PERMIT TO ALLOW MINOR USE AND SUPPLY OF AN AGVET CHEMICAL PRODUCT FOR MONITORING OF CARPOPHILUS SPECIES IN SELECTED POME, STONE AND BERRY FRUIT ORCHARDS PERMIT NUMBER - PER12798 This permit is issued to the Permit Holder in response to an application granted by the APVMA under section 112 of the Agvet Codes of the jurisdictions set out below. This permit allows a Supplier (as indicated) to possess the product for the purposes of supply and to supply the product to a person who can use the product under permit. This permit also allows a person, as stipulated below, to use the product in the manner specified in this permit in the designated jurisdictions. This permit also allows the Permit Holder, the Supplier (if not one and the same) and any person stipulated below to claim that the product can be used in the manner specified in this permit. THIS PERMIT IS IN FORCE FROM 9 AUGUST 2011 to 30 JUNE 2012. CONDITIONS OF USE Products to be used: BEETLEJUICE CARPOPHILUS TRAPPING SYSTEM is an unregistered product comprising two feeding attractants and an aggregation pheromone lure, which are prepared and/or placed into a plastic-moulded Attract Em Carpophilus Trap. The three chemical components contain the following active constituents: 1. BeetleJuice A containing a mixture of ethanol, ethyl acetate, 2-methyl-1-propanol, 2- methyl-1-butanol and 3-methyl-1-butanol with a combined total of 352g/L, 2. BeetleJuice B containing a mixture of ethanol and acetaldehyde with a combined total of 970g/L, and 3. BeetleJuice Pheromone Lure containing 15 mg/lure of Carpophilus Aggregation Pheromones. The Beetlejuice Carpophilus Trapping System is to be used in conjunction with the registered product: KILLMASTER ZERO PEST STRIP containing 186 g/kg DICHLORVOS as its only active constituent. Directions for Use: For monitoring of Carpophilus beetles in peaches, nectarines, plums, apricots, cherries, apples, pears, nashi and berry fruit only in accordance with instructions for preparation and use of the Beetlejuice Carpophilus Trapping System as specified on labels in Attachments 1, 2 and 3. Jurisdiction: NSW, QLD, SA, VIC and WA only. Additional Conditions: Users: PERSONS who wish to prepare for use and/or use products for the purposes specified in this permit must read, or have read to them, the details and conditions of this permit. Killmaster Zero Pest Strip only: This permit provides for the use of the product Killmaster Zero Pest Strip in a manner other than specified on the approved label of the product. Unless otherwise stated in this permit the handling, storage and disposal of the product Killmaster Zero Pest Strip must be in accordance with instructions on its label. Supplier/Permit holder: supply and record keeping requirements: The supplier must supply each chemical component of the Beetlejuice Carpophilus Trapping System (comprising of BeetleJuice A, BeetleJuice B and BeetleJuice Pheromone Lure) in containers that comply with the requirements of section 18(1) of the Agricultural and Veterinary Chemicals Code Regulations. Attached to the containers must be labels which are identical in content and format to the relevant labels as contained in Attachment 1, 2 and 3. The permit holder or supplier must not supply an amount of the product Beetlejuice Carpophilus Trapping System (comprising BeetleJuice A, BeetleJuice B and BeetleJuice Pheromone Lure) that exceeds the quantities required to monitor a nominated area for block sizes greater or less then the specified 10 hectares (as per label critical comments under Trap Setup/Placement for Attact Em Traps). A receipt for each supply must include details of the person/s, quantity of each component supplied, the area to be monitored (i.e. greater or less than 10 hectares) and date when supply occurred. The permit holder or supplier must at the time of the supply provide to the person a copy of this permit in full setting out the conditions and/or instructions for use or schedule to the permit approved by the APVMA that sets out the conditions and/or instructions for use. This permit allows only the person or company listed as the Supplier to possess the product for the purposes of supply and to supply the product to a person who can use the product under the conditions of this permit. The permit holder must maintain records of supply under this permit. Specifically the records must include the person/s supplied, quantity of each component supplied, the area to be monitored (i.e. greater or less than 10 hectares) and date when supply occurred, and confirmation that at the time of supply of the product a copy of the permit was provided to them in full setting out the conditions and/or instructions for use or schedule to the permit approved by the APVMA that sets out the conditions and/or instructions for use. The records must be progressively maintained whilst the permit is in force and for a minimum period of two years from the date of expiry of this permit and must be made available to the APVMA upon request. Upon a request being made, the records are to be provided: (i) immediately if the request is verbally from an APVMA Inspector who has attended the premises, and (ii) in the time specified in the written correspondence containing the request. The records regarding the person/s supplied, quantity of each component supplied, the area to be monitored (i.e. greater or less than 10 hectares), and date when supply occurred, and confirmation that at the time of supply of the product a copy of the permit in full setting out the conditions and/or instructions for use or schedule to the permit approved by the APVMA that sets out the conditions and/or instructions for use was provided to them must be made at the time of supply of the chemical product. Maximum Amount of Product to be Supplied: The quantity of the chemical components of the Beetlejuice Carpophilus Trapping System (comprising BeetleJuice A, BeetleJuice B and BeetleJuice Pheromone Lure) that can be supplied under this permit MUST be consistent with quantities approved by the APVMA in writing. Critical Comments TRAP SETUP Placement of Attract Em Traps Trap application rate is based on block size: Block size No. traps/block < 10 ha 2 10ha 4 The precise number of traps required will depend on the varieties, history of infestation, and physical layout of the orchard. The fruit grower will have to use his own judgement. Locate traps 6 to 15 metres outside the perimeter of the field to be protected on a corner on the upwind edges of the field. The traps should be no closer than 30-40 metres apart. Secure metal ring to star picket at a height of 1 to 1.5 metres above the ground. Place trap in ring. Under no circumstances place the traps inside the field as this could increase crop damage. Monitoring should be conducted at least 4 weeks prior to harvest, 2 weeks during harvest period and 2 weeks post harvest. Beetlejuice A and B Pour the 20ml bottle of Beetlejuice B into the 2L bottle of Beetlejuice A. Replace cap and mix bottle thoroughly. Pour 250ml of Beetlejuice mixture into the wide-mouthed jar supplied. Cover the jar with the mesh and secure with the lid. Place upright inside the Attract Em Carpophilus Trap. Beetlejuice Pheromone Lure Suspend the Beetlejuice pheromone lure below the cone in the Attract Em trap using the wire clip. Killing Agent Use of a dichlorvos pest strip is necessary for monitoring of Carpophilus beetles with the Beetlejuice trapping system. Put a 20 mm square piece of a registered pest strip containing dichlorvos into the bottom of each trap. Bioglobal does not supply a pest strip with its Beetlejuice trapping system. TRAP MONITORING & MAINTENANCE For trap monitoring the grower must regularly monitor for the presence of beetles and empty dead beetles from traps. Monitoring should be conducted twice per week during the period when the crop is susceptible (i.e. at least 4 weeks prior to harvest, 2 weeks during harvest period, and 2 weeks post harvest). In order to maintain effectiveness of the monitoring system every two (2) weeks: Remove spent Beetlejuice mixture from the jar and replenish with another 250ml of mixture. When removing spent Beetlejuice mixture from the field pour onto the soil in a location remote from vulnerable crops. Failure to do this can increase beetle damage in the crop. Add a new pheromone lure every 2 weeks. Retain the old lure in the trap as it remains attractive. Repeat this process until 2 weeks post harvest. NOT TO BE USED FOR ANY OTHER PURPOSE OR IN ANY MANNER CONTRARY TO THIS LABEL UNLESS AUTHORIZED UNDER APPROPRIATE LEGISLATION. WITHHOLDING PERIOD: Not required when used as directed. STORAGE AND DISPOSAL: Keep out of reach of children. Store unopened original container in the freezer. Always store the Beetlejuice mixture in a cool, dark place. Dispose of excess Beetlejuice mixture after one season. Dispose of empty bottle in municipal rubbish. SAFETY DIRECTIONS: Wear nitrile gloves when handling Beetlejuice A, B and pheromone lure and the dichlorvos pest strip. Wash hands after use. Bioglobal Ltd does not accept any liability for harm or damage resulting from: 1) this product or its use; or 2) the purchaser acting on advice given in good faith by any representative of the company unless the product itself can be shown not to comply with the specifications on the label and damage results from that noncompliance. If these conditions are unacceptable to the purchaser, the goods should be returned unopened within 7 days for a refund of the purchase price. Critical Comments TRAP SETUP Placement of Attract Em Traps Trap application rate is based on block size: Block size No. traps/block < 10 ha 2 10ha 4 The precise number of traps required will depend on the varieties, history of infestation, and physical layout of the orchard. The fruit grower will have to use his own judgement. Locate traps 6 to 15 metres outside the perimeter of the field to be protected on a corner on the upwind edges of the field. The traps should be no closer than 30-40 metres apart. Secure metal ring to star picket at a height of 1 to 1.5 metres above the ground. Place trap in ring. Under no circumstances place the traps inside the field as this could increase crop damage. Monitoring should be conducted at least 4 weeks prior to harvest, 2 weeks during harvest period and 2 weeks post harvest. Beetlejuice A and B Pour the 20ml bottle of Beetlejuice B into the 2L bottle of Beetlejuice A. Replace cap and mix bottle thoroughly. Pour 250ml of Beetlejuice mixture into the wide-mouthed jar supplied. Cover the jar with the mesh and secure with the lid. Place upright inside the Attract Em Carpophilus Trap. Beetlejuice Pheromone Lure Suspend the Beetlejuice pheromone lure below the cone in the Attract Em trap using the wire clip. Killing Agent Use of a dichlorvos pest strip is necessary for monitoring of Carpophilus beetles with the Beetlejuice trapping system. Put a 20 mm square piece of a registered pest strip containing dichlorvos into the bottom of each trap. Bioglobal does not supply a pest strip with its Beetlejuice trapping system. TRAP MONITORING & MAINTENANCE For trap monitoring the grower must regularly monitor for the presence of beetles and empty dead beetles from traps. Monitoring should be conducted twice per week during the period when the crop is susceptible (i.e. at least 4 weeks prior to harvest, 2 weeks during harvest period, and 2 weeks post harvest). In order to maintain effectiveness of the monitoring system every two (2) weeks: Remove spent Beetlejuice mixture from the jar and replenish with another 250ml of mixture. When removing spent Beetlejuice mixture from the field pour onto the soil in a location remote from vulnerable crops. Failure to do this can increase beetle damage in the crop. Add a new pheromone lure every 2 weeks. Retain the old lure in the trap as it remains attractive. Repeat this process until 2 weeks post harvest. NOT TO BE USED FOR ANY OTHER PURPOSE OR IN ANY MANNER CONTRARY TO THIS LABEL UNLESS AUTHORIZED UNDER APPROPRIATE LEGISLATION. WITHHOLDING PERIOD: Not required when used as directed. STORAGE AND DISPOSAL: Keep out of reach of children. Store unopened original container in the freezer. Always store the Beetlejuice mixture in a cool, dark place. Dispose of excess Beetlejuice mixture after one season. Dispose of empty bottle in municipal rubbish. SAFETY DIRECTIONS: Wear nitrile gloves when handling Beetlejuice A, B and pheromone lure and the dichlorvos pest strip. Wash hands after use.
