please have a copy of the following signed by each athlete entered in the Canadian Open Championships The completed forms are to be returned with the entry forms to Synchro Canada, Projects & Events Manager. You can either scan them and send them by email at vivianne@synchro.ca or mail them to Synchro Canada, 700 Industrial Ave, suite 401, Ottawa, ON, K1G 0Y9. WHAT TO DO IF YOU MUST TAKE A MEDICATION 1. The athlete must inform the physician prescribing the medication that she is an athlete under doping control restrictions and that she cannot take any substance on the WADA (World Anti-Doping Association) banned list. 2. The athlete should ask the physician to determine if there is an alternative drug that can be taken if the recommended drug contains a banned substance. 3. If the physician indicates that there is no alternative drug available for the athlete to take, the athlete should obtain a letter IMMEDIATELY from the physician indicating: a) The name of the drug he/she is taking, b) The specific banned substance(s) in the drug, c) The reason the drug must be taken, d) Confirmation that there is no substitute drug to be taken. This letter should be forwarded by the athlete IMMEDIATELY to Vivianne Slade Synchro Canada 700 Industrial Ave, Suite 401 Ottawa (Ontario), K1G 0Y9 NOTE: An exemption must be granted by the Canadian Centre for Ethics in Sport (CCES) prior to the athlete competing or the sample could be considered positive. If the athlete wishes to obtain a second opinion on the drug she is to take, she should call Synchro Canada s consulting physician on anti-doping, Dr. Margo Mountjoy - she may be able to recommend an alternative drug. 4. A reminder that anti-doping controls may be conducted at competitions or in the athlete s own training pool on an UNANNOUNCED basis, therefore, it is imperative to act on this matter immediately. DO NOT PROCRASTINATE! 5. The undersigned athlete hereby consents to the use of his or her performances, name, biography and likeness on or in connection with any television or radio program, video/dvd, web streaming, web archiving, print media or the advertising and publicizing of such program as may be designated by Synchro Canada and waives all rights to remuneration or otherwise in connection with the above. The foregoing is subject to assurance that the eligible status of the athlete will be fully protected by Synchro Canada. LIABILITY WAIVER The undersigned understands, acknowledges and agrees that Synchro Canada shall not be liable for injury or loss occasioned by the athlete while travelling to or from or during the 2016 Shiseido Canadian Open Synchronized Swimming Championships event nor shall Synchro Canada be responsible for any damages or losses caused by the athlete during the same time. The undersigned athlete and/or athlete's parent(s)/legal guardian agrees to indemnify Synchro Canada and hold it harmless from any claims or demands in respect of such loss or damage. CALCULATION OF RESULTS WAIVER The undersigned understands, acknowledges and agrees that there is a possibility of human error in the calculation and posting of results. Synchro Canada will make every effort to ensure results are accurate including announcing corrections when they are required and in accordance with Synchro Canada rules. I, the undersigned hereby certify that I have received this copy of the above Synchro Canada Waivers and that I have fully read and understood and agree to such Synchro Canada Waivers. I am in good health, without injury or illness, and have not been told by any doctor that I may not participate in any physical activity. In consideration of my entry and of my own free will, I do hereby for myself and my heirs, executors, and administrators, waive, release, and give up any and all claims, demands, liability, damages, costs and expenses of any kind whatsoever (including personal injury to me or my wrongful death) against Synchro Canada and its instructors, volunteers, employees, agents and any other persons involved in the event, that may arise from my participation in the event or while traveling to and from the event, even if caused in whole or in part by the negligence or other fault of the aforementioned parties or persons. I am aware of and fully understand the inherent and significant risks associated with my participation in the event. I fully understand that I may injure myself as a result of my participation in the events and hereby release Synchro Canada and its instructors, volunteers, employees, agents and any other persons involved in the events from any liability, now or in the future, including but not limited to all physical, mental or emotional illnesses or injuries, however caused, occurring during or after my participation in the event. I agree that all my participation in the event, as well as travel to and from the event shall be at my own risk. I acknowledge my obligation to immediately inform the nearest employee or others of Synchro Canada if I feel any pain, discomfort, fatigue or other symptoms that I may suffer during and immediately after my participation in the event. I understand I may stop participation at any time, and I may be requested to stop by an employee or others of Synchro Canada who observes any symptoms of distress or abnormal response.
