DEDHAM PEE WEE FOOTBALL ASSOCIATION, INC. (DPWFA) Adult Behavior: PARENT CODE OF CONDUCT 1. At any DPWFA or American Youth Football & Cheerleading event, practice or competition, any adult who: 1) verbally abuses; 2) attempts to intimidate; 3) is flagrantly rude, or, 4) cannot control their language or actions with an official, coach, or American Youth Football & Cheer volunteer will be asked to leave the event. He or she will receive a written warning regarding their behavior. His or her child(ren) will be immediately removed from the event. 2. Any adult that commits a second similar offense will be banned from DPWFA and American Youth Football & Cheerleading events for the remainder of that season and their child(ren) removed from DPWFA for the remainder of that season. No refund will be issued. 3. Any adult who physically assaults an official, coach or American Youth Football & Cheerleading volunteer will be banned from DPWFA and their child removed from the DPWFA program for one year from the date of the offense. The child(ren) may not participate in another American Youth Football & Cheerleading association during the sanction period. After one year, the parent may apply for reinstatement of his or her child. If the adult commits a second offense, he or she will be permanently banned from DPWFA and the child(ren) permanently removed from American Youth Football & Cheerleading. The term physical assault includes, but is not limited to: hitting, slapping, pushing, spitting, kicking or striking in any way with any part of the body or any physical implement. (DPWFA) 2018 Season Football & Cheerleading Uniform & Equipment Agreement I understand that I am responsible for the care and return of the football / cheerleading uniform to DPWFA at the end of the scheduled season. To keep the costs of this program down, it is important that all uniforms be returned to us as soon as possible after the end of the season. Uniforms not immediately returned have a tendency to never be returned. To have enough equipment for next year's cheerleaders / football players, DPWFA must buy replacements of all items not returned. At the end of the season at a schedule date, I will return to the Equipment Manager / Cheer Coordinator, or my child's coach, all items issued to him / her. We ask that you please clean the uniform before it is returned. In addition, I agree to pay Dedham Pop Warner the replacement cost of any item(s) not returned by 12/31/2018, according to the Dedham Pop Warner 2018 Equipment Price List. Players / Cheerleaders will be ineligible to participate in the 2019 season if all equipment and uniforms are not returned or paid for by 12/31/2018. DEDHAM PEE WEE FOOTBALL ASSOCIATION, INC. (DPWFA) 2018 Season Football Refunds/Team Placement Form (FOOTBALL RETURN ONLY!) Please note that DPWFA s complete refund policy can be found in our 2018 Standard Operating Procedures & Policies which can be downloaded at www.dedhamyouthfootball.com. This form only pertains to refunds associated with player placement. The Dual County Tri Valley League (DCTV) is a member of American Youth Football (AYF). DCTV is not part of AYF s National Division and therefore teams are created based upon grade. Players who register to play will be assigned to the team for which grade they will be entering in the 2018-2019 school year (3 rd grade, 4 th grade, etc). The only exception will be for a player who has stayed back in school and is older than the protected age for that grade level. Those players will be placed on a higher grade team playing with others their age. DPWFA and the coaches will be responsible for player placement. If a particular grade team does not have the minimum of 16 rostered players by August 1 st, DPWFA has the option to combine that grade with the next highest grade. For example, if only 7 players are registered to play for the 5 th grade team, we may cancel the 5 th grade team and move them up to play on the 6 th grade team. Also, if there are too many players at a particular grade and the grade just above them needs additional players to make a team, DPWFA may move enough players from one grade to the next to give that team enough players. For example, if 28 players sign up for 7 th grade and only 10 sign up for the 8 th grade, DPWFA may move at least 6 players from the 7 th grade team to play on the 8 th grade team so they have the 16 players required to have a team. Those players moved up will be determined by DPWFA and the coaches. We will only move up players who we believe are physically and mentally able to perform at that level. Refunds will not be issued to anyone who feels as though their player should not be on a particular team for any reason.
