2 te: Consideration must be given to any mixing of substances and the products of any reaction. Substances of unknown toxicity should be assigned a High or Very High rating. CHIP-RISK CODES may identify other properties of the substance (eg. flammable) that may entail additional precautions (see control measures). II. Exposure Potential For exposures by inhalation: Consult EH40 Occupational Exposure Limits (OEL) listings (published annually by HSE), available from the safety officer φ. Occupational Exposure Standard (OES) inhalation should be reduced to this level or lower. Maximum Exposure Limit (MEL) must never be exceeded. Substances may have short-term (15min) or long term (8h) limits in ppm or mgm -3 (ppm=gm -3 ). If this does not apply continue with the assessment. PART 4 CONTROL MEASURES I. Special SAFETY Precautions (identified from CHIP-CODES) Specify: NONE II. Engineering Control Measures The entire procedure can be carried out safely on the open bench # Good general ventilation (eg. window open) is required and sufficient Full containment is required during part or all of the procedure*# *Specify the type of enclosure and when it must be used: Partial containment (Local Exhaust Ventilation) is required during part or all of the procedure** **Specify the type of LEV to be used and when it must be used: Where engineering controls are used eg. fume hoods, etc. are these subject to a formal performance test, at least every 14 months, and records kept? If no, this must be arranged. Is air monitoring required to validate control of exposure to ANY of the substances? If YES, then specify the arrangements for this to be carried out: II. Personal Protective Equipment (PPE) Where adequate control of exposure to the hazardous substance(s) cannot be achieved by substitution or engineering controls the following type(s) of PPE will be required for part or all of the activity. Supervision Workers must be aware of who their direct supervisor is for the work (in the laboratory) Are all users aware of who will directly supervise the activity? VII. Implications for Persons t Involved in the Work Activity Identify any persons in the following groups, not directly involved with the work activity, who may be at risk from the hazards of the activity. Academic staff Technical staff Postgraduates Undergraduates Maintenance staff Secretaries Cleaning staff Emergency personnel Contractors Visitors Others Persons identified above may need to be informed, in part or in full, of the information contained in this risk assessment. VIII. Health Monitoring Is monitoring required to ensure that the control of exposure to the hazardous substance(s) is adequate? Are there sensitizers or carcinogens in use? If yes, the Occupational Health Unit must be notified and a surveillance record be completedφ.

5 I. Instructions for the Work Activity All laboratory activities require precise written procedures (Standard Operating Procedure-SOP). A SOP form should be completed using information from this (and any other) risk assessment. A SOP has been completed and is attached (reference n o ). Accreditation and Verification of Assessment The completed assessment must be signed and dated by the person making the assessment and then checked and signed by the senior person with responsibility for the area in which the activity is to be undertaken. 9 2) It is important that the evaluation is based only on estimates of potential exposure arising from the activity itself without additional control measures. The effect of secondary containment, such as fume hoods, should not be included as this would pre-judge descisions on the level of containment required. 3) In deciding on the volatility of a liquid, the volatility at the temperature of use must be taken into account. 4) Primary containment is that containment provided by the apparatus or equipment in which the substance is handled. Secondary containment is the additional containment needed to ensure appropriate control of exposure.
