The goal of our program is to provide a positive, healthy and safe environment lead by our highly skilled coaching staff. Even if you have never cheered or danced before our program and championship staff is designed to teach and train you to be a successful and competitive all star cheerleader or dancer. EVALUATION/TEAM PLACEMENT CHECKLIST: We look forward to our upcoming evaluation/team placements. Please sure to arrive with the final forms already filled out and completed on your evaluation date. ATHLETE EVALUATION/PLACEMENT DATES: 8 & under Monday, May 14 th 6:00p.m.-8:30p.m. 9-11 yrs Tuesday, May 15 th 6:00p.m.-8:45p.m. 12-18 yrs Thursday, May 17 th 6:00-9:00p.m. FLYER EVALUATIONS Friday, May 18 th 6:00p.m.-8:00p.m. Please Note: Potential flyers still must attend regular scheduled evaluation date as well. INVITE ONLY IMPORTANT: AGE GRID: CHEER & DANCE Below are the age guidelines set forth by the USASF in regards to the ages per team. This information will hopefully help you better understand what team your athlete can potentially be on for the 2018-2019 season. NOTE: Age as of August 31 st Team Tiny Mini Youth Junior Senior Levels 1-4 Age 6 years and under 8 years and under 11 years and under 8 to 14 years 11 to 18 years

Levels: There are 4 levels for the regular teams this season. The Level refers to the level of skill such as stunts and tumbling that that particular team can safely perform. Levels per athlete are determined solely by the coaching staff! Athlete and Flyer Evaluations: Athlete: Evaluation sessions are not designed to be stressful for our athletes. They are designed for our athletes (new or returning) to showcase the skills they have already developed and look for their potential to learn new skills. With that being said, evaluations are closed to parents as we feel athletes are less stressed when parents are not watching. Coaching Staff will evaluate each athlete based on the following criteria during evaluations: v Tumbling difficulty v Tumbling Execution v Athleticism v Jump Technique v Willingness to accept instruction v Attendance and attitude from past seasons v Stunting position (flyer, base, back spot)

We diligently work to select teams that have the chance to be successful during the season. We definitely try to match athletes by skill level as much as possible, but there are other factors that go into making the decision. These factors include: stunting, jumps, dance, motions and age. Therefore, athletes may be on any given team meaning they maybe at a different level then their teammates. We choose our teams based on the positions that are needed to fill each team (bases, flyers, tumblers, etc.) Every athlete is on the team for a reason PLEASE TRUST OUR COACHING STAFF! FLYER EVALUATION: This additional evaluation helps coaching staff consider who can potentially be a flyer for the upcoming season, as well as give athletes time to showcase their flying skills! This evaluation will be by coaches invitation only! We will have flyers from all levels will come in at the same time and be split up into groups based off tumbling levels and prior flying experience. Coaches will then stunt with athletes to determine their ability and appropriate level. All potential flyers still need to attend normal scheduled evaluations so that ALL skills are evaluated. During flyer specific evaluations coaches will be evaluating: flexibility and flyer technique. NO OTHER SKILLS WILL BE EVALUATED during this time.

Attending the flyer evaluation DOES NOT guarantee your athlete will be a flyer. If for some reason you are unable to make the scheduled evaluation dates, you will have to make arrangements to have a private evaluations. There will be a fee of $30 for doing our evaluation outside of the scheduled dates, so it will benefit all athletes to attend all scheduled dates. Team Placement and Choreography: Once all evaluations have been completed, our staff will begin assembling rosters for the upcoming season. Once rosters are set, an email will be sent to each family letting them know results based off the evaluations. This process will be completed by Monday, May 21 st. This email will also include practice days/times, coaching staff as well as specific team coaches names. Team placements are not finalized until August. Athletes who do not have the skills performed during evaluations may move to a different team in August. ONLY those who have shown a significant amount of improvement may be moved up a level. We definitely do the best we can to place our athletes on the appropriate team during evaluations. Skills MUST be maintained in order to stay on teams. MEET THE COACHES: Teams will have an initial meeting with coaches, parents, and athletes after placements on : May 23rd & 24 th Times: TBA (depends on team placement)

CHOREOGRAPHY CAMPS will begin for teams beginning in June (mandatory for each athlete to attend). Teams (Prep/Half Season or Full Season: Prep/Half Season (MAY-DECEMBER) Practice 2/days week for 1-2 hours each Perform at up to 4 competitons throughout the season (No weekend practices with the exception of possible choreography) Full Season Teams (MAY- APRIL) Fall/Spring(Winter) Practice 2/days per week 2-3hrs per week (Some weekend practices)

Tuition/Cost: We try to keep the total cost for our program the most afforable for our families, and like to be as upfront as possible when it comes to fees. We offer affordable monthly payment plans throughout the season that cover most of the program expenses. *Full* Season All-StarTuition (Cheer & Dance) $125/month beginning May 14 th -April 1 st 2019 (full season tuiton includes tumbling and all classes)*see CLASS SCHEDULE* *Full* Season Tuition (Prep Teams) $100/month beginning May 14 th - April 1 st (does not include tumbling or classes) *Half* Season Tuition/Prep Teams (Cheer and Dance) $100/ beginning May 14 th - December 1 st Payments will begin the week of evaluations in May All other payments starting in June will be due the 1 st of each month! Each athlete must have a credit card on file, but you can change form of payment on due date.

CROSSOVERS: Crossover fees are determined per event producer/events attended. No increase in tuition will be due, but additional crossover fees will need to be paid prior to event. Hotel Accomodations: Some events may require hotel stay. If so we will provide you with the team hotel information prior to event. DISCOUNTS: v 10% Discount if paid in FULL v Sibling Discount : 20% off Second tuition (discount will be applied to lower tuition rate if applicable) Payments: v All monthly payments are due at the 1 st of each month. (cash, check, credit card, money order) v Any payments made after the 5 th will result in a $25 late fee v Valid credit card must be on file for all athletes, however you may choose to pay with alternate method on payment due date. v All checks are made payable to: B FLY ATHLETICS v $25 returned check fee for any returned checks. If a second check is returned we will no longer accept check as your payment for the remainder of the season. v Auto Pay will will run on the 1 st of the month.

v Monthly Tuition cannot be prorated for any reason and is Nonrefundable. v By the 15 th of each month all accounts must be in good standing. Otherwise your athlete will be moved to an alternate positon and/or be asked to sit out of practice. v Written notice must be given prior to withdrawing or quitting. No refunds will be issued for any reason. Attendance: We expect 100% commitment and priority from every athlete! CDX is a big commitment throughout the season. Practices will begin in July and continue throughout the season! ALL athletes are expected to attend all competitons as a team. We do know many athletes are involved in other activies, however we can not make exceptions for scheduling conflicts. Please keep in mind we will only allow a total of 6 EXCUSED absences for Full season teams and 8 EXCUSED absences for Half season and prep (full or half) teams. Excused and Unexcused absences are defined in our handbook. Please read carefully. (SEE HANDBOOK)

Parent Communication: As a parent you will receive information in several ways from your Team Manager and/or Coach. Please also make sure to visit our website frequently (www.cheerdancextreme.com) as well as follow our social media outlets to stay up to date on information. PARENTS/ GUARDIAN CODE OF CONDUCT/FINANCIAL AGREEMENT Cheer & Dance Xtreme is making a very strong effort to create a positive and encouraging environment for our athletes. The parent/ guardian agreement form has been created to ask our parents to be active partners in helping our staff at Cheer & Dance Xtreme succeed in these efforts. Please Initial All: I will support and encourage all Cheer & Dance Xtreme athletes, staff members, and coaches at all practices and competitions. I understand that tuition is due at the first of each month and there are no refunds or proration of any fees for any reason. I understand additional practices maybe called prior to a large competition, and I understand there is no additional charge for additional practices. I understand I am required to have a CC/debit card on file for tuition charges. I understand I am subject to a LATE FEE after the 5 th of the designated due date, and excessive late payments can result in your athlete not competing or dismissal from the program. I understand in the event your athlete leaves the program for whatever reason, by signing this contract I am soley responsible for any tuition fees and all fees paid prior are NON- refundable and NON- transferable. I understand CDX is not responsible for travel fees and responsible for any events that maybe canceled or changed by the event producers I will abide by all gym policies. I understand that any violation of the gym policies may result from my athlete being removed from the Cheer & Dance Xtreme program. I understand that any violent or aggressive behavior towards my athletes, parents, coaches, Cheer & Dance Xtreme staff will result in permanent removal from the facility and possible contacting of the authorities depending on the nature of the incident. I will show good sportsmanship by cheering on all of the squads that compete against Cheer & Dance Xtreme. I will not speak negative of athletes, parents, coaches or Cheer & Dance Xtreme staff on any social network sites (Facebook, Twitter, Instagram, etc.) or to any other parent.
