MISSION STATEMENT To provide all players with a world class basketball experience by placing an emphasis on player development. A lley-oop 360 Club will seek to have all kids reach their full potential by combining world class training, coaching, and game play to maximize results. We will emphasize that academic development is as important as athletic development. We strive to make all players great basketball players and great members of the community. We will provide great coaches that know the game, perfect the fundamentals, and will create an environment that is fun and competitive. The players will get an opportunity to perfect their skills by playing league games and will have an opportunity to compete at high levels by participating in tournaments. The players in Alley-Oop will have a lot of game exposure to allow them to perfect their craft. 1

Benny Hoang played and started 4 years Varsity at Eagle Rock High School. As a freshman at Eagle Rock High School earned All-League Honorable Mention accolades. He also earned All League Selection for Four consecutive years. As a High School Senior, earned First Team All 3A-City Honors. CLUB DIRECTOR PROFILE BENNY HOANG CLUB DIRECTOR After playing high school basketball, he attended LA Valley College. As a sophmore campaign, earned Co-MVP All Western State Conference honors and Honorable Mention All -State JUCO accolades. After JUCO, Benny attended and received a Full Scholarship playing for Cal State LA Golden Eagles where he led the team in free throws percentage of (85.7%). Led his team in Assist his Jr Season. Also, came in 2nd for Most Consecutive Free Throws Made ( 11 for 11)against the Eventual National Champions UC Davis on 11/22/1997. BENNYHOANG@ALLEY-OOP360.COM BRYAN GRAY ALLEY-OOP 360 FOUNDER Benny enjoys helping kids develop and build confidence through basketball. It's all about development and empowerment in providing kids an opportunity for basketball and life success. There should be "No Kids Left Behind for the opportunity of playing basketball". This is why he wanted to help the Founder of Alley Oop, Bryan Gray, in setting up the Club Basketball through Alley-Oop 360 Club. Benny currently resides in Cypress, California. He has two boys currently playing Club Basketball and Alley Oop Leagues. 2

PROGRAM DESCRIPTION This registration is for the Alley-Oop 360 Club program. Players will have an option to commit for 1 year or 6 months. By joining Alley-Oop 360 Club our commitment to the parents and the participant will be: To provide a world class experience. We will provide a committed coach for the team. The coach will be committing to developing all players and work to ensure they reach their full potential. We will achieve this objective through providing great practices that will stress fundamentals, improve individual and team skills. The players will learn how to play the right way. The players will be applying what they learn by playing games on a consistent basis. The players will play league games that will be focused on development and tournaments that will be focused on winning. Player / Participant Commitment: In return, we expect players to bring their best effort and a positive attitude to every practice, training session, game, and tournament. We expect players to be fully committed and to not miss activities and to exhibit great sportsmanship with their teammates and coaches. You can pay all at once (for a 10% discount) or on a monthly basis. The price per month is $225. When you register you will have the option to pay all at once or select the payment plan. 3

THE ALLEY-OOP 360 CLUB PROGRAM Enjoy a complimentary session. The first session is a try-out to give our program a try. We are confident once you experience the program you will want to join the club. ENROLLMENT FEE: ANNUAL DUES: $225 $2400 If paid up front, enrollment fee is waived plus 10% discount (full price without discount $2700) MONTHLY INSTALLMENTS: $225 /month ENROLLMENT FEE INCLUDES: Locks in your spot on the Alley-Oop 360 Team Custom designed shooting shirt Practice Jersey Custom backpack Custom designed Home and Away uniform MONTHLY DUES INCLUDE: 2 Team Practices per week for 1.5 hours 1 Tournament per month 1 Team registration into each Alley-Oop League per season 4

TOURNAMENTS I ncluded in Alley-Oop 360 Club are planned tournaments schedule for the 2018 season, in which the indicated/assigned teams will play, on average, in one tournament per month. We make every effort to preschedule tournament dates so that families can adequately plan their schedules well in advance. Please understand that changes to the schedule may occur, and that we will make every attempt to provide timely notification of any changes. The cost of team registration for tournaments are completely covered in the club dues; associated costs NOT covered in the club dues include (but are not limited to): travel to and from tournaments, parking and spectator admission, and for out-of-town tournaments, travel expense, hotel accommodations and food/meals. PER DIEM FOR OUT-OF-TOWN TOURNAMENTS A per diem fee will be assessed at the beginning of the season in the Payment Schedule to cover the coaches cost of travel, accommodations and meals for the out-of-town tournament listed on the Alley-Oop 360 Club website. Additional per diem fees will also apply to any teams traveling for AAU Nationals, and will be assessed once attendance/registration in the tournament(s) is confirmed. Per diem fees are calculated based on a full team and will not be refunded to players who decide not to travel to these tournaments with the team. The 2018 season per diem fees are $100.00, and are not included in your club dues. 5

OFF-WEEKEND TOURNAMENT PROGRAM In order to provide our players I with more opportunities for "in game experience," we have implemented an off-weekend tournament ( OWT ) program. This optional program will allow your child to play more weekends during a given month (that is, on the weekends that are part of our regular schedule, as well as on the off weekends), if desired. An email will go out at the beginning of the month with upcoming tournament dates and a link to register. If you would like your son or daughter to play on an off weekend, you will need to respond to the email no later than the deadline indicated in the email. We will take players on a first-come, first-served basis, and will only allow up to 9 players to be on the team. This is to ensure that participating players all receive an adequate amount of playing time. The number of OWT s offered each month will vary between 1 and 3 (generally 2 on average). There will be an additional fee to play in these off-weekend tournaments. The fee will be $40 per player per tournament. This fee covers the tournament registration and coach stnd. Payment for the tournament will be due upon signing up your son or daughter to play. 6

PARENT CODE OF CONDUCT I agree that: I will not force my child to participate in a Alley-Oop 360 Club program practice, clinic, or game. I will refrain from coaching my child or other players during games and practices. I will respect the officials and their authority during games and will never question, discuss, or confront coaches at the game, and will take time to speak with coaches at an agreed upon time and place. I will commit to have my child attend every practice and game. In the event of a emergency or illness I will inform the coaches of an absence. I will demand a sports environment for my child that is safe and free from drugs and alcohol and I will refrain from their use at all sports events. I will promote the emotional and physical well-being of the my child athlete ahead of any personal desire I may have for my child to win. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence I will demand that my child treat other players, coaches, officials and spectators with respect regardless of race, creed, color, sex, or ability I will teach my child that doing one s best and exhibiting good sportsmanship is more important than winning. I understand that mistakes are part of the game and will not ridicule or yell at my child or other participants for making a mistake or losing. I will inform the coach of any physical disability or ailment that may affect the safety of my child or others. I will learn the rules of the game and the policies of the program. My guests and I will be positive role models for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players. 7

WAIVER AND ASSUMPTION OF RISK In consideration of being allowed to participate in any way in the above listed program, related events or clinics, and activities, I, the undersigned, acknowledge, appreciate and agree that: The risk of injury from the activities involved in these sport events is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist. I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation. I willingly agree to comply with the stated instructions and policies and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately. I for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify and hold Alley-Oop Youth Basketball, LLC and their officers, agents and/or employees, other sports participants, or sports facilities (AIM Sports Plex, The MAP Sports Facility, etc ), sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event or activity harmless with respect to any and all injury, disability, death, or loss or damage to person or property, to the fullest extent permitted by law. 8

PHOTOGRAPHY By participating in the Alley-Oop 360 Club program or any related clinics or activities, I am also agreeing to having my child be photographed, interviewed and or video taped by Alley-Oop Youth Basketball, LLC approved sources such as photographers, videographers, or any other media outlet for use in promotional materials, brochures, pamphlets, websites, etc to help promote the program. I also agree that no monetary payment will be made for use of any and all of these photographs/videos taken. POSTING OF PHOTOGRAPHIC IMAGES A lley-oop 360 trusts its parents will use good judgement when posting on social medial. We ask that the parents only post items that show Alley-Oop 360 Club or other club programs in a positive light. We also ask you refrain from posting any negative social media and refrain from using names of children or adults. If at anytime you have questions on what is acceptable to post please contact any Alley-Oop representative for assistance. OPEN DOOR POLICY A lley-oop 360 understands that its participant players and families are vital to the success of the organization. We value our participants feedback and will make every attempt to accommodate a open discussion at anytime; UNLESS it is right before, during, or right after a game. Typically those types of discussions become very emotional and the outcomes are rarely positive. So we ask that you wait one full day after a game or event so that a constructive discussion can take place. 9
