2017 Fall Sports Paperwork Information and Important Dates All 6 th -12 th grade students are permitted and encouraged to try out for a team. With this is mind, below is a list of important upcoming dates and requirements for any athlete planning on trying out for a Fall Sport in the 2017-2018 school year. Please keep in mind that starting dates are tentative and could change (field/court conditions, weather, etc.). Fall Sports Offered: Middle School Level (6 th -8 th Grade) Boys Soccer, Girls Soccer Junior Varsity/Varsity Level (9 th -12 th Grade) Boys Soccer, Girls Soccer, Co-Ed Cross Country, Girls Tennis, Girls Cheerleading Requirements: All forms can be found at http://cca.ccphilly.org/athletics/sports-forms/. We must receive the following items prior to your son or daughter trying out for a team: PIAA Physical (Sections 1-6). Sections 1-5 need to be filled by a parent/guardian along with your athlete s signature. Section 6 must be filled out, dated, and signed by a licensed physician on/after June 1 st, 2017. Physicals that are not completed and dated on or after this date will not be accepted. In addition, any physical that is filled out on a non-piaa form will not be accepted by the Athletic Office. If you are having trouble obtaining a physical due to insurance restrictions, we have found that Minute Clinics (CVS, Walgreens, Rite Aid, etc) will complete PIAA physicals for athletes for a fee. Urgent Care Centers can also provide physicals for a fee. This would be a recommended option if you are not able to schedule a physical with your athlete s doctor in the time frame needed. We strongly suggest that all athletes (fall, winter, spring) have their physicals completed over the summer. This will prevent any possible delays with Doctor s appointment scheduling or any unforeseen issues that may arise. Registration Form Commitment Contract Athletic Fee- $175 ($200 for Cheer). Please make checks payable to CCA. Credit cards are not accepted. This is a very big accomplishment that I am pleased to be a part of by photographing your senior pictures this year! Here is some information I hope you find helpful regarding senior pictures: When: THERE IS AN ATTACHMENT BELOW THAT TELLS YOU WHEN YOU ARE TO BE AT THE SCHOOL READY FOR YOUR SENIOR POR- TRAITS. PLEASE DO NOT BE LATE. :) Where: The location of the portraits will be taken place in the "Bride room" next to the sanctuary at Calvary. (Walk into the front of the church entrance and walk left) Cost: When you arrive to have your pictures taken you will need to bring your sitting fee with you. Your Yearbook Only picture taken is $20.00. A full photo session with casual shots in addition to the yearbook shots is $40.00. ~The Yearbook Only session includes the yearbook shots and the cap and gown shots. You must at least have your yearbook pictures taken for the school yearbook. ~The Full Photo Session includes the yearbook shots as well as an additional casual photos; These photos are to showcase your interests and personality. Tips- Guys will be photographed in a tux that we provide. The cap and gown will also be provided and for the guys it is dark blue and it shows your collar underneath so you may want to bring a dress shirt and tie for under it. Or you can use the tux shirt that we provide for under the gown or you can wear a polo shirt or t-shirt depending on how dressy or casual you want the pictures to be. Girls will be photographed in a black v-neck drape that we provide- you will need a tank top for underneath and be able to pull the straps down because the top part of your shoulders will show. We will also have the cap and gown for those shots. The cap and gown will also be provided and for the girls it is white and see through so make sure you don t have on anything with color underneath which could show up in your pictures. Remember: *Be careful with sunburn and/or tan lines as they will show up in your pictures. Try to remember this for the whole week prior to your photo session. *If you wear glasses, consider being photographed without them or without the lenses because sometimes a glare is created that cannot be fixed. *Don t experiment the day before your pictures with a new hair cut or style just in case it does not work out. Guys should make sure that they shave on the day of your session, five o clock shadows don t look very nice. Payments: ***Payments must be made in checks or cash. All checks must be made payable to Toni Valentini Photography. Payment is due when you arrive for your portraits. There will be a fee for any bounced checks.
