1 Permit To Work A permit to work system is designed to ensure that all necessary actions are taken before,during,and after most of task on plant. Permit to work are formal documents specifying the work to be done and precautions to be taken. work can only start when safe procedures have been defined and put in place. The permit provides a clear written record, signed by responsible person, that all foreseeable hazards have been considered and all necessary actions have been taken.

3 Purpose All work activity, the hazard involved and precaution to be taken are defined prior to execution of work. All activity are coordinated to provide safe working environment for all personal working on around installation under operation.

4 Do we need permit for every task on site? Certain routine tasks, which do not create any potential hazard to personnel, equipment, or the Site, may not require a Permit. Such tasks shall be listed by the Commissioning Manager for all areas under his control. These listed tasks must be covered by detailed Procedures or Job Safety Analysis to ensure safe performance and must have written approval from the Commissioning Manager for all areas under his control. for instance: vibration monitoring, temperature measurement, hot bolting activity, solo run test, repair and lamp replacement, transformer check, inspections and checking of air fin coolers for noise, greasing, check and calibrate control valve, check and verify alarms, burners flame scanners.

6 Main Naked Flame MF Any work involving equipment giving rise to a definite source of ignition. Oxy-acetylene gas cutting Gas welding, electric-arc welding Electric-arc welding, Propane torch heating And burning, Grinding, use of a hot-air gun And certain stress relieving Equipment for piping(pwht)

8 Main Hot MH Any work involving, or giving rise to, a potential source of ignition. Using non-intrinsically safe equipment for instance: lap top, Mobile phone, Camera with flash, drilling machine Opening of explosion proof fittings in hazardous areas All electrical work, unless the equipment is fully isolated,insulation activity when is require drilling machine

12 Complementary Permit There are 8 Separate complementary Process or Mechanical Isolation. Entry to Confined Spaces *this includes Vessel Entry. Excavation. High Voltage Electrical Isolation. Low Voltage Electrical Isolation. Sanction for Test. Limitation of Access. Radiography.

13 Process or Mechanical isolation Process Isolation: means Valve Isolation, can be achieved using a combination of the following: Single valve isolation: Only for short duration when there is no alternative,an acceptable isolation method if the equipment to be worked on less then 40 bar pressure and non toxic, at ambient temperatures. Double block: Nipples,nozzels,vents,drain,etc not used for routine operation must be capped or blanked off. For connections need frequent routine operation and sampling, must have double valve isolation Double block and bleed: A typical assembly is vented spoolpiece section of line between two block valves

14 Double, double block and bleed isolations Mechanical isolation: means Positive isolations, physical disconnection Physically disconnections Blinding/de-blinding or spading/de-spading Remember mechanical isolation can not be done until adequate process isolation done completely. Vessel entry: isolation requirement for entry Inside vessels Or Confined spaces is by physical disconnection. if this is not practical then spading is acceptable as an alternative. Remember mechanical isolation is mandatory for vessel entry and Work involving naked flame Work involving naked flame: if welding involved, then spading plus physical disconnection is required.

19 Confined Space entry A confined space is defined as a space: Is large enough for an employee to enter and perform work, Has limited or restricted means for entry or exit, Is not designed for continuous human occupancy, (tanks, vessels, pipes, sumps, or pits) area from which escape could become difficult REMEMBER: Mechanical isolation is mandatory for confined space entry This complementary permit valid only for only (12hours) if work under this permit continue beyond 12 hours a new permit must be issued.

22 High or low voltage Isolation This Complementary permit is required to work under safe conditions on normally energised equipment. This can be High voltage or Low voltage electrical isolation. High voltage from 33 to 6kv (In this refinery) Low voltage less then 400V This Isolations Can be done only by authorized electrical person This complementary are normally valid for duration of related Main Permit. Remember Push Button is not acceptable as Electrical Isolation

23 Tag Out/Lock Out System A summery of all electrical energy sources and associated electrical isolation devices, these must be identified and isolated in order to achieve a zero energy state prior to the performance of any specified maintenance, inspection or construction activity. This padlock shall not be removed till relevant permit cancelled and completed. The only person could removed padlock is SAEP/AEP (Senior Authorize Electrical Person/Authorized Electrical Person). Each substation operator is responsible for implementing,monitoring and auditing the lockout process

25 Sanction for test This complementary Permit is required to test individual or multi-discipline items of equipment. it allows for temporary removal of safety isolations and provision of temporary actions to override or remove equipment to carry out such tests. Sanction for test valid only for one shift incase of extended they have to apply another one. This complementary valid for only one shift

27 Limitation Of Access This complementary Permit is required to work any area close to live electrical system or equipment. A limitation of Access is not required where the work to be performed is already covered by any other permits and isolations, and shall be authorized by Senior electrical persons.

Ensures that persons working under his all control comply with permit condition at time. 2- Area authority: Authorizes to work go ahead Define plant status for work. 3- Area operator: To check,sign and tagged isolations 4-Permit Controller: Checking of permits before issue. Ensure no conflict of work between area authorities 5-safety Authority: Audit and inspection working practice 6-Senior authorize electrical person (SAEP): Performs electrical isolation 7-Senior panel operator: Applied required inhibits on fire and gas and ESD systems for work to be proceeded.

31 Permit Life Cycle INITIATION / PREPARATION OF PERMIT TO WORK LIST OF EXTERNAL REQUIREMENTS SUCH AS: SAFETY EQUIPMENT, STATUS OF PLANT, NEED FOR COMPLEMENTARY PERMITS ETC PERMIT AUTHORISATION, ISSUE AND ACCEPTANCE FOR WORK TO START PERMIT REVALIDATION, SUSPENSION OR CANCELLATION ON COMPLETION OF WORK

33 IMPORTANT It is strictly prohibited for the same person to sign as Task Supervisor/Work Organiser and as Authorizing Authority. New permit application have to be at permit office before17:00on the day before work required to start. Dayshift permits must be at permit office by 18:30 or they shall not be revalidated for the following day.

35 Plant Status Requirements Define by Area authority And Area operator Ensures any required equipment status including isolations are in due place.

36 Fire and Gas,ESD System Requirements Define by Area authority and perform safety system override by senior panel operator

37 Complementary Permits to attach required complementary by Initiator and Area Authority to check and confirm

38 Authorization Area authority the work to proceed and Permit Controller check the permit and the work may proceed.

41 Authorized Gas Tester to perform gas test prior commencing task. If task suspended or incomplete with any reason Task supervisor to declare reason of suspension. Work party to filled by Task supervisor prior enter to restricted area.

42 Distribution White (Top) Copy, in permit office Green copy, with Safety Authority form checking of continuous compliance Pink Copy, kept at the work site, on display, for duration of activity Yellow Copy, with Area Authority for checking of continuous compliance The Main Work Permits are valid for maximum 7 consecutive days
