Information Sheet Ce formulaire est disponible en français. Before you start For the purposes of the Firearms Act, a minor is an individual less than eighteen (18) years old. This form is for use by resident and non-resident minors: who are at least 12 years of age but have not yet had their 18th birthday, who are under 12 years of age and hunt or trap to sustain themselves or their family, or who are applying under the Aboriginal Peoples of Canada Adaptations Regulations (Firearms) for a licence to possess non-restricted firearms. A minor cannot hold a licence to possess prohibited or restricted firearms or to acquire any kind of firearm. Minors must have the consent of a parent or legal guardian to be eligible for a firearms licence. To be eligible for a minor's firearms licence, you must have taken and passed the Canadian Firearms Safety Course. Exceptions do exist (see Section E for further information). You may be required to attend an interview with your parent or guardian present. The chief firearms officer of your province or territory will contact you. The following information explains certain parts of the form and will help you answer some of the questions. You should read the instructions as you fill in your form. If you are still unsure about a question, call 1 800 731-4000 for assistance. A - Licence Information Box 1 Put an "X" in the appropriate box. If you are applying under the Aboriginal Peoples of Canada Adaptations Regulations (Firearms), please attach a copy of your completed declaration form (RCMP GRC 5642). B - Personal Information Box 3 If you have changed your name several times, please indicate the most recent previous name on the form and list all other previously used names on a separate sheet of paper. Attach the sheet to your completed application form. Box 9 Photo Instructions You are required to provide a photograph of yourself. The photograph must be no larger than 45 mm by 57 mm (1 3/4" x 2 1/4") and the size of your head in the photograph must be at least 30 mm (1 3/16") high. See Figure 1 below. The photograph must show a full front view of your head and shoulders. Your entire face must be clear in the photograph, with no hat, sunglasses or other objects hiding your features. Put an "X" in Box 9 to show you have included a photograph.your parent or guardian must sign the back of your photograph. If you need additional space, list all information requested on a separate sheet of paper, add your name to the top of each sheet and attach the sheet to your application. Mail your completed application form, all attachments and enclosed payment, if you are paying by cheque or money order, to the Chief Firearms Officer of your province or territory. - Information Sheet Box 11 You are required to provide an official piece of identification from a federal, provincial, territorial, regional or municipal government. For example, if you use your provincial or territorial driver's licence, print "Driver's Licence" in Box 11 a). Examples of other types of identification you may use to prove your identity are: passport health card birth certificate social insurance card citizenship certificate landed-immigrant document Print the number of that piece of identification in Box 11 b) (for example, your passport number or driver's licence number). Print the name of the federal, provincial, territorial, regional or municipal government that issued the identification in Box 11 c). Do not send original documents or photocopies with your application. Only provide the information requested. D - Purpose Box 15 Put an "X" in the box that shows the reason you want a firearms licence. E - Safety Training Certification Box 16 Except for minors who need firearms to hunt or trap in order to sustain themselves or their family, you are required to take the Canadian Firearms Safety Course and pass the test. Print the city and province in which you passed the test and the date on which you took it. If you have proof that you passed the test (for example, a certificate or a copy of the course report that your instructor gave you), put an "X" in the box provided and attach a copy of the document to your application form. H - Fees Boxes 21 to 25 If you are applying for a minor's firearms licence under the Firearms Act for the first time, the fees are based on the number of years for which the licence is required: Up to one year $10.00 Up to two years $20.00 Up to three years $30.00 The fee is non-refundable. Indicate the method of payment. Do not send cash. Make your cheque or money order payable to the Receiver General for Canada. Please note: If paying by personal cheque, please allow a minimum of ten (10) business days for bank clearance. Administrative fees and any applicable interest will be applied to all dishonoured cheques. If you are not a resident of Canada and are paying by cheque or money order, please make your fee payable in Canadian dollars. Fee waivers If you require firearms because you hunt or trap to sustain yourself or your family, do not enclose a fee. The Chief Firearms Officer of your province or territory will assess whether the fee waiver applies.
