Shall - the instruction is compulsory. Should - the instruction is preferred but allows for initiative to be exercised. A JHA should be written for any variation in procedure or for the control of any new hazards that are identified. 3 points of contact: 3 points of contact maintained at all times Selected contact points are adequate to prevent a fall e.g. Good grip and foot placement. Driver faces the equipment whilst climbing up and down. Driver visually observes placement of their foot onto a step/ rung before lowering down to put weight on their leg. Definitions Driver holds onto railing/ ladder with hands between elbow and shoulder height. Driver removes excessive build-up of dirt from steps before climbing onto equipment e.g. Good housekeeping. Correct manual handling technique: Utilise suitable Personnel appropriate for the task- consider age, special skills, capabilities and knowledge. Determine a lifting leader who will provide verbal instructions during the manual handling task. Ensure all Personnel involved are warm and stretched prior to undertaking any manual handling tasks. Ensure lifting point, path and set down points are free from obstacles and that the floor surfaces are not slippery. Remember to keep your spine in a neutral posture throughout the manual handling task. to minimize the weight lifted by Personnel. Avoid jerky movements, side bending or twisting during the manual handling process. Ensure the load is kept as close to the body as possible with the load sustained below your shoulders but above your mid-thigh. PPE Requirements PPE requirements for this task are: High visibility reflective long sleeved shirt and long pants Safety glasses Gloves Safety boots Hard hat. Take 5 A Take 5 is to be completed every time a task is undertaken or the environment of operation has changed. JHA Prior to usage a JHA is to be completed to ensure the planned activities of using the shears is clear and the entire process of work to be conducted is stepped out. Contact with fluid under pressure Hoses and fittings are free of damage Prestart Excavator Excavator must be fitted with an approved roll-over protective structure (ROPS) unless specifically exempted by the appropriate statutory authority. Check fuel, hydraulic oil, engine oil, transmission oil, coolant and battery. Check condition and tension of tracks (refer to operator s manual to determine correct sag range). Inspect for worn or missing teeth or worn cutting edges. Check pins, bushes and connections for excessive wear and ensure they ve been greased daily. Check rams hoses and connections for splits, leaks or fractures. All fasteners are in good working order and tightened as required. Correct pressure. Maintained as per manufacturer s instructions. Safety decals are attached and legible. Slips, trip and falls Pinch Points Crush Injury Environmental Spills Maintain housekeeping at all times. Be aware of surroundings at all times. Maintain 3 points of contact. Take care when opening/closing bonnet. Wear gloves. Isolate equipment if entering footprint of vehicle; Never inspect inside of bonnet whilst the engine is running Ensure oily rags are disposed of correctly. If a spill occurs, use the spill kit. Set Up Work Area Establish 75feet (23mtrs) exclusion zone and place barricades / signs. Control exclusion zones through two way communication between operator and observer. Clear debris, wire, waste, tools or other potential tripping hazards. Develop site specific traffic management plan for roadside works i.e. barricades, warning signs, witches hats, traffic controller, high visibility clothing. Provide safe travel for pedestrians if required. Ensure pedestrians are not diverted onto roads. Slips, Trips and Falls Sprains, strains Electric shock Eyes on path. Keep work area clean and tidy. Correct manual handling technique Work area to be established outside powerline corridor (23mtr standoff) Connecting Shears Ensure attachment is level. With jaws of shear facing excavator, operate the excavator into position lining the stick up with the shear. Using the quick hitch mechanism attach the shear (re excavator user manual) Insert quick hitch safety pin and attach circlip Connect hydraulic hoses as required. Check correct flow and pressure (as per manufacturer). Remove non-essential persons from area. Operator to raise excavator arm slowly and test attachment is secured for use. Check for oil leaks. Pinch Points Slips, Trips and Falls Sprain, Strains Environmental Ensure the correct PPE is worn. Be aware of pinch points. Clear area of any trip hazard and Maintain good housekeeping at all times. Be aware of surroundings. Maintain 3 points of contact. Position excavator at suitable position for safe material handling and / or traversing to stockpile. 2. Open shear and position opening around material 3. Close shear to secure material 4. Lift material off the ground and close shear 5. Repeat steps 2, 3 and 4 until material is cut as desired 2 3 Loss of Control Roll Over Crush Only trained and competent operators to use machinery ROPS. Only trained and competent staff to operate Ensure ground is stable. Do not shear material in a position where it can fall on excavator. Never leave load suspended in air. Do not use the shear like a lever. Do not shear pressurized fire extinguishers or pressure vessels, waste chemical containers. Use caution not to hit shear against objects (including excavator Loading material 1. Continue cutting until a decent stock pile is achieved 2. Position the skip / trailer as close as possible to the stockpile to avoid unnecessary traversing 3. Using the cutting action without fully closing the shear pick up individual pieces of cut material 4. Position material over the skip / trailer and using the shear open movement release the material into position, do not drop the material 5. Repeat steps 3 and 4 stacking material neatly until the skip / trailer is full Traffic and People Crush 75feet (23mtr) exclusion zone, horn signals, Reverse alarm, Use radio communication and hand signals, Cease operation if approached, place attachments on the ground Do not shear material in a position where it can fall on excavator. Never leave load suspended in air. Do not lift loads over people, buildings or vehicles (including excavator). 3 4 On completion Lower the boom down so the shear is lying flat on ground. Idle engine for 5 min minimum to allow temperatures and pressures to stabilise. Turn off engine. Clean unit and inspect for any damage, leaks or faults. Report as required. If shear is to be detached Check whether hose pressure needs to be relived before detaching. Use caution, fluid is under pressure and hot. Using the quick hitch mechanism disconnect she shear Block shear up off ground. Grease shears Contact with fluid under pressure Slips, trips and falls. Strains/sprains (MSD) Burns (hot fluid). Release stored pressure in hose assembly and close valves Clear area of any trip hazard and Maintain good housekeeping at all times. Be aware of surroundings.
