1. Lake Management: Heritage Palms CDD Severn Trent Services Field Management Report Site Inspection 3/23/2016 a. Lake Maintenance Summary: The lakes were in moderate shape this month. Shoreline weeds such as alligator weed and torpedo grass and are slightly increasing throughout the property, water levels are about average for this time of year, submerged weeds are hardly present and algae is randomly throughout the community. Additional lake management details are provided below. b. Aerators: Issues reported / observed this month included: i. 2 of the 3 diffusers are boiling in Lake L-12. ii. I believe there are multiple diffusers that appear to be broken throughout the community. Normally a diffuser disburses the air evenly and you ll see a bubble pattern similar to the one shown below to the left. I m seeing bubble patterns that look more like a small geyser which is an indicator that the diffuser plate has disconnected from the pvc pipe. This concern will be reported to Vertex. Even Disbursement Geyser Disbursement iii. The compressor unit for Lake 17 was found not operating. The cabinet was extremely hot and when I opened the lid I found that the fan wasn t plugged in. The fan was plugged back in and normally once the motor cools down it turns back on. The seal around the door also has to be replaced. This system will be monitored. c. Fountains: No issues observed this month. 3

d. Algae: Algae issues observed this month included the following: i. Low density on Lakes: 2-7, 10, 15, 29, 33, L-9 and Canals C-7 & C-15 (Continue treatment where applicable) ii. Medium density on Lakes: 14 & 31. Treatment needed. Lake 14 Lake 31 iii. High density on Lakes: None. e. Littorals: i. No new issues observed this month. ii. Soft rush was observed growing along the side of 11335 Wine Palm Road. We ll need to monitor the plants to ensure that their growth doesn t get out of control. f. Fish: No issues observed this month. g. Trash in Lakes: Minimal trash observed this month. 4

h. Bulkheads / Rip Rap: Weed growth was observed on all of the bulkheads and rip rap. These areas are not being maintained by Lake Masters. Lake 17 Lake 27 i. Dissolved Oxygen (DO) Tests: The next tests are scheduled for July 2016. j. Grass Clippings/Landscaping: Once the water gets low enough the dead / cut vegetation should be removed from the north end of Lake 30. This will help avoid any potential blockages in the future. 2. Lake Bank Erosion: Erosion concerns observed this month included: a. Erosion is occurring behind the rip rap at 11212 King Palm Court. Additional rip rap is needed along the bank. 5

b. The banks around Lake 21 are beginning to show a vertical drop greater than 9 inches. If the erosion continues to occur the CDD may need to consider repairing the shoreline again. Lake 21 North Bank 3. Preserves: No issues observed this month. The next semi-annual maintenance event is anticipated for May 2016. 4. Roadways: a. Asphalt: i. Micro Surfacing repairs are scheduled to be completed in early May. Areas Marked for Repairs ii. The sink hole across from the Veranda s located at 20260 Washingtonia Palm Way is increasing in size. We ll inform the engineer of this and will seek out a contractor that can give us an estimate to repair it. 6

b. Signage/Landscaping: The Master Board has installed a couple of lighted stop signs along Wine Palm road. c. Roadway Inlets: The catch basin at the end of Gator Palm Drive is covered with leaves and needs to be cleared off. The built up debris will impact the drainage flow in this area. d. Curbing: In the next month or so we ll be conducting the annual concrete inspection. A repair list will be completed for the Board to review. 5. Sidewalks: No issues observed this month. 7

6. CDD Fences: a. All of the CDD fences have been inspected and the following repair list has been generated. A repair estimate has been obtained and will be reviewed at the March 28 th Board meeting. i. Rusted cap behind 7930 Tiger Palms Way ii. New fence ties needed behind 7954 Tiger Palm Way iii. New post needed behind 7966 Tiger Palm Way iv. 40+ feet of posts have rusted out and need to be replaced behind 7978 Tiger Palm Way v. New clamps needed behind 7984 Tiger Palm Way vi. Rusted out top bar and 2 rusted out brackets behind 7418 Heritage Palms Estates Drive vii. Missing cap behind 7346 Heritage Palms Estates Drive viii. Brackets need to be replaced behind 11091 Sierra Palm Ct. ix. Fence ties need to be replaced behind 11151 Sierra Palm Ct. x. Top bar needs to be replaced along the side of 11149 Wine Palm xi. Multiple sections of Top bar and brackets need to be replaced along the side of 11166 Wine Palm (this is a U shape fence with many issues. xii. Replace rusted bracket behind 11247 Wine Palm. Falling Fence Rusted Fence Post 7. Storm Drainage System: a. Overflow Weirs: No functionality issues observed this month. b. Canals: Besides for the weeds and algae concerns stated above no other issues were observed. c. Gutter Pans: No functionality issues observed. d. Catch Basins: No functionality issues observed. e. Inter-Connect Pipes: No new issues observed. 8

f. Drain Pipes: Two drainage pipes in Lake 24 are floating up. This is unsightly from the golf course and Curry Palm road; the pipes should be anchored down. g. Swales: No issues observed with the swale cuts in Preserve C-4. h. Illicit Discharges: No issues observed. i. Spill Prevention: No issues observed. j. Maintenance Yard Inspection: No illicit discharges or spills observed. 8. Residential Complaints/Concerns: a. We were informed by Casa Del Lago of a dead pine tree in the CDD preserve that was a potential safety hazard to 7391 Sika Deer Way. The tree was looked at and we agreed that it s a potential hazard. The tree is scheduled to be cut down and removed. b. The resident living at 10518 Washingtonia Palm Way informed us of algae forming on the lake behind her home. Lake Masters was contacted and they were onsite the following day to treat it. 9

9. Non CDD Issues Observed: a. One of the wooden fence posts on Curry Palm Lane has fallen out of place. b. One of the bridge planks on Lake L-4 has uplifted. 10. Fish/Wildlife Observations: Bass Bream Catfish Gambusia Egrets Herons Coots Gallinules Anhinga Cormorant Osprey Ibis Woodstork Otter 3 Alligators Snakes Turtles Other Species: Ducks 11. Completed events in February / March: a. Report all lake issues to Lake Masters. Task completed on 2/24/16. b. Report all aeration issues to Vertex. N/A. c. Have the outflow pipe Curry Palm cleaned out. Task pending until we get a new maintenance technician. d. Have the uplifted sidewalk panel at the end of Curry palm grinded down. Task pending. 10

12. Follow up & Upcoming events for March / April: a. Report all lake issues to Lake Masters. b. Report all aeration issues to Vertex. c. Have the outflow pipe Curry Palm cleaned out. d. Have the uplifted sidewalk panel at the end of Curry palm grinded down. e. Monitor the aerator compressor for Lake 17. f. Obtain a repair proposal for the sink hole on Washingtonia Palm Way. g. Conduct annual concrete inspection. 11
