During a patient visit they can be provided immediately to give improvement in comfort and relief for pain. Formthotics Original models fit the majority of patients for a standard volume shoe (eg. a casual or running shoe). Formthotics versatility A wide variety of shapes and sizes If you have a one off need for a patient and don t want to stock the model, check with your distributor, you might be able to order in time for the next appointment with your patient. Examples of Formthotics applications: different models in 23 the medical range At risk feet (diabetes, arthritis, neuropathy or fat pad atrophy) High risk feet need accommodative, full contact and cushioning qualities for orthoses. Models: Original Single Soft, Original Single X-Soft, Original ShockStop Excessive pronation in adults For those who have pain related to over pronation, requiring support and comfort. The shape choice of Formthotics also depends on the shape of the footwear (eg. a narrow shape for soccer boots). Models: Consider a medium to hard density eg. Original Single Medium or Original Dual Medium in either medium, firm or hard densities. Note: flat feet with excessive pronation may require a low profile model. Excessive supination/high arches (more rigid foot type) Excessively supinated, rigid foot types generally need cushioning and full contact support to increase the weight bearing surface area and reduce joint load. The system is a comprehensive clinical method to improve lower limb function with custom-fitted Formthotics and includes: 6 Tests Objective clinical tests 6 Steps Select, fit and adjust Formthotics Custom Medical Orthotics Test 1: Foot morphology and motion test Relates to the general principles of open kinetic chain foot biomechanics; joint position and passive range of motion. Test 2: Alignment test The foot and lower limb skeletal alignment is examined in stance to determine if and/or how it compensates when weight bearing. Test 3: Windlass (Jack's) test Assessment of the windlass mechanism using the Jack s test. Looks at the functional dorsiflexion range of motion availability of the hallux associated with heel lift and effective propulsion. Test 4: Supination resistance test Relates to the position of weight bearing axis of the subtalar joint. To assess, the clinician manually supinates the foot and grades the force required. Test 5: Proximal control knee bend test Assesses lumbopelvic postural stability and proximal control systems. The clinician observes the stability of balance, duration of balance, and the loss of balance (medial or lateral). Test 6: Neuromotor balance test Relates to propulsion. The clinician observes the relative ease of rising onto the toes, stability and the tendency of inversion instability. There are 3 options depending on the patient s strength and balance ability and what is being assessed. Option 1: Open/closed eye, single limb balance test Option 2: Star excursion balance test Option 3: Forefoot stability test Step 1. Formthotics selection and fitting Select the Formthotics model and size - consider foam density and shoe shape and then fit Formthotics into the patient s shoes. Step 2. Patient adjustment period Allow a 3 to 7 day break-in period for the patient to adapt to Formthotics. Step 3. Rear foot modification The rearfoot is adjusted by functional modification by way of applying a wedge to the underside of Formthotics. Step 4. Forefoot modification The forefoot is adjusted by functional posting. The improvement of function should be verified by the patient. Step 5. In-shoe testing Check the patient s stance and gait to assess the functional effects of Formthotics while the patient is wearing them in their shoes. Step 6. Follow up and ongoing adjustment Formthotics should be checked and modified at 3 to 6 month intervals especially in the initial period.
