Just Hoop Basketball (Girls Basketball Club Program) (Ages 9 17 Years Old) HANDBOOK ACCEPTANCE FORM I acknowledge that I have received, read and discussed with my daughter the information in the Player Parent Handbook. I understand all of the policies as outlined in this handbook and agree to abide by the rules of the organization. A player may not begin her season with the Just Hoop Basketball until all required forms are signed and submitted to the Club Director. These forms include: Handbook Acceptance, Registration Form, Parent Permission & Waiver Release Form, Player s Code of Conduct, Parent s Code of Conduct and Financial Responsibility Form. In addition, each player must submit a copy of her Birth Certificate, a copy of her most recent report card and 1 st payment before she will be eligible to start her Just Hoop Basketball Season. Just Hoop Basketball Parent Permission Form, Liability Waiver and Release, Indemnity Agreement and Authorization for Medical Treatment of a Minor The parent(s) of participant(s) must complete and return this agreement. It is understood that I (the participant), cannot participate in Basketball Practices, Workout Sessions and Tournaments until this Waiver Form has been completed. Parents or Guardians are responsible of obtaining complete medical information about their child (participant) before signing and reading this agreement. I, the undersigned, the parent and/or legal guardian of, (participant) in consideration of being permitted to participate in the Just Hoop Basketball (Program) hereby grant permission for the Player to play, compete, train, and otherwise participate in the Just Hoop Basketball Club Program including but not limited to practice, games, tournaments, clinics, camps, volunteering, social activities, travel by and all means including private, public, and commercial transport by ground, air, and water based vessels, and other activities sponsored by the Just hoop Basketball. I, the minor s parent and/or legal guardian, understand the nature of athletic activities and the game of basketball ( Activity ) and the player s (participant) experience and capabilities and believe the player is qualified, in good health and in proper physical condition to participate in such Activity. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue my child s participation in the activity. I FULLY UNDERSTAND and recognize that: (a) BASKETBALL (ATHLETIC ACTIVITIES) IS A CONTACT SPORT INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH ( Risks ); (b) these Risks and dangers may be caused by my child s own actions or inactions, the actions or inactions of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE RELEASEES NAMED BELOW; (c) there may be OTHER RISKS AND SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and, on behalf of my child, I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation or that of the player (participant) in the Activity. I HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE Just Hoop Basketball, as well as its founders, owners, officers, directors, employees, coaches, trainers, volunteers, affiliated organizations, sponsors, vendors/suppliers, City of Cerritos, and all the owners and lessors of premises on which the Activity takes place, or any facility utilized by the Just Hoop Basketball, (each considered one of the RELEASEES herein) FROM ALL LIABILITY, CLAIMS, EMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS, NEGLIGENT SECURITY, TRAVEL, AND RECREATIONAL OPERATIONS AND ACTIVITIES; AND I FUTHER AGREE THAT if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the Releasees, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost of which any may incur as the result of such claim. I/We, the undersigned, parents and/or legal guardian of, (Player), a minor, do hereby grant permission for the coaches, employees and its officers of the Just Hoop Basketball Club Program to seek emergency care for my child (Player) if I/We cannot be reached in the event of illness or injury. It is understood that this authorization is given in advance of any specific diagnosis or treatment being required, and I/We agree that I/We will pay for such emergency medical treatment. It is our understanding that we will be contacted in the event of illness or injury if at all possible. This authorization shall remain effective until the end of the program unless revoked sooner. I understand that these rules extend to my conduct in all activities and events sanctioned or sponsored by the Just Hoop Basketball Program, including practices, travel to and from events, tournaments, and overnight stays. I also understand that if I violate any of the following rules, I may be subject to disciplinary action as deemed appropriate by authorized persons of the Just Hoop Basketball Program. Disciplinary actions may include loss of playing time and/or the Club sending a player home from practice or a tournament at the parent s expense and/or dismissal from the Club. The following actions are prohibited: 1. Illegal transport, possession, or use of drugs, alcohol or other illegal substances. 2. Physical damage to a facility or theft of items from a room, dormitory, residence or other person. (Restitution will be part of any penalty imposed). 3. Physical or verbal intimidation of any individual. 4. Use of foul, vulgar or abusive language. Players are required to: 1. Be on time. 2. Call your coach if you are going to miss a game/practice before the event happens. 3. Arrive a one hour early to all games unless otherwise instructed. 4. Wear your Just Hoop Basketball uniform to all games. 5. All players are required to wear their Just Hoop Basketball t-shirts/sweatshirts when traveling to and from a tournament. 6. Practices are Mandatory Do not use homework as an excuse. Do all your homework early or bring it at practice!! TOURNAMENTS & TRAVELING RULES: It is important to remember that each player and parent is representing the Just Hoop Basketball Club Program when we play in local tournaments and travel to out-of-state events. Traveling with the team is an honor and needs to be respected. Traveling Rules: 1. All players are required to stay at the Team Hotel. Staying at the same location promotes team unity and makes team meetings and outings easier to organize. Parents are free to stay at the hotel of their choosing (High School Teams Only). 2. Curfew is 10:00 PM. No player is to be outside of her hotel room unless accompanied by a coach or parent. NO EXCEPTIONS! 3. No players are allowed to go to another players room after curfew. If caught you will not be playing the next game day. NO EXCEPTIONS! 4. No players are to invite players from other Club Organizations into your hotel rooms. If caught you will not be playing the next game day. NO EXCEPTIONS 5. No players are to go to any Boys hotel rooms or to invite them to your hotel rooms. If caught you will be send home on your parents expense and you will not be able to play for the rest of the tournament. NO EXCEPTIONS! 6. Room phone numbers are not to be given out to anyone except members of your team, a Just Hoop Basketball or your parents. 7. All players are required to wear their Just Hoop Basketball t-shirt/sweatshirt when traveling to and from a tournament. 8. Players must use THE BUDDY SYSTEM at ALL times. Player Name Player Signature Date Just Hoop Basketball PARENT S CODE OF CONDUCT & PARENT/PLAYER/COACH RELATIONSHIP PARENT S CODE OF CONDUCT We believe that many lifelong lessons can be taught through the game of basketball. Players can be taught how to be a cooperative team member, how to cope with adversity and how to achieve mental toughness. We believe you chose the Just Hoop Basketball Club Program because it provides a good and positive environment for your daughter. All of our coaches have been chosen to make sure that your daughter s experience is rewarding and fun. In light of this, we ask for the parents support in achieving this goal. You may give your support in the following ways: 1. Encourage your daughter to follow her coach s instructions. 2. Cheer from the stands at practices and games but do not coach from the stands. 3. Respect the integrity and judgment of game officials. 4. Absolutely no foul language will be tolerated. 5. Refrain from criticizing opposing players, other parents, other clubs, teammates or officials. Cheer for our team not against the opponent. 6. Support your coaches and staff. 7. Make sure your daughter arrives to games and practices ON TIME and is prepared. It is the coach s responsibility to enforce our club rules. If a player s parent/guardian exhibits inappropriate behavior, our coaches have been instructed to send that player to go sit with her parents/guardians to help them conduct themselves in a manner befitting our guidelines. A parent, player or spectator may be asked to leave the premises if they become disrespectful to coaches, officials, other players or spectators. The parent should call the coach and schedule an appointment for the player, parent and coach to meet at an appropriate time. If the above steps do not remedy the problem, the parent should call the Director of Just Hoop Basketball to schedule a meeting with the player, parent and coach to arrive at a solution. UNDER NO CIRCUMSTANCES is a parent to approach a coach with a complaint or an evaluation of coaching philosophy. The above procedure must be followed in a professional manner dealing with any situations that may arise. The administration of Just Hoop Basketball will NOT ALLOW THE COACHES OR PLAYERS TO BE BERATED by parents or fellow teammates. If you or your daughters refuse to follow the guidelines set forth, you will be asked to leave the program. Please help the staff and administration of Just Hoop Basketball teach the girls in the Program how to respect their coaches and fellow teammates.

JUST HOOP BASKETBALL Financial Responsibility Agreement Youth Club Tuition Fees: Just Hoop Basketball 4 th, 5 th, 6 th, 7 th & 8 th Grade Teams Qualified players attend 1-2 Practices per week and participate in one or two tournament per month. Tuition cost for Youth Teams is as follow: $85 per month for one tournament or $120 for two tournament a month. Also (practice/gym/tournaments) added and due by the 1 st of each month. If monthly dues paid after the 5 th a late fee of $15 late fee will apply. If you quit Just Hoop Basketball and your team, there will be no refund on any tuition or club fees paid. All fees are NON- REFUNDABLE. Youth Club Fees: $90 monthly dues on the 1 st of each month Training Only $140 - $160 monthly dues on the 1 st of each month for Tournaments $150 one time fee for Uniform: Jersey & Shorts, Sports Bag and Just Hoop T- Shirt Please make checks payable to Just Hoop Basketball (Give it to Dominic Freeman the Club Director) Parent/Guardian, you are fully responsible for your daughter s traveling expenses to participate in all tournaments. Parents are required to transport their child to tournaments and practices. Coaches are not responsible for your daughter s transportation to tournaments. You are also responsible for food and hotel accommodations for out of town tournaments. Parents and player should be aware of the financial commitment your family is making, we would ask that the Parent/Guardian sign below. By signing this agreement, you are committing to pay.
