Description Of Work: Sequence of Basic Job Steps Potential Hazards Controls & Recommended Actions Verify that the input breakers to the high energy power supplies are locked off 480kW White Circuit Bias 480kW White Circuit Pulser B2-B6 Mains Copley bulk supply None Verify that the keys to the above locks are captured in the Booster Lock Box in Building 140 None Apply Lock and Tag to the Booster Lock Box in Building 140 None Verify that the high energy power supplies cannot be turned on 480kW White Circuit Bias 480kW White Circuit Pulser B2-B6 Mains Copley bulk supply Potential of arc flash if The inputs to the supplies are energized A bolted fault short occurs at the supply while the attempt to turn on the supply is made Attempt to turn on the supplies remotely, if possible. Set the supply to remote turn-on One worker attempts the remote turn-on from the control panel while another worker observes the supply from a distance of greater than 4 feet (NFPA 70E, 130.3(A)) to verify that the supply does not turn on. If the supplies have not been designed to remotely turn-on, wear the appropriate PPE for arc flash hazard. In the case of the three large power supplies 480kW White Circuit Bias 480kW White Circuit Pulser B2-B6 Mains one should wear PPE appropriate for Hazard/Risk Category 1. For all other supplies, the PPE should be appropriate for Hazard/Risk Category 0. Field repair/warning signs of jagged edges. Carefully pull rack away from installed position and rotate so that verification of zero stored energy can be made; and to disconnect the wiring to the Copley chassis so that it can be removed from the rack. Verify that there is no stored energy in the supplies by verifying that the output busses TB5 and TB6 (schematic SD- 444-254-33) of the supplies are at 0V, using the appropriate meters and PPE Copley bulk supply Removal of Copley Driver chassis from rack. Foot Crushing Avoid placing feet under any possibly crushing load. Shock hazard if Restricted Approach Boundary is crossed without appropriate PPE Flash hazard if Flash Protection Boundary is crossed without appropriate PPE Struck by falling chassis Back or other strain injury from Meters should be rated to measure 600V For shock hazard Avoid contact with other supplies Remain greater than 1 in away from the other supplies PPE should be appropriate for Hazard/Risk Category 0 Get help or use lifting tools Use proper lifting techniques (Avoid twisting while lifting, use your knees, etc.) overexertion. Chassis removed from service. None Inform SPEAR Operations of chassis removal. Re-installation of Copley Driver chassis. Carefully rotate and position rack for bolting rack back down to floor. Sign appropriate documentation to certify that this procedure has been followed. None Struck by falling chassis Back or other strain injury from overexertion. Get help or use lifting tools Use proper lifting techniques (Avoid twisting while lifting, use your knees, etc.) Foot Crushing Avoid placing feet under any possibly crushing load.
