Statement of intent HEALTH AND SAFETY AT WORK ACT 1974 THIS IS THE HEALTH AND SAFETY POLICY STATEMENT OF INTENT FOR: Evolution Media Installations Ltd The Directors of Evolution Media Installations Ltd believes that health and safety is an important issue and therefore, provide and maintain working conditions, equipment, and systems of work which will protect the health and safety of all its employees and others who may be affected by its operations and ensure legislative obligations are met. Adequate resources, including money, time and any other requirements, will be allocated to meet this objective. The Director fully accept their responsibilities to their employees to ensure, so far as reasonably practicable, their health, safety and welfare at work, and any other persons who maybe affected by Evolution Media Installations Ltd s work operations and to commit to the below statements. Evolution Media Installations Ltd will monitor and inspect all work places by using a competent advisor as required under section 7 of the Management of Health and Safety at Work Regulations 1999. - To provide adequate control of the health and safety risks arising from work activities and assess all hazards and associated risks; - To consult with our employees on matters affecting their health and safety; - To provide and maintain safe plant and equipment; - To ensure safe handling and use of substances and articles; - To provide information, instruction, training and supervision; - To provide safe place of work and access and egress; - To provide safe working environment and adequate welfare facilities; - To prevent accidents and cases of work related ill health; - To maintain safe and healthy working conditions; - To review and the health and safety policy annually or when significant changes take place. The control of the risks associated with the work done by Evolution Media Installations Ltd can only be achieved by the proper use of risk assessments. Risk assessments will be carried out for all work done by Evolution Media Installations Ltd. They will be reviewed at regular intervals. Duties under fealth and safety In addition to His duties as administrator is also the Health and Safety Manager has additional responsibilities for: (1) Administer and interpret the effective implementation of Evolution Media Installations Ltd policy document. (2) Understand the application of the Health and Safety at Work etc Act 1974 and all other legislation relevant to Evolution Media Installations Ltd s business. (3) Providing a company statement on any health and safety issues that require one. (4) Investigate all accidents and ensure compliance with RIDDOR 1995. (5) Arrange for funds and appropriate facilities to be available to meet the requirements of the policy. (6) Ensuring that regular safety inspections of work places are carried out to check that the requirements of this policy are observed. (7) Advising the Employees of any issues affecting or likely to affect Evolution Media Installations Ltd and through the Employees to promote a pro-active approach to positive health and safety. (8) Ensuring there is adequate health and safety advice awareness and training at all levels of Evolution Media Installations Ltd. (9) Discipline any member of staff failing to comply with the requirements of the policy (10) Ensuring all risk assessment details are passed to Employees prior to the commencement of any works and ensures all necessary equipment for safe working is available. (11) Good housekeeping and fire safety on site. (12) Set a personal example at all times with respect to health and safety (13) Have sufficient knowledge of the required regulations, legislation and codes of practice. (14) be competent in the maintenance of the relevant statutory records and registers. (15) Ensuring that there are adequate means of distributing and communicating health, safety and welfare information, from the HSE, safety organisations, and new methods of accident prevention, new legislation and codes of practice. (16) Organising an on-going programme of training for all employees. (17) All arrangements and requirements relating to Waste Management including monitoring and reviewing. (19) In addition to their general duties they will also with regard to site health and safety, organise the workplace so that work carried out is to a satisfactory standard of safety. (21) Ensure that control measures in place are at all times operable and not interfered with (22) Be responsible for all company employees and contractors on site complying with company and Clients safety rules and practices. (25) Ensure that work on site follows Evolution Media Installations Ltd policy and procedures so that the work is carried out with the minimum risk to operations, other contractor s members of the public both during and outside normal site hours. (26) Ensure that all equipment, plant and equipment required for safe working is present on site and being correctly used. (27) Inspections of work place and monitoring of health and safety performance on work site. (28) During site inspections ensure that actual site conditions are adequately covered in all health and safety documentation and procedures.

(29) Prepare risk assessments and ensure details are passed to Employees prior to the commencement of any work and ensure all necessary equipment for safe working is available. (30) Site risk and COSHH assessments. (31) Liaise with clients and their representatives to ensure good co-operation and co-ordination on health and safety issues. (32) Ensure Evolution Media Installations Ltd health and safety policy is brought to the attention of all employees particularly new starters. (33) Toolbox talks. Duties of all employees The Health and Safety at Work at 1974 requires all employees to take reasonable care of themselves and others who may be affected by their acts or emissions at work; and to co-operate with Evolution Media Installations Ltd by observing its rules and procedures and so enable Evolution Media Installations Ltd to comply with its statutory duties. Use all items provided by Evolution Media Installations Ltd correctly in accordance with their training and the instructions they received to use them correctly. Report accidents/incidents, which have lead or may lead to damage to plant or equipment. Suggest ways of eliminating hazards. Set a personal example, especially to junior members of the workforce. Report all plant, equipment and tools that are defective. Wear PPE correctly, and report any defects immediately to their supervisor. Health and safety arrangements Evolution Media Installations Ltd recognises that to be successful in health and safety management they must identify the relationship between controlling risks and general health. Our ultimate goal is a company in which accidents and ill health are eliminated, and work forms part of a satisfying life that contributes to physical health and wellbeing, to the benefit of both the individual and Evolution Media Installations Ltd. Evolution Media Installations Ltd believes in an integrated approach, in which the needs of the people and the prosperity of Evolution Media Installations Ltd can go hand in hand, in policies for the control of risks, environmental pollution and safety. Planning To reduce risks in company operations, plans and performance standards will be set with the intention of eliminating and controlling risks. Evolution Media Installations Ltd will establish, operate and maintain planning systems which: - Identify objectives and targets for their achievement within a specific period. - Set management performance standards designed to initiate, develop, maintain and improve a health and safety culture in control, competence, communication and co-operation. - Set performance standards for risk control based on hazard identification and risk assessment, where legal requirements are the minimum acceptable standard of performance which emphasise: - The elimination of risks by substitution of safer premises, plant or substances and where this is not reasonably practicable. - The control of risks by physical safeguards which minimise the need for employees to follow detailed systems of work or to use protective equipment. - Establish priorities for the provision and maintenance of control measures by using risk assessment techniques, giving priority to high risks and using temporary control measures to minimise risks where satisfactory control cannot be achieved immediately. - Set performance standards that control risks to employees and others who may be affected by Evolution Media Installations Ltd s activities, products and services. - Ensure the adequate documentation of all performance standards. - Monitoring Evolution Media Installations Ltd will measure their health and safety performance against pre-determined plans and standards, Evolution Media Installations Ltd operates and maintains systems which ensure that their performance is measured objectively by active monitoring systems which: - Measure objectives and specified standards. - Reflect risk control priorities by concentrating on high risk activities which are monitored in more depth and/or more frequently. - Reactive monitoring systems which collect and analyse health and safety information, including systems for reporting:

- Injuries and cases of ill health. - Other loss events. - Incidents. - Hazards. - Weaknesses or omissions in performance standards. - Report and response systems that ensure evaluation of information by competent people to identify situations which create an immediate risk to health or safety and ensure that appropriate remedial action in investigation systems which ensure: Reviewing - The investigation of reports arising from active and reactive - Monitoring, with priority given to those circumstances which present the greatest risk. - Identification of both the immediate and the underlying causes of events. - The referral of information to management with authority to initiate the necessary remedial action, including organisational and policy changes. - The analysis of collected data to identify common features or trends and initiate improvements. Evolution Media Installations Ltd uses reviewing to maximise learning and ensure that appropriate action is taken to improve the control of specific risks. To improve health and safety overall, including performance and development of policies. These reviewing systems ensure that: - Evolution Media Installations Ltd can develop its health and safety policies and improve the control of risks. - Auditing the whole health and safety management planning and control system for validity and reliability. The overall effectiveness of the policy implemented is assessed in relation to: - The assessment of the degree of compliance with health and Safety performance standards. - Identify areas where standards are absent or inadequate. - The assessment of achievement of specific objectives. Accident, ill health and incident data, together with analyses of immediate and underlying causes, trends and common features. Company duty to consult and provide facilities and assistance Evolution Media Installations Ltd encourages the employees safety representatives to take an active interest in matters that concern them, relating to health and safety and ensure they consult with these representatives in good time in regard to: (a) (b) (c) (d) The introduction of any measures at the workplace, which may substantially affect the health, and safety of its employees, the safety representatives concerned represent. Advising them in regard to the persons responsible in Evolution Media Installations Ltd for health and safety and fire and emergency evacuation procedures. Any health and safety information, including planning and organisation training, relating to employees the safety representatives concerned represent. The health and safety consequences for the employees by the introduction of new technology (including planning), the safety representative s concerned represent. Evolution Media Installations Ltd provides facilities and assistance to safety representatives for the purpose of carrying out their functions. Safety culture Evolution Media Installations Ltd sets standards in health and safety management that demonstrate the existence of a safety culture in the workplace. Evolution Media Installations Ltd through effective management will encourage and promote the development and growth of a positive safety culture. The expansion of the safety culture is one that is regarded by everyone in Evolution Media Installations Ltd as having a responsibility in its development. Safety rules are understood and complied with and lead to improvements in performance, quality standards and staff morale. Evolution Media Installations Ltd believes the successful development of a safety culture is dependent on the following: - Acceptance of responsibility for health and safety from senior management and a clear commitment on the part of management to protecting the safety of all employees. This is demonstrated via a company health and safety policy statement. - Ongoing monitoring of safety in the workplace through hazard identification and risk assessment and the setting of realistic and achievable safety objectives.

- Thorough and systematic investigation of all incidents. - Immediate action to remedy any deficiencies once discovered. Safe systems of work Evolution Media Installations Ltd uses a formal procedure to examine how tasks are performed. This includes every day working, maintenance, changes to normal procedures, working alone, emergency procedures, etc. Where hazards cannot be eliminated by physically separating them from people and elements of risk remain with the task, to identify the hazards and assessment the risk, Evolution Media Installations Ltd employs a systematic approach to control them. Safe systems of work will be introduced to ensure that hazards are eliminated or any remaining risks are minimised. In low risk situations, a safe system of work may consist of simple verbal instructions given by a manager or supervisor. As the level of risk increases then written procedures will be provided. Evolution Media Installations Ltd uses job safety analysis techniques by applying the following five basic steps: - Assessment of the task. - Hazard identification and risk assessment. - Identification of safe methods. - Implementing the system. - Monitoring the system. In assessing the safe system of work, Evolution Media Installations Ltd will look at all aspects of the task and written details recorded to ensure that nothing is overlooked. A company manager in conjunction with workers involved in the task will do this. Managers know that their duties include devising and maintaining the safe system of work, including its implementation and any necessary revisions. The assessment will take into account: - What is used - plant, substances, machinery failures, electrical needs. - Sources of error - human failures, short cuts, emergency work. - Where task is carried out - working environment, protection demands. - How task is carried out - procedures, training, and frequency. To ensure a safe system of work will be successful, written method statements or the more formal permit-to-work system will be produced as well as orally explained briefings. This will ensure everyone who has to work with it understands the safe system of work, and it must be carried out on each occasion. Evolution Media Installations Ltd realises that everyone appreciates the need for the system and its place in the accident prevention programme. Where necessary, training will be given to all concerned in the safe system of work. This will give the necessary skills, the awareness of the system and the hazards which it is aimed to eliminate by the use of safe procedures. There is a requirement in each safe system of work produced for the stoppage of work when a problem appears which is not covered by the safe system of work and not resumed until a safe solution has been found. Evolution Media Installations Ltd will monitor every safe system of work by regular checks to ensure the system is still appropriate for the needs of the task and that it is being fully complied with the needs of the task. Evolution Media Installations Ltd appreciates that safe systems of work are prevention techniques of design, guarding and other methods, which aim to eliminate the possibility of human failure. Risk assessment We will comply with the statutory requirement to undertake a risk assessment of every task which is likely to be undertaken in the course of our business. By doing this activity we will provide not only cover employees at work but also others who may be affected by our work activity. Our risk assessments will be suitable and sufficient and cover both employees and non-employees affected by work. Any risk assessment will be reviewed if there is reason to suspect that it is no longer valid or if a significant change has taken place. The term suitable and sufficient is important since it defines the limits to the risk assessment process. A suitable and sufficient risk assessment will: - Identify the significant risks and ignore the trivial ones. - Identify and prioritize the measures required to comply with any relevant statutory provisions. - Remain appropriate to the nature of the work and valid over a reasonable period of time. We will use qualitative assessment the more common form of risk assessment which based purely on personal judgement and will be normally defined as high, medium or low. Where generic risk assessments are used, they will be appropriate to the particular job and will be extended to cover additional hazards or risks.

Definitions Whilst we understand there is no legal definition of the terms used in risk assessments we regard them to be: - A hazard is something with the potential to cause harm. - A risk is the likelihood that the harm from the hazard will be realised. Health risks Risk assessment will not only concerned with injuries in the workplace but also consider the possibility of occupational ill-health. Health risks fall into the following four categories : - Chemical (e.g. paint solvents, exhaust fumes). - Biological (e.g. bacteria, pathogens). - Physical (e.g. noise, vibrations). - Psychological (e.g. occupational stress. The two possible health effects of occupational ill-health are: - Acute meaning they occur soon after the exposure and are often of short duration, although in some cases emergency admission to hospital may be required. - Chronic meaning that the health effects develop with time. It may take several years for the associated disease to develop and the effects may be slight (mild asthma) or severe (cancer). Risk process Our system will identify hazards, assess risks, develop control measures and monitor the adequacy of such risk control systems. This requires us to: - Identify hazards. - Persons at risk. - Evaluation of risk level. - Risk controls (existing and additional). - Record of risk assessment findings. - Monitoring and review. Hierarchy of risk control When we assess the adequacy of existing controls or introducing new controls a hierarchy of risk controls will be considered, in accordance with the general principals of the Management of Health and Safety at Work Regulations 1999. Which are: - Avoid the risk altogether. - Evaluate the risks that cannot be avoided. - Combat the source at risk. - Adapt work to the individual wherever possible. - Adapt to technical progress. - Replace the dangerous with the non-dangerous or less dangerous. - Develop a coherent overall protection policy which covers such items as technology, working conditions and social conditions. - Adopt measures that protect the greatest number of individuals. - Give appropriate information, instructions and training to employees. - Provide personal protective equipment. These principles are not exactly a hierarchy but will be considered alongside the usual hierarchy of risk control which is as follows: - Elimination. - Substitution. - Engineering controls (e.g. isolation, insulation and ventilation). - Reduced or limited time exposure. - Good housekeeping. - Safe systems of work. - Training and information. - Personal protective equipment.

- Welfare. - Monitoring and supervision review. Record of risk assessment findings A written record of the risk assessment will be kept. For an assessment to be suitable and sufficient only the significant hazards and conclusions will be recorded. The record will also include details of the groups of people affected by the hazards and the existing control measures and their effectiveness. The conclusions will identify any new controls required and a review date. Drugs and alcohol All alcoholic drinks and unauthorised drugs are strictly prohibited from Company site and any person found in possession or under the influence of such substances will be refused access or immediately removed from site as appropriate. Immediate disciplinary action will be against those personnel. Information for employees Evolution Media Installations Ltd provides all employees, including trainees and those on fixed-duration contracts with comprehensive and relevant information on any risks to their health and safety which are identified by risk assessment. This information will be relevant to the risks and on preventive and protective measures and be limited to what employees need to know to ensure their health and safety. Information will also be provided on the emergency arrangements in place and the identity of staff nominated to assist in the event of evacuation. Any information provided to employees will be comprehensible and be capable of being understood by those to whom it is addressed, this will take account of their level of training, knowledge and experience. Special consideration will be given to any employees with language difficulties or with disabilities, which may impede their understanding of the information given. Information will be given in the most suitable form, including verbally instruction, dependant upon the circumstances, but will always be clear and comprehensible. Consultation with employees. Employees working in Evolution Media Installations Ltd do not belong to a trade union but are still consulted on health and safety matters in accordance with the Health and Safety (Consultation with Employees) Regulations 1996. This consultation will be on an individual basis. Evolution Media Installations Ltd will consult on the following: - The introduction of any measure or change which may substantially affect employees health and safety. - The arrangements for the appointment of competent persons to assist in following health and safety law. - Any information resulting from risk assessments or their resultant control measures which could affect the health, safety and welfare of employees. - The planning and organisation of any health and safety training required by legislation the health and safety consequences to employees of the introduction of new technologies into the workplace. However, Evolution Media Installations Ltd will not disclose information if: - It violates a legal prohibition. - It could endanger national security. - It relates specifically to an individual without their consent. - It could harm substantially the business of the employer or infringe commercial security it was obtained in connection with legal proceedings. Capabilities It is Evolution Media Installations Ltd policy when allocating work to its employees, to ensure that the demands of the job do not exceed the employees ability to carry out the work without risk to themselves or others. Evolution Media Installations Ltd will take account of the employees capabilities and the level of their training, knowledge and experience and where necessary additional training will be provided. Training All employees from senior management to trainees will receive relevant training. Evolution Media Installations Ltd uses training as an important way for its employees to achieve competence and therefore help convert information into safe working practices. Evolution Media Installations Ltd also believes that training contributes to its health and safety culture. Risk assessment is used to help determine the level of training and competence needed for each particular type of work activity, even in some cases where an employee already holds a formal qualification.

Training will include basic skill training, on-the-job training and also training in health and safety or emergency procedures. Where a specific risk is known, precise training will be arranged. Health and safety training will take place during working hours, where training is required to take place outside Evolution Media Installations Ltd normal hours this will be treated as an extension of the time at work. Where these changes are significant they will be reviewed and a re-assessment of the risk(s) will take place. This may result in the requirement for additional training. Evolution Media Installations Ltd uses refresher training periodically to ensure the continuation of competence of its employees. In instances where employees occasionally deputise for others, and there is a possible under-development of skills, the requirement for more frequent refresher training will be introduced. Information from personal performance monitoring, health and safety checks, accident investigations and near miss incidents will be used to help establish the requirement for further refresher training. Health and safety As part of our health and safety culture and as a legal requirement, under the Management of Health and Safety at Work Regulations 1999 and other regulations. Evolution Media Installations Ltd will provide training on recruitment, at induction or on being exposed to new or increased risks due to: - Being transferred to another job or given a change in responsibilities. - The introduction of new work equipment or a change of use in existing works equipment. - The introduction of new technology. - The introduction of a new system of work or the revision of an existing system of work. - An increase in the employment of more vulnerable employees (young or disabled persons). - Particular training required by the organisation s insurance company (e.g. specific fire and emergency training). Additional training will be provided if needed following a single or series of accidents or near misses, the introduction of new legislation, the issuing of an enforcement notice or as a result of a risk assessment or safety audit. Induction training All new employees, trainees will receive induction training. This training covers items such as pay, conditions, quality and health and safety. The inductee will sign a record to the effect that training has been received. This record may be required as evidence should there be a subsequent legal claim against the organisation. The induction training programme would include the following topics: - The health and safety policy including a summary of the organisation and arrangements including employee consultation. - A brief summary of the health and safety management system including the name of the employee s direct supervisor and source of health and safety information. - The inductee s responsibility for health and safety including any general health and safety rules (e.g. smoking prohibitions). - The accident reporting procedure of the organisation, the location of the accident book and the location of the nearest first aider. - The fire and other emergency procedures including the location of the assembly point. - A summary of any relevant risk assessments and safe systems of work. - The location of welfare, canteen facilities and rest rooms. Additional items, specific to Evolution Media Installations Ltd, i.e. the correct use of personal protective equipment, maintenance procedures, manual handling techniques, etc will also be included. Job-specific training Job-specific training will be carried out to ensure that the employee undertakes his job in a safe manner. This training is a form of skill training and will be done as on the job / toolbox training. Details of the safe system of work or, in more hazardous jobs, a permit to work system will also be covered. In addition to normal safety procedures, emergency procedures, standard safety checks, maintenance requirements and the correct use of personal protective equipment. This type of training will follow an operational procedure in the form of a checklist which the employee can sign on completion of the training. The new employee will be subject to close supervision for some time after the training has been completed. Monitoring performance Evolution Media Installations Ltd will set performance standards and continually appraise the extent to which those standards are being attained. Monitoring health and safety performance will enable Evolution Media Installations Ltd to: - Assess the effectiveness of policies and procedures in place. - Identify areas where additional safeguards are necessary.

- Comply with legal record keeping requirements. - Show the ongoing commitment to sustaining a safe and healthy workplace. Active and reactive monitoring Company monitoring systems will incorporate both active and reactive monitoring. Active monitoring will measure performance against pre-determined standards to ensure that systems and procedures are being property implemented and those standards are being achieved. These activities will be concerned with prevention rather than cure in that their aim is to identify areas of poor performance before an incident occurs. Active monitoring activities will consider the following: - Are performance standards being achieved? - Are current control measures being properly implemented? - Is all Personal Protective Equipment being properly used? - Do all employees know and obey Evolution Media Installations Ltd safety rules? Evolution Media Installations Ltd recognises that it is almost inevitable that accidents will happen. People take short cuts, forget to use protective equipment and fail to pay attention to what they are doing. Reactive monitoring is concerned with analysing incidents as they happen and aims to discover: - Exactly what happened. - Why it happened. - How it can be prevented from happening again. Evolution Media Installations Ltd recognises that accidents do happen and will endeavour to learn from our mistakes. Our reactive monitoring activities centre on the study of: - Accident reports. - Attendance sheets. - Inspection records. RIDDOR In accordance with the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995 (RIDDOR 95) we keep records of reportable incidents. We keep records of all accidents, no matter how minor, as these enable us to assess health and safety performance. Records are also kept for: - Accidents and near misses. - Hazards (workplace inspection/risk assessment records). - Attendance. Details of any accidents are recorded on the correct form as soon as possible after the event and where appropriate diagrams and photographs will accompany the report. Near misses are treated in the same way as accidents in terms of reporting and investigation. A near miss is defined as an unplanned and unforeseeable event that could have resulted, but did not result in human injury, property damage or other form of loss. Not all incidents will merit immediate thorough investigation but if the potential outcome of the near miss was a serious accident then the incident will be investigated as a matter of priority. Attendance records are kept that show absence from work due to sickness as an indicator of health and safety performance. These will help identify absence patterns, which could be improved. Information on employee illness is usually obtained from self-certification sickness form. Incident investigation recording and reporting Evolution Media Installations Ltd believes that good investigation is a key element to making improvements in health and safety performance. Incidents and accidents, whether they cause damage to property or more seriously injury and/or ill-health to people, will be properly and thoroughly investigated to allow Evolution Media Installations Ltd to take the appropriate action to prevent a recurrence. Incident/accident investigation will be based on the logic that: - All incidents/accidents have causes. - Eliminate the cause and eliminate future incidents. - The direct and indirect causes of an incident/accident can be discovered through investigation. - Corrective action indicated by the causation can be taken to eliminate future incidents/accidents.

Investigation will not be a mechanism for apportioning blame. Incident investigation will be part of a reactive monitoring system because it is triggered after an event. The range of events includes: - Injuries and ill-health, including sickness absence. - Damage to property, personal effects, work in progress, etc. - Incidents which have the potential to cause injury, ill-health or damage. Injury accident As it is not possible to determine the potential for harm simply from the resulting injury, Evolution Media Installations Ltd will investigate all accidents. The amount of time and effort spent on the investigation will vary depending on the level of risk (severity of potential harm, frequency of occurrence). Evolution Media Installations Ltd will focus on significant events involving serious injury, ill-health or losses and events which have the potential for multiple or serious harm to people or substantial losses. These factors should become clear during the accident investigation and be used to guide how much time should be taken. Incidents (including near miss) All incidents requiring reports to the regulatory authorities will be investigated thoroughly. Investigation of other incidents will be judged individually, but if there is likely to be a complaint or an enquiry by a regulatory authority, an investigation will be carried out. Therefore, unless there are good reasons against, all reports of incidents, near misses and hazards will be investigated. Investigation will be carried out as soon as possible after the incident to allow the maximum amount of information to be obtained. Consideration will be given to asking the person to return to site for the accident investigation, if they are physically able rather than wait for them to return to work. Evolution Media Installations Ltd will investigate using the following four basic elements: - Collect facts about what has occurred. - Assemble, and analyse the information obtained. - Compare the information with acceptable industry and company standards and legal requirements to draw conclusions. - Implement the findings and monitor progress. Information will be gathered from all available sources, witnesses, supervisors, physical conditions, hazard data sheets, written systems of work, training records, etc. The amount of time spent will not, be disproportionate to the risk. The aim of the investigation, being to explore the situation for possible under lying factors, in addition to the immediately obvious causes of the accident. Recommendations The investigation will highlight both immediate (peoples behaviour, training and competence, etc) causes and underlying (previous incidents, supervision, etc) causes. Recommendations both for immediate action and for longer-term improvements should come out of this, for management to implement improvements recommended by the report. Follow up Evolution Media Installations Ltd will check on the implementation of the recommendations and also review the effect of the recommendations to check if they have achieved the desired result and whether they have had unforeseen knock-on effects, creating additional risks and problems. The investigation report will be circulated to relevant people within the organisation. Evolution Media Installations Ltd will report certain more serious accidents and incidents to the HSE or other enforcing authority and to keep a record. The reporting and recording requirements are as follows: Death or major injury If an accident occurs at work and: - An employee or self-employed person working on the premise is killed or suffers a major injury (including the effects of physical violence). - A member of the public is killed or taken to hospital. The responsible person in Evolution Media Installations Ltd will notify the enforcing authority without delay by the quickest practicable means, i.e. telephone. They will give brief details about the organisation, the injured person(s) and the circumstances of the accident and, within ten days, the responsible will also send a completed accident report form, F2508. Over seven day lost time injury If there is an accident connected with work (including physical violence) and an employee, or self-employed person working on the premises, suffers an injury and is away from work or not doing their normal duties for more than three days (including weekends, rest days or holidays) but not counting the day of the accident. Evolution Media Installations Ltd will send a completed accident report form, F2508, to the authority within ten days.

Disease If a doctor notifies Evolution Media Installations Ltd that an employee suffers from a, work-related disease a completed disease report form, F2508A, must be sent enforcing authority. Evolution Media Installations Ltd will confirm with the HSE Info Line or the Incident Contact Centre to check whether a particular disease is reportable. Dangerous occurrence If an incident happens which does not result in a reportable injury, but have done, it could be a Dangerous Occurrence. All Occurrences must be reported immediately by i.e. telephone, to the enforcing authorities. Evolution Media Installations Ltd will confirm with the HSE Info Line or the Incident Contact Centre to check whether a dangerous occurrence is reportable. Evolution Media Installations Ltd will send a completed accident report form, F2508, to the authority within ten days. Reports Reports will be sent to the Incident Contact Centre (ICC) directly. Reports will be sent by telephone, fax, the Internet, or by post. If reporting by the Internet or telephone a copy of the report is sent to the responsible person for correction, if necessary, and for their records. Postal reports should be sent to: Incident Contact Centre, Caerphilly Business Park, Caerphilly, CF83 3GG Internet reports go to: www.riddor.gov.uk or link through www.hse.gov.uk First aid In accordance with the Health and Safety (First-Aid) Regulations 1981 Evolution Media Installations Ltd will provide adequate and appropriate equipment, facilities and personnel to enable first aid to be given to employees if they are injured or become ill at work. Evolution Media Installations Ltd will inform all its employees of the arrangements made in connection with the first aid regulations including the location of equipment, facilities and the identification of trained first aid personnel. New employees will be informed on induction, of the location of first aid equipment and personnel, and if appropriate, facilities. As a very minimum first-aid provision on site Evolution Media Installations Ltd will at all times provide people at work with: - A suitably stocked first-aid box. - An appointed person to take charge of first-aid arrangements. At least one notice will be posted, in a conspicuous place in all workplaces, giving the location of first aid equipment and facilities and the names and locations of first aid personnel. Employees will be informed of the first-aid arrangements at induction. This will be supplemented by notices telling staff who and where the first aiders or appointed persons are and where the first-aid box is. Appointed person An appointed person will be appointed by Evolution Media Installations Ltd to be available at all times when people are at work on site to: - Take charge when someone is injured or falls ill. This includes calling an ambulance if required. - Look after the first-aid equipment, keeping the first-aid box replenished; keeping records of treatment given. The training for appointed persons will be repeated every three years to keep up to date. Reporting incidents We will report any death, reportable injury, disease or dangerous occurrence under The Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995 (RIDDOR) to the relevant enforcing authority. We will provide full information about RlDDOR incidents to the principal contractors so that they can monitor compliance with health and safety law and, if necessary review the arrangements for the management of health and safety. Doors and gates Where it is necessary to prevent the risk of injury to any person, any door, gate or hatch will incorporate or be fitted with a suitable safety device. Doors, gates or hatches will not comply with this regulation unless: - If it is a sliding door with a device to prevent it coming off the track. - If it is a upward opening door with a device to prevent it falling down. - If it is a powered door with effective features to prevent it from injuring and trapping any person and any powered door can be operated manually unless it opens automatically if the power fails. This will not apply to any door or gate that forms part of any piece of mobile plant or equipment.

Prevention of risk from fire etc Arrangements will be in place to prevent, so far as is reasonably practicable, the risk of injury to any person during the carrying out of work arising from any fire or explosion, flooding, or any substance that is liable to cause asphyxiation. Emergency routes and exits A sufficient number of emergency routes and exits will be provided and any emergency route or exit will lead directly, or as directly as possible, to an identified safe area. The emergency route or exit will be kept clear and, where necessary, provided with emergency light so that it can be used at any time. Emergency exits or routes will have regard for: - The type of work for which the construction site is being used. - The characteristics and size of the construction site, and the number and location of places of work on the site. - The plant and equipment being used. - The number of persons likely to be present on site at anyone time. - The physical and chemical properties of any substances or materials likely to be on site. All emergency routes and exits will be identified by suitable signs with all signs conforming to the Health and Safety (Safety Signs and Signals) Regulations 1996. Emergency procedures We will make suitable and sufficient arrangements to deal with any foreseeable emergency including arrangements for evacuation of site. These arrangements will be tested at suitable intervals. Fire detection and fire fighting We will arrange for suitable fire fighting equipment, fire alarms and fire detection systems, to be installed and located in appropriate places. This equipment will be maintained and subjected to periodic tests and examination so as to ensure that the equipment remains effective. All fire fighting equipment must be indicated by suitable signs. All signs conform to the Health and Safety (Safety Signs and Signals) Regulations 1996. Every person at work on a construction site will be instructed in the use of any fire fighting equipment where it may be necessary for them to use. If a work activity gives rise to a particular risk of fire, the person authorised to carry out such work will have received instructions on how to prevent the risk of a fire starting. Welfare facilities Evolution Media Installations Ltd will be responsible for ensuring that there will be on site: - Sanitary conveniences Evolution Media Installations Ltd provides relevant and adequate sanitary conveniences that are located at easily accessible places these conveniences are kept clean and in good order, and are sufficiently ventilated and illuminated. These are provided together and in proportion to the size of the workforce. - Washing facilities are kept clean and well lit, have running hot and cold water, and are large enough for washing the face, hands and forearms. These facilities will a good supply of warm water, soap and towels are provided as close to the sanitary facilities as possible. These facilities are well ventilated and lit and cleaned on a regular basis. - Drinking water is adequate, wholesome and readily accessible to the entire workforce. Normally mains water is used and is marked as drinking water. - Accommodation for clothing and facilities for changing clothing Facilities are provided which are clean, warm, dry, well-ventilated and secure. Where workers are required to wear special or protective clothing, arrangements will be made for workers own clothing not to be contaminated by any hazardous substances. - Facilities for rest and eating meals are provided so that workers may sit down during break times in areas where they do not need to wear personal protective equipment. These rooms will be designated No smoking areas. Arrangements are in place to ensure that food is not contaminated by hazardous substances. Suitable and sufficient facilities for rest, including provision for non smokers, pregnant women and nursing mothers, will be provided or made available at readily accessible places. The facilities provided will include the means for boiling water and suitable arrangements to ensure that meals can be prepared and eaten. - Temperature and weather protection During working hours a reasonable temperature so far as is practicable, will be achieved at any indoor place of work with regard to the purpose of the place is being used. - Good Housekeeping We will so far as is reasonably practicable we will keep site and parts of site in good order and in a reasonable state of cleanliness. Where it is necessary in the interests of health and safety, the perimeter of the site will be identified by suitable signs so that the arrangements and extent of the site is readily identifiable.

Inspection Where an inspection by a competent person is carried out under this regulation is not satisfied that co work can be carried out safely, they will be responsible for putting in place arrangements to ensure that it is not used until matters identified as being unsatisfactory have been remedied. Inspection of a place of work will include an inspection of any plant, equipment and materials which may affect the safety of that place of work. Workplace (health safety and welfare) Evolution Media Installations Ltd will ensure, as far as is reasonably practicable, the protection of our employees health, safety and welfare whilst they are at work. Maintenance of workplaces equipment devices and systems Company workplaces, equipment and the devices within, will be maintained to an efficient standard, kept in good working order and repair. Where any defects is discovered they will be returned to good order as soon as possible. Evolution Media Installations Ltd has procedures in place to ensure that repairs and maintenance work is carried out properly. Ventilation Ventilation of the workplace is effective and sufficient and free of any impurity, with air inlets sited clear of any potential contaminant. Workstations will be place so that workers are not subject to uncomfortable draughts. The air that is introduced will as far as is possible, be free from impurities, which may cause ill health or be offensive. This air will be introduced by, windows or electrical systems, which will provide ventilation for parts or all of the workplace. There is a maintenance procedure in place, to test and exam electrical systems on a frequent basis. Heating and temperature During working hours, the temperature in all workplaces inside buildings will be reasonable (not uncomfortably high or low). No heating or cooling method will be used in the workplace which produces fumes, injurious or be offensive to any person. Such equipment will be regularly maintained. Where, despite the provision of local heating or cooling, the temperatures are still unreasonable, suitable protective clothing and rest facilities will be provided. Where employees are required to work in normally unoccupied storerooms, temporary heating will be supplied. Lighting All workplaces have suitable and sufficient lighting and this will be natural lighting as far as is reasonably practicable. Suitable and sufficient emergency lighting is provided and maintained in rooms where workers are particularly exposed to danger in the event of a failure of artificial lighting. Windows are kept clean and free from obstruction so far as is reasonably practicable. Stairway lighting is placed so that shadows are not cast over the main part of the treads. Light switches are positioned so that they may be found and used easily and without risks. Workstations will be sited to take advance of the available natural light. Cleaning Regular cleaning is carried out by an effective and suitable method that does not create or expose anyone to a health or safety risk. Interior walls, ceilings and work surfaces are cleaned at suitable intervals. Room space and dimensions All workrooms are careful planned to have sufficient space to allow employees to move to and from workstations and within the room freely. Stacking and racking Materials and objects are stored and stacked in such a way that they are not likely to fall and cause injury. All racking is of adequate strength and stability to sustain any loads that are placed upon it. The following precautions are considered in stacking and storage: - Banding or wrapping to stop individual articles falling out. - Maintaining a height limit for stacks to ensure their stability. - A regular check procedure of stacks to identify and remedy any unsafe stacks.
