Message from our Chairman It may be no coincidence that the Phoenix Children's Hospital 17th Annual Golf Tournament is during the Thanksgiving season, for I am feeling extremely grateful as I chair the 17th playing of the event. First of all, I m grateful for the overwhelming support shown for this tournament. I ve been involved in many charity tournaments, but never have I seen such commitment from the community. On behalf of the patients and families of Phoenix Children s Hospital, we thank you for your support and for taking the time to participate in the tournament. I m also grateful for Phoenix Children s Hospital, a locally grown institution with a clear mission to provide hope, healing and the best healthcare to children and their families. It is also clear by your presence here today that you share in my gratitude. We have every reason to be thankful. Every day, children who are sick or injured enter the doors of Phoenix Children s Hospital, and they trust the hospital with their care, with their very lives. Phoenix Children s serves as the indispensable regional provider of high acuity care for patients with medically complex conditions like cystic fibrosis, muscular dystrophy, cerebral palsy, congenital heart defects and many other diseases. The number of medically complex 2

children is growing at a rate of about 6 percent a year. Since 2009 the percentage of patients that have either major or extreme severity of illness has risen from 20% to more than 33% -- a full one-third of the hospital s patients. If you want great outcomes for the sickest of the sick, you need the best people in a place that serves a broad geographic region, available to treat rare cases and conditions that many hospitals might not see once in a year. You need special technology and medical equipment at the ready. You need programs that allow kids to be kids, even while in the hospital. PCH arose from this community and serves this community. This simply would not be possible without the generous philanthropy of people like you. Your gifts make the difference. Since its inception, the tournament has raised over $5 million for the patients and families at Phoenix Children s Hospital! We all have much to be thankful for. Tournament Format and Rules TEES: Men play from the SILVER Tees Ladies play from the JADE Tees FORMAT: 4 Person Scramble RULES: Each player tees off and the best drive is selected and marked. The next shot is taken by each player from the marked spot. This process is repeated until the ball is holed, including putts on the green. Pace of play should not exceed 5 hours. Local Desert Rule If a ball comes to rest in the desert, the player may drop within two club lengths of the nearest point of grass relief, no nearer the hole, with a one-stroke penalty. Hole Location: Zone 1 Please turn in scorecard in the Pinnacle Ballroom upon completion of round. 4

Hole Contests and Prizes Hole #1 ATI Par 4 Everyone plays for free Contest to be determined. Prizes presented after the tournament in the clubhouse. Hole #8 Infiniti Hole-in-One Par 3 Everyone plays for free A hole-in-one on this hole will win a 2-year lease for a 2016 Infinity QX-50. Only the first shot counts for the contest, no mulligans allowed. Professional golfers are not eligible to win. Hole #9 Gateway Golf Group Ball Launch Par 4 Player Pays $10 to shoot their ball with the launcher Balls can reach the green with the launcher. Gateway will also be running a closest to the pin contest. The foursome proceeds to their shots. If one is closer to the pin than the marker, that golfer will move the marker and write their name on it. Hole #13 Arizona Coyotes Closest to the Pin Par 3 Everyone plays for free; Extra shots are $10 per player. The player wearing hockey gloves who lands their drive closest to the pin wins a prize from the Arizona Coyotes. Write your name on the post sheet if you are closer to the pin than prior golfers. No mulligans allowed for this contest. 5

Hole Contests and Prizes Hole #14 Forever Resorts Longest Drive Par 5 Everyone plays for free Men and Women compete separately to have the longest drive of the day. A prize is given to each winner. There are two card posts in each fairway, one for ladies, one for men. If your drive goes past the appropriate post, move the post to your landing spot and write your name on the card. Only the first drive counts for the contest, no mulligans allowed. Hole #17 Arizona United Soccer Team Eagle the Hole Par 4 $20 per Foursome Each paying foursome will take their drive and drop it anywhere behind the orange cone (approx. 75 yards from the pin) lying 0, which offers a good opportunity to score an eagle. After leaving the teebox, drive to the beginning of the fairway where the Arizona United Soccer Team representatives will have you take a free kick at a soccer goal. Those who score will be entered in a drawing to win an all-inclusive suite for a game, valued at $2500. Hole #18 Forever Resorts Longest Putt Par 4 Everyone plays for free The player who sinks the longest putt from a location on the green wins a prize. The putt is attempted from the team s actual lie following the approach shot. If you make a putt from further away than the marker, write your name and move the post marker. 6
