LOCATION: Nelson Park - Lake Decatur, Decatur, IL (Directions located at the bottom of the meet packet) RACE DESCRIPTION: Beach start and finish. The Course is and out and back for all individuals in the 300m course and triangular 1,000m course for all individuals competing in the 1,000m, 2,000m, or 3,000m with laps based on the distance you are swimming. The water depth ranges form 4 feet to 10 feet and water temperature will be in the 70 s

STARTING TIMES: Warm-ups will be available at 8:00 a.m. with the competition beginning at 8:30a.m. for 10 and under and 11 12 age group competing in the 300 meter race. All others will warm-up following the 12 & under 300 meter, which should be at approximately 9:00 a.m. and with the competition beginning at 9:30 a.m. ENTRY LIMITATIONS: Swimmers may enter 1 individual event per day due to the nature of the meet. REGISTRATION: Registration will be easiest if you e-mail your entry list including Name, Birthday, ISI number if they have one, and the event distance. We ask that you bring checks on the day of the event to pay for your entries. DECK ENTRIES: Deck entries will be taken up to 35 minutes before the start of the meet. Deck entrants should sign up at the check in table. Deck entries are $20 for individuals and $40 for relays, Deck entry swimmers new to the meet must provide proof of USA Swimming registration or a USA membership for the day can be purchased for $27.50 ELIGIBILITY: Swimmers must be athlete members of USA swimming. Coaches must be current coach members of USA Swimming and must display their coach membership cards on deck. Age is as of the first day of the meet, June 20, 2009. LATE REGISTRATION: On deck USA Swimming registration will be available for swimmers or coaches but a one-day Open Water USA registration will be available for $27.50 WAIVERS: Please photocopy the enclosed waiver and have this completed by each competitor. QUALIFYING TIMES: A times or ability in the longest freestyle event available for their age group. SWIMMERS WITH A DISABILITY: Entries for swimmers with a disability should be handled in the following manner: (1) enter the swimmer in the Hy-Tek database (or on the regular entry form); (2) list in the email accompanying the Hy-Tek entry file (or on a separate sheet of paper) the name of the swimmer, the stroke(s)/distance(s) s/he wishes to swim, the entry times, the day/session s/he wishes to swim the event(s), and the manner in which s/he prefers to be seeded (with a longer distance or with a different age group); and (3) provide any information about special accommodations needed by the RULES AND REGULATIONS: All current USA Swimming and ISI Rules and Regulations for open water apply. USA Swimming safety rules will be strictly enforced.

RAIN DATE: Rain will not stop this event from happening, but in the case of lighting there will be a 30-minute delay. If the weather looks like it s not going to break the event will be pushed back one day to Sunday the 21 st on June. SWIM WEAR: All USA swimming wear is acceptable, wetsuits will be allowed, but will make you ineligible to win an award. We eventually would like to have a wetsuit division in each age group, but being are first year were going to see how it goes and maybe next year open that up. COACHES: Coaches there is a large grassy area behind the beach to set up a camp site. During the event we are allowing only man powered watercrafts example kayaks, canoes, blow up rafts, etc to be in the middle of the course. If you bring a gasoline-powered watercraft you must stay on the outside of the course and please be careful to run your watercraft only when necessary. The fumes make open water swimming tougher than it needs to be. There will be judge s boats on the corner of each turn for safety and to make sure the swimmers are following the course path.

THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS AND DEPRIVES YOU OF THE RIGHT TO SUE DECATUR SWIM CLUB, MILLIKIN UNIVERSITY AND OTHER PARTIES INVOLVED. DO NOT SIGN THIS AGREEMENT UNLESS YOU HAVE READ IT IN ITS ENTIRETY. SEEK THE ADVICE OF LEGAL COUNSEL IF YOU ARE UNSURE OF ITS EFFECT. IN CONSIDERATION of USA swimming allowing me to participate in any USA swimming sanctioned event Lake Decatur Open Water Challenge as either a member of USA swimming or through the issuance of a single event license or permit; I, for myself, and on behalf of my spouse, children, parents, guardians, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors and assigns, hereby agree to and make the following contractual representations pursuant to this Waiver and Release of Liability, Assumption of Risk and Indemnity Agreement. 1.I hereby represent that I am in good health and in proper physical condition to participate in the Event; and I am not under the influence of alcohol or any illicit or prescription drugs which would in any way impair my ability to safely participate in the Event. I agree that it is my sole responsibility to determine whether I am sufficiently fit and healthy enough to participate in the Event. 2. I understand and acknowledge the physical and mental rigors associated with open water swimming are inherently dangerous and represent an extreme test of a person s physical and mental limits. I understand that participation involves risks and dangers which include, without limitation, the potential for serious bodily injury, permanent disability, paralysis and death; loss or damage to property; exposure to extreme conditions and circumstances; accidents, illness, contact or collision with other participants, spectators, vehicles or other natural or manmade objects; dangers arising from adverse weather conditions; imperfect course conditions; water, road and surface hazards; equipment failure; inadequate safety measures; participants of varying skill levels; situations beyond the immediate control of the Event Organizers; and other undefined harm or damage which may not be readily foreseeable, and other presently unknown risks and dangers. I understand that these Risks may be caused in whole or in part by my own actions or inactions, the actions or inactions of others participating in the Event, or the acts, inaction or negligence of the Released Parties defined below, and I hereby expressly assume all such Risks and responsibility for any damages, liabilities, losses or expenses which I incur as a result of my participation in the Event. 3.I agree to be familiar with and abide by the Rules and Regulations established for the Event, including but not limited to the Competitive Rules adopted by USA swimming and the Guide to Prohibited Substances and Prohibited Methods of Doping adopted by the United States Anti-Doping Agency. I also accept sole responsibility for my own conduct and actions while participating in the Event, and the condition and adequacy of my equipment. 4.I hereby Release, Waive and Covenant Not to Sue, and further agree to Indemnify, Defend and Hold Harmless the following parties: USA swimming, the Event Organizers and Promoters, Race Directors,

Sponsors, Advertisers, Host Cities, Local Organizing Committees, Venues and Property Owners upon which the Event takes place, Law Enforcement Agencies and other Public Entities providing support for the Event, and each of their respective parent, subsidiary and affiliated companies, officers, directors, partners, shareholders, members, agents, employees and volunteers (Individually and Collectively, the "Released Parties" or "Event Organizers"), with respect to any liability, claim(s), demand(s), cause(s) of action, damage(s), loss or expense (including court costs and reasonable attorneys fees) of any kind or nature ("Liability") which may arise out of, result from, or relate to my participation in the Event, including claims for Liability caused in whole or in part by the negligence of the Released Parties. I further agree that if, despite this Agreement, I, or anyone on my behalf, makes a claim for Liability against any of the Released Parties, I will indemnify, defend and hold harmless each of the Released Parties from any such Liability which any may be incurred as the result of such claim. I hereby warrant that I have read this Agreement carefully, understand its terms and conditions, acknowledge that I will be giving up substantial legal rights by signing it (including the rights of the minor, my spouse, children, parents, guardians, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors and assigns), acknowledge that I have signed this Agreement freely and voluntarily, without any inducement, assurance or guarantee, and intend for my signature to serve as confirmation of my complete and unconditional acceptance of the terms, conditions and provisions of this Agreement. This Agreement represents the complete understanding between the parties regarding these issues and no oral representations, statements or inducements have been made apart from this Agreement. If any provision of this Agreement is held to be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Agreement and shall not affect the validity and enforceability of any remaining provisions. DIRECTIONS FROM NORTH, EAST, OR WEST: Take Interstate 72 to Exit 144 (IL-48) Exit on IL-48 Southbound IL-48 becomes 22nd Street within the Decatur city limits Take 22nd Street (IL-48) South approximately 6 Miles Turn Left into Nelson Park at Cantrell DIRECTIONS FROM THE SOUTH: Proceed North on US-51 to Decatur Exit US-51 for Lake Shore Drive Eastbound (IL-105) after crossing the Lake Decatur Veteran's Bridge Follow Lake Shore East approximately 3 miles to the 4 way Stop Sign at Nelson Park Turn Right into Nelson Park EVENT PARKING: Vehicles entering Nelson Park from 22nd Street will be directed to park near the Lake Decatur boat ramps. Overflow parking will be directed to park in the lower lot at Nelson Blvd and Lake Shore Drive.
