Teeny Weeny Program Details Welcome to the Florida Top Dog All-Stars! We are honored that you have selected our program and we welcome you to our Ohana! Our mission at Top Dog is to continue to strive to be one of the top competitive cheer programs in the country along with instilling positive characteristics that your child will take with him/her into his/her future. Our goal each day is to teach our athletes the value of commitment, teamwork, integrity, leadership, self-confidence and to allow them to create memories that will last a lifetime. Our Teeny Weeny Fundamental Program is designed to provide a strong foundation for ages 5-8 year olds in the basics of cheerleading and tumbling, while making it fun and building confidence. The Teeny Weeny program includes one cheer class per week and one tumbling class per week. The cheer class will focus on jumps, stunts, motions. Coaches will choreograph a short routine that the athletes will perform by the end of the session. Program Checklist o Register online under the Top Dog All-Star Button for the Teeny Weeny Program and pay $40 fee. o Download the packet under the Exhibition Tab. Fill out the forms listed below and bring to the first practice along with the monthly fee of $85 o Medical Release Form and Waivers o Cheerleader Info and Application o Parent Obligation and Financial Agreement with credit card info. The weekly Cheer Class will be Wednesdays 6:30-7:30 p.m. Tentative Tumbling Class Options: Monday 6:00-7:00 p.m. Little Puppies 2 Tuesday 5:00-6:00 p.m. Little Puppies 2 Thursday 5:00-6:00 p.m. Little Puppies 3 Thursday 6:00-7:00 p.m. Little Puppies 2 Parents will need to create an account online under the Just Tumblin Portal in order to attend the tumbling class as this is where we keep track of tumbling class attendance. We are updating our Little Puppies Tumbling Classes in August so some times may be adjusted for tumbling options.

Top Dog Program Costs Parents will need to create an account and register their child through our Top Dog All- Star Parent Portal. Please make sure you register for Teeny Weeny Team. We have limited space so it will be on a first come first serve basis. Teeny Weeny registration is $40 and must be paid online when you register. We will be offering 2 sessions for this program. Parents can opt to do one or both sessions. This is a committed team for each session. Please do not sign up and then decide your child does not want to do as we usually end up with a waiting list for this team. All fees will be paid directly to Florida Top Dog All-Stars. In doing this, each cheerleader has the opportunity to raise 100% of his/her share of participation expenses. All funds turned in are considered sponsorship and donations to the Florida Top Dog All-Stars; therefore if a cheerleader quits or is dismissed from the team, no money will be refunded nor can money be requested to transfer to another cheerleader. Participation Costs: $85 monthly fee to be paid on the first of the month. ALL Payments must be paid through the online portal with your credit card. No payments will be taken at the front desk. If your fee is not paid by the 1 st then on the first we will charge your cc on file. The $85 includes one practice a week and one additional tumbling class per week. Top Dog Program Costs Financial Information and Payment Policy You will be paying monthly dues that will be deducted from your credit/debit card at the beginning of each month. All team members must have a Credit Card Authorization Form on file for their account. This form will be due with the tryout packet and returned if the athlete does not make the team. All Top Dog fees will be billed on or by the 1st of each month. All fees must be paid in full on or by the 1 st of each month. On the 1st of each month, any outstanding balances will be charged to your credit card. Once your child has been placed on a team, you must go online to your parent portal and input your payment information. Select manage payment options and choose how you want your fees to be paid. Your credit/debit/e-check numbers will be stored and drafted on the 1st of the month through your online account. Auto-pay is not saved on our computer nor is it saved within iclass pro. It is saved by the Payment Gateway Program. Any payments resulting in NSF, declined credit card or expired credit card will incur a $30 service charge. Return checks and fees must be paid in cash. It is your responsibility to update your information if your information changes. An athlete s account must be current and in good standing in order to participate in practices, private lessons, competitions, and special events. Florida Top Dog reserves the right to remove any athlete from his/her team for failure to keep up with financial obligations. All accounts must be current before an athlete may collect any clothing/uniforms, or other retail items. If an athlete chooses to leave or is asked to leave for any reason before the season is over, any and all funds are non-refundable. Gym fees cannot be pro-rated and will not be adjusted during times the gym is closed, vacation days, practices cancelled, etc. Page 2 of 2

Top Dog Parent & Athlete Policies 1. Attendance is crucial to team success. An absent request form is available online and must be filled out prior to the absence. 2. Encourage good sportsmanship by being an example. Be positive in communicating with your teammates. We will not tolerate bad mouthing other all-star programs, teammates, coaches etc. from our athletes and/or parents. 3. No jewelry should be worn during practices and competitions. Cell phones should be put away. 4. We are not responsible for lost or stolen items. 5. All funds turned in are considered sponsorship and donations to the Florida Top Dog All-Stars; therefore, if an athlete quits or is dismissed from the team, no money will be refunded nor can money be requested to transfer to another athlete. 6. Most of our communication is done through email, team facebook pages, and first team touch. Please check this regularly. 7. You cannot create or sell your own Top Dog clothing using the Florida Top Dog Name, Logos, or specially designed team logos. 8. Due to the busy nature of our facility and programs, we ask that athletes do not arrive more than 15 minutes prior to their scheduled practice/class time unless they have permission from their coach to come in early to work on necessary skills for their routine.
