Tempe Preparatory Academy Chapter of the National Honor Society Student Application Form 2014 Due: September 5 to Mrs. Moffitt in Room 508 Directions: Please complete all sections. Type or print all information and submit it by the due date above. Do not be modest. Every bit of information will be used by the Faculty Council to assist with the fair consideration of your candidacy during the selection process. Completion and submission of this form does not guarantee selection. Should you have questions about this form, please contact Mrs. Karin Moffitt, TPA Academic and College Counselor at kmoffitt@tempeprep.org. Requirements to Be Invited to Become a Candidate for membership in NHS (due September 5): 1. Scholarship: Students eligible for selection must be members of the Junior or Senior class and should have a minimum cumulative grade point average (GPA) of 3.0 by the end of the prior school year. 2. Leadership: Significant and active participation with leadership responsibilities in 2 or more extracurricular activities (including school and community activities). 3. Service: 10 or more hours for each of the prior school years must be completed and recorded in x2vol.com. 4. Character: No Academic Honor Code violations, no more than 4 detentions (or 2 Saturday detentions), and zero suspensions in the prior or current school year. 5. Pay annual membership fee of $20.00 due September 5 with application (cash or check payable to TPA ). Requirements to be Inducted as a Member of NHS and Maintain Your Membership (due December 13): 1. Scholarship: Maintain a minimum cumulative Grade Point Average (GPA) of 3.0 at the end of each semester. 2. Leadership: a. Attend a minimum of 6 NHS meetings each semester (approximately 8 meetings for each semester) and serve in a leadership role for at least one NHS activity. b. Significant and active participation with leadership responsibilities in 2 or more extracurricular activities (including school and community activities) each year, including Senior year. c. Take leadership responsibility or coordinate at least one NHS group activity in the academic year. d. Attend at least one TPA Board Meeting (see school calendar for dates). 3. Service: Complete a minimum of 15 community service hours must be completed and recorded in x2vol each semester (by December 10 for the fall semester and May 1 for the spring semester), a. All members are required to participate in at least 5 hours of TPA NHS Chapter sponsored group service projects. b. The 10 additional hours may be in TPA NHS Chapter sponsored group service projects or other individual service projects that comply with TPA community service guidelines and selected by members according to their own talents and interests. c. All service hours must be recorded on x2vol within 7 days (1 week) of the service performed. d. Service must include at least one service experience at each of the following by December 13: i. Serve on a jury for Teen Court. ii. Serve the homeless at St. Vincent de Paul, Paz de Cristo, Child Crisis Center, or St. Mary s Food Bank. iii. Pack food for international relief at Feed My Starving Children. e. All service hours must be recorded on x2vol within 7 days (1 week) of the service performed. 4. Character: No Academic honor Code violations and no more than 4 detentions (or 2 Saturday detentions) and zero suspensions in the prior or current school year. 5.
