REGISTRATION PROCESS Entry Form: Complete all sections AND sign liability waiver. SENIOR GAMES GENERAL INFORMATION Who: Adults age 50 years or better! Participants can live in other counties as our games are considered open games Athletic Events: Competition is among those of the same sex and participants will compete against those in their age groups: 50-54, 55-59, 60-64, 65-69, 70-74, 75-79, 80-84, 85-89, and 90+. (Age is determined as of 12/31/16.) Art Events: Competition is among participants according to sub-category (painting, photography, poems, etc.). Awards: Medals will be presented to 1 st, 2 nd, and 3 rd place winners in each athletic & art event at the closing ceremony on April 30. Each athlete or artist will receive one medal for each place. Additional awards will be in recognition only. Cancellations: Games officials reserve the right to cancel an event due to insufficient registration and/or combine age categories for competition purposes. Inclement Weather: Games officials reserve the right to cancel or postpone events due to inclement weather or unusual and extenuating circumstances. State Final Qualifiers: Athletics 1 st, 2 nd or 3 rd place. Golfers must place 1 st, 2 nd or 3 rd AND meet a minimum performance score to qualify for State Finals. LIABILITY WAIVER Required and must be signed Sporting Events Participants In consideration of being allowed to participate in any way in the Madison County Senior Games (athletics/ fitness/sports/arts) program & related events & activities, I, the undersigned agree that prior to participating, I will inspect the facilities and equipment to be used, and if I believe anything is unsafe, I will advise the coach, volunteer, or supervisor of such conditions and refuse to participate. I acknowledge and fully understand that I will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from my own actions, inactions, or negligence, but the actions, inactions, or negligence of others, the rules of play, or condition of the premises or of any equipment used. Further, I acknowledge that there may be other risks not known to us or not reasonably foreseeable at this time & I assume all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent disability, or death. I understand that to promote total health and physical fitness, events of longer duration and lower intensity are strongly recommended and that higher-intensity types of activities are offered for the conditioned, trained athlete. I release, waive, discharge, and covenant not to sue Madison County Senior Games, its affiliated clubs, their respective administrators, directors, agents, coaches, volunteers, and other employees of the organization, other participants, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event, all of which are hereinafter referred to as releases, from any and all liability to each of the undersigned, my heirs and next of kin for any and all claims, demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the release or otherwise. I agree that all press coverage of the Senior Games may be used for promotion of the Madison County Senior Games or for promotion of the North Carolina Senior Games. Anyone officially connected with Madison County Senior Games or with radio, television, or the newspaper may interview or take my picture for official use only with Senior Games. Finally, I agree that it is my responsibility to check with the local Senior Games Coordinator immediately following my Local Senior Games to determine if I qualified for State Finals and to get a State Finals Entry Packet. The State Finals Entry Form must be received in the NCSG office in Raleigh by 5:00 p.m. on August 1st. I understand this deadline is STRICTLY enforced in fairness to all, and understand it is MY responsibility to mail my entry form in on time. LIABILITY WAIVER Required and must be signed by Silver Arts Participants I, the undersigned, hereby relieve the Madison County Senior Games Program, NC Senior Games, Inc., their agents, sponsors, or representatives of any responsibility for the loss of or damage to item(s) entered in the Silver Arts competition of the Madison County Senior Games. I agree that all press coverage of the Senior Games may be used for promotion of the Madison County Senior Games or for promotion of the North Carolina Senior Games. Anyone officially connected with Madison County Senior Games or with radio, television, or the newspaper may interview or take my picture for official use only with Senior Games. SILVERARTS GUIDELINES 1. All persons entering SilverArts must be 50 years of age or better by December 31 st, a NC resident for a minimum of 3 consecutive months of the calendar year, and must qualify through a Local Senior Games SilverArts program for State Finals. 2. Entries must be the original work of the artist and must have been completed within 2 years of the Local Games qualifying date. 3. An art entry may only be entered in one (1) Silver Arts sub-category at State Finals. 4. Artists must not submit an entry if the piece/performance/entry has been showcased in the previous SilverArts Showcase at State Finals. 5. Art entry/entries incorrectly categorized by the artist, or not in compliance with NCSG Guidelines and Category Requirements, will not be considered in the judging process. SilverArts SPECIFIC CATEGORY REQUIREMENTS A. Heritage Arts Entries must be the work of the individual and not a group. Heritage Art entries must be an original work. Patterns may be used, but no stamped pieces will be accepted. Pottery must be hand built or thrown. Latch hook rugs and pieces from ceramic molds will not be accepted. All framed work must be securely wired, ready to be hung (no serrated hooks or string). Two dimensiona work not properly framed and wired will not be displayed. Framed works can be no larger than 36" x 42", including matting and frame. Entries can be no heavier than 50 pounds. All entries, except framed work, must be able to be displayed in an area 3'(W) x 3'(L) x 5 (Height). Groupings (sets) shall have no more than 3 separate pieces per set. All objects within each piece of the set must be contained within or attached to one of the 3 pieces of the set. Artist's name and title of entry must appear on, or be attached to, entry for identification. State Finals ONLY: Artist must send a photo of entry with State Finals Entry Form and SilverArts Information Form. Photos cannot be returned. Artists, or their representatives, are responsible to deliver and pick-up their entry at/during the designated times. The Heritage Arts entry entered at State Finals must be the same entry that won at the Local Games. An artist may only submit one (1) entry per sub-category at State Finals. B. Visual Arts All artwork, except sculpture, must be framed, securely wired, and ready to be hung (no serrated hooks or string). Artwork, except sculpture, not properly wired will not be displayed. Artwork, except sculpture, can be no larger than 36" x 42", including matting and frame. Entries can be no heavier than 50 pounds. Sculpture groupings (sets) may have no more than 3 pieces per grouping (set). Mixed media may be a freestanding piece. Freestanding entries must be able to be displayed in an area 3 (W) x 3 (L) x 5 (height). Artists, or their representatives, are responsible to deliver and pick-up their entry at/during the designated times. If on-site assembly is required, details must be specified on the Information Form for NCSG prior approval.

Silver Arts SPECIFIC CATEGORY REQUIREMENTS (Continued) C. Literary Arts All entries must be typed, double-spaced, on 8-1/2 x 11 paper and have a minimum of 1 margins. A minimum font size is 10-pt type (preferably Times New Roman or Arial ). Poems can be no longer than 40 lines. Short stories, essays, and life experiences can be no longer than 8 pages. All entries must have a title page, which includes title of entry, sub-category, and name of artist. Artist's name must not appear on the content pages. A display-quality copy of the winning Literary Arts entries, and two (2) additional copies, should be submitted. No framed or matted entries will be accepted. The original and all copies will be retained by NCSG. The literary arts entry entered at State Finals must be the same entry that won at the Local Games. An artist may only submit one (1) entry per sub-category at State Finals. l
