About Our Pre-Paid Multi-Week Program PARTICIPATION~~ THIS is a 5-week program available to children in GRADES 3 AND UP. To be eligible you must have 15 or more participants in your group. Each participant must register for all of the groups ski days, pre-pay for the tickets, and receive the tickets in advance. Each participant must purchase the same type of ticket/package for all weeks of the program (There will be no exceptions.) TICKETS~~ Each participant will receive one dated ticket per ski day. We suggest tickets be handed out each day your group skis, AFTER CONFIRMING the ski day will take place. Note: once tickets are attached to coats they will be valid for that day only. One Chaperone ticket will be issued for each 15 pre-paid participants and are ONLY good for the day printed on the ticket unless the group is rescheduled. These tickets have no trade-in or cash value. Tickets will be printed and ready for pick up at Mount Southington starting December 15, 2017. Please call to verify that your tickets are ready for pick up prior to coming to the ski area. UPON ARRIVAL~~ Upon arrival those participants that need rental equipment will head to the Rental Department. To expedite the rental process, each participant should know their rental card number (assigned by the coordinator & found in the top right hand corner of the rental card). Any participant taking lessons should then meet at Ski School (far left) to be put into the appropriate lesson for their ability. Lessons begin at 3:30, 4:30 or 5:30. For experienced skiers we suggest taking the 5:30 lesson, since this time is usually less congested. GROUP TRIP RESCHEDULING~~ During the course of the season, if your group needs to reschedule a trip, the Group Coordinator must call Mount Southington ASAP so that we can adjust our staff accordingly and reschedule your group. If Mount Southington needs to close for unsettling weather, we will email the Group Coordinator and discuss a mutually agreed upon reschedule date. When a trip is rescheduled the group wears the ticket that displays the date the trip was canceled.

Coordinator Checklist for Organizing a Club Select the Group s Trip Dates - Select the group s preferred ski dates using the Pre-Paid Multi-Week Group Information Form. Fax, email, or mail this form to Andrea, the Mount Southington Group Coordinator. *Upon receipt of the Group Information Form, Mount Southington will send you a packet of forms (including registration forms and rental cards). * Distribute the Registration Forms and Rental Cards to all participants. Registration Forms Ensure that all forms are filled out with one package type selected for each participant for all the group s trips and that the Group Waiver on the back of the form is signed by a parent or guardian. Only one registration form is needed per family. Group Waivers Schools that use their own registration forms will be provided with only the Group Waivers to be signed by a parent/guardian and returned to the ski area. Only one waiver form is needed per family. Rental Cards All participants who are renting equipment must complete the appropriate rental card (one per person). Please make sure that the rental cards are completed, front and back, and signed by the user and a parent/guardian. Please number each card beginning with 1. To expedite the rental process, each participant should know their rental card number. Coordinator Worksheet - This worksheet has been enclosed for your personal use, to assist in filling out the Ticket Tally Sheet. Sending it to us is optional. Ticket Tally Sheet The Ticket Tally Sheet is used to track how many tickets your group needs for each ticket/package type. Your tickets are printed from this sheet so please be accurate. For participants who register after the original Ticket Tally Sheet has been sent to us there is a $25 late fee. Encourage all participants to sign up in a timely fashion to avoid this fee. Payment Collect the appropriate amount of money per participant. We accept cash, checks made payable to Mount Southington Ski Area, or, our easiest form of payment, MasterCard, Visa, Discover or American Express, made online. To participate in the online payment option, check the appropriate box on the Group Information Form. Ticket Pick-Up - Your packet will be reviewed upon receipt and accepted when a Ticket Tally Sheet is completed and all forms are filled out appropriately. Tickets will be available for pick-up starting December 15, 2017. Please call 860-628-0954 x216 to verify that your tickets are ready prior to coming to the ski area.

SCHOOL S OUT SKI CLUB Pre-Paid Sunset/Night Group Rates 2017-2018 Season This is a school group program that reserves your group s spot on the mountain for 5 or more visits of your choosing & is paid for prior to the beginning of the season. Night Lessons are 1 ½ hours in duration and begin promptly at 7pm. SPECIAL COORDINATOR BENEFIT: One 2017-2018 Season Pass Voucher will be issued to the Group Coordinator of a Pre-Paid group that schedules and prepays for 5 or more visits during the season, and has 15 or more participants. -There are NO School Groups on the following Dates: New Year s (1/1), MLK Day (1/15), & Feb Break (2/19-2/20). - If you are injured and cannot complete your season, a refund for unused tickets is available with a doctor s note. *Other time frames and rates available upon request.

GROUP LESSON PROCEDURES All Pre-Paid Multi-Week participants must be in Grade 3 or above It is important for students that are taking lessons to get to the lesson area on time. Lessons cannot be held up for students that are late. Lessons begin promptly at 3:30pm, 4:30pm, and 5:30pm. All Beginners should attend the 3:30pm or 4:30pm lesson if possible. We suggest that intermediate and advanced students attend the 5:30pm lesson. Skiers will be skied-off into groups according to ability on their first day, if necessary. We suggest each group provide some type of identification (armband, ribbon, tag, button, etc.). This helps both us and your chaperones keep track of the group. Along with filling out the Ski Patrol Emergency Form we ask that the Chaperones let Ski School know where they will be at the ski area if an accident should occur. This is a winter sport. The temperature goes down with the sun. Be sure your students are properly dressed. If a child s attire is not appropriate, i.e. a health hazard, they may not be allowed in Ski School or on the slopes. Knit mittens, long scarves and long hats can be dangerous and should be avoided. Coordinators and Chaperones are invited to participate in any group lesson. We only ask that you attend a lesson that members of your group are participating in. NOTE: Pre-Paid Multi-Week Group Lessons are purchased on a Group basis and can only be made up as a group. There will be no individual lesson make-ups. If a group cancels and does not rescheduled, the lesson will be forfeited, however unused tickets can be used for their face value at the ticket window during the current season.

Know the Code Mount Southington Ski Area Your Responsibility Code Observe the code listed below and share with other skiers and riders the responsibility of a great winter recreation experience. Always stay in control and be able to stop or avoid other people or objects. People ahead of you have the right of way. It is your responsibility to avoid them. You must not stop where you obstruct a trail, or are not visible from above. Whenever starting downhill or merging into a trail, look uphill and yield to others. Always use devices to help prevent runaway equipment. Observe all posted signs and warnings. Keep off closed trails and out of closed areas. Prior to using any lift, you must have the knowledge and ability to load, ride, and unload safely. For Your Safety Safety Bar: While riding the chairlift the safety bar MUST be used as prompted by signs upon loading and before unloading. Also, to avoid slipping off the chairlift sit with your back against the chair and remain facing forward. Please, no horse play, for safety sake! Use of Helmets: Mount Southington recommends the use of helmets. In some situations, they can be helpful in avoiding an accident. There have been many topics concerning the pros and cons of helmet use. The final decision rests with the parent/guardian. At the present time Mount Southington does not rent helmets. Helmets may be purchased at our Ski Shop when the ski area is in operation or at various ski outfitting stores and should be properly fit to maximize their effectiveness. Mount Southington s Courteous Skier/Rider Policy We at Mount Southington believe all skiers and riders have the right to a safe and pleasant place to ski. Equally, each skier and rider has the responsibility to protect others rights to the same skiing/riding experience. Therefore, the Management, the Rangers, Ski Patrol, and Ski School Instructors are authorized to rescind the skiing privileges of anyone who creates a nuisance or endangers themselves or others. Dangerous or undesirable conduct is: 1. Foul language. 2. Lift line cutting. Skiing/Riding at excessive speeds. 3. Refusal to use the safety bars provided. 4. Jumping from the chairlift or bouncing the chair. 5. Removing hazard markers, flags, or poles. 6. Unauthorized or unpaid skiing or riding through race courses. 7. Use of alcohol or drugs. Please duplicate and distribute, or review these policies with your group on your bus trip to Mount Southington.

MOUNT SOUTHINGTON SKI AREA GROUP PROGRAMS WAIVER, ASSUMPTION OF RISK, RELEASE & ARBITRATION AGREEMENT In consideration of being allowed to participate in a group program (the Program ) at Mount Southington Ski Area (the "Facility"), the Participant, and the Participant s parent(s) or legal guardian(s) if the Participant is a minor, do hereby agree, to the fullest extent permitted by law, as follows: 1) TO WAIVE ALL CLAIMS that they have or may have against the Facility, Mount Southington Limited Partnership (the Partnership ), and/or Mount Southington Restaurant and Lounge, Inc. (the Lounge ) arising out of the inherent risks of participating in the Program; 2) TO ASSUME ALL RISKS INHERENT IN PARTICIPATING IN THE PROGRAM; and 3) TO RELEASE the Facility, the Partnership, the Lounge, their owners, affiliates, officers, directors, employees, agents, and shareholders, from all liability for any loss, damage, injury, or expense that the Participant (or his/her next of kin) may suffer, arising out of the inherent risks of participation in the Program, which include, but are not limited to, the instruction received while participating in the Program. The Participant acknowledges and agrees that the inherent risks of participating in the program are in addition to those referenced in Connecticut General Statutes 29-212. The participant acknowledges and agrees that it is their responsibility to close the restraining device on the chair lift, as referenced in Connecticut General Statutes 29-213. The Participant also acknowledges that this agreement does not, in any way, change the rights or obligations of the Facility, the Partnership, the Lounge, or the Participant, as set forth in Connecticut General Statutes 29-211, et. seq., other than as set forth in this agreement. Arbitration The Participant hereby agrees to submit any dispute arising from participation in the Program to arbitration, for the sole purpose of determining whether the alleged injury arises from a risk inherent in the activities engaged in by the Participant while participating in the Program. For such disputes, there shall be a three-member arbitration panel, consisting of two party-appointed arbitrators (one arbitrator to be appointed by each party) and one neutral arbitrator (collectively, the Panel ), to be chosen by the party-appointed arbitrators. In the event that the two party-appointed arbitrators are not able to agree on a third, neutral arbitrator, the neutral arbitrator shall be appointed by the United States District Court, for the District of Connecticut. Each party shall pay its own costs, including the costs associated with the party-appointed arbitrators, and the parties shall share equally the costs associated with the neutral arbitrator. The arbitration proceeding shall proceed in Hartford, Connecticut and shall be governed by the Federal Rules of Evidence. The Panel shall establish a reasonable and appropriate discovery schedule to expeditiously resolve this matter. In the event that the Panel determines the alleged injury arises from a risk inherent in the Participant s participation in the Program, the claim shall be deemed barred, as a matter of law, and the Participant shall be barred from recovering any compensation from the Facility, the Partnership, and/or the Lounge. In the event that the Panel determines the alleged injury did not arise from a risk inherent in the activities engaged in during the Program, the Participant shall proceed to the Superior Court of Connecticut, or if appropriate, the United States District Court, for the District of Connecticut, for a trial de novo. In the event your Group misses a ski day; a Group make-up day will be re-assigned by Mount Southington in conjunction with your group coordinator. 2. Members should not put their lift tickets on until the day of the ski trip. If a day is cancelled, that lift ticket will be used for the make-up day. For example, if your group misses Jan. 9 and your make-up day is scheduled for Feb. 6, members will wear the Jan. 9 ticket on Feb. 6. 3. In the event Mt. Southington closes for the season (before your Group completes its scheduled program) remaining unused tickets are valid for their face value the following season towards the purchase of a ticket at the ticket window. See ticket redemption information below in the individual make-up procedure. Individual Make-Up Procedure: 1. If an individual misses a scheduled ski day, any remaining unused lift tickets and rentals may be evenly exchanged at the ticket window during the Ski Club time slot (Excluding Martin Luther King Day 1/15/2018 and February Break 2/19 2/20/2018) during the current season. The Ski Club time slot is Monday thru Friday 3pm 8pm (non-holiday periods). 2. NOTE: Lesson make-ups will not be available on an individual basis; they must be made up with your group, otherwise the lesson is forfeited. 3. All unused tickets may be redeemed for their face value (printed on ticket) towards the purchase of any ticket type at the ticket window anytime during the current season. They cannot be used towards future Ski Club purchases. Refunds: 1. A refund is issued only in the event a student is out for the remainder of the season due to illness or injury.
