CODE OF CONDUCT In an effort to provide a positive, fair and professional competition environment, The Orlando Magic has implemented a Code of Conduct for all events. This Code of Conduct is to be followed by all coaches, competitors, family, friends or anyone else associated with the below stated program. RULES/REGULATIONS/SPORTSMANSHIP 1. I have read, understand and will abide by all Orlando Magic rules and regulations. 2. I understand that competitive cheerleading and dance is an activity that presents the possibility of injury. As a coach who understands this risk, my athletes have received proper progression training. 3. I understand all risks associated with competitive cheer and will always place more emphasis on the safety of my athletes than on competitiveness. 4. I will follow all Orlando Magic, USASF and AACCA rules and regulations. I understand that any violation of these rules and regulations could result in a penalty point deduction, or, disqualification. 5. I will abide by the age and/or grade requirements (USASF/AACCA Guidelines) for all divisions that my teams are competing in. I understand that I will be required to provide proper age and/or grade verification in the event that the age of one of my athlete s is questioned. Falsifying age and/or grade requirements will result in disqualification from competition. 6. I will accept all judges decisions as final and not question the integrity of said judges. If there is a valid protest, the head coach may file a complaint to The Orlando Magic. At no time may anyone other than the head coach file a complaint. If, for valid purpose, I register a complaint, I understand that this complaint will be addressed after the competition. 7. I understand that any action taken by a parent, family member, friend or competitor in approaching an Orlando Magic staff member or judge will result in a penalty point deduction as well as forfeiting the coach s right to protest or review tape. 8. At all times, I will support the results of the competition as I am a role model for my program. 9. I will promote fairness and demonstrate appropriate sportsmanship throughout the competition, as well as require all persons affiliated with my program to conduct themselves with the same appropriate sportsmanship. This includes if a division of mine is combined with small teams and a large or vice versa. I am for the spirit of the competition. 10. I understand that The Orlando Magic reserves the right to remove any persons from a competition for unsafe or unsportsmanlike actions. Force Majeure: The Orlando Magic shall not be liable for failure or other delay in performance of obligations under any agreement to the extent such failure or delay is due to circumstances beyond its reasonable control, including, without limitation, acts of God, acts of public enemy, fires, floods, war, civil disturbance, participation, sabotage, lock-outs, labor disputes, acts of any governmental or quasi governmental authority, theme park, or any other force majeure. As the coach of the cheerleaders/dancers, I agree to the above guidelines. Head Coach Signature Date 2

ALL STAR TEAM ROSTER About the USASF The US All Star Federation (USASF) was founded in 2003 with the core principle of making All Star a safer sport by establishing fair and consistent rules and competition standards. The organization credentials coaches, certifies safety judges, sanctions events and maintains and adjusts (as needed) safety guidelines, all with the goal of providing the safest possible environment for cheer and dance athletes to train and compete. We are a not for profit corporation established in Tennessee and are governed by Bylaws, officers, a Board of Directors, and 15 standing committees. The day to day operation of the USASF is handled by full time, part time, and volunteer staff. Athlete Membership Starts August 1, 2014 A new, comprehensive and streamlined Athlete Membership program will begin with the 2014-15 membership season. An annual Athlete Membership fee of $30 for all athletes will begin August 1, 2014. Athlete ID is EASY Step 1: Prior to attending your first event in the 2014-2015 season, register all cheer and dance athletes by entering information in member profiles at USASF.net. Athlete birth certificates will be uploaded to USASF secure serves for birthdate verification by USASF staff. Once birthdates are verified, birth certificate files are digitally destroyed and not stored on any server. Step 2: Create and print USASF Official Event Rosters for each team prior to each event and submit to event producers at the event. 6
