2 Case Studies for Group Discussion MODULE 5, LESSON 5 RESPONDING TO CARBON MONOXIDE Background This next lesson focuses on the most potentially dangerous product of incomplete combustion, carbon monoxide. In addition to leak and odor complaint calls from customers, you may also handle situations involving the presence of carbon monoxide in a customer's home or their business. It is important that students understand their company s policies related to investigating carbon monoxide calls. You will serve as the expert for this case study and will work with your students to introduce them to the hazards of incomplete combustion. Directions If you have 8 or more students, divide them into small groups of 4-5. Instruct them to read the entire case study silently. Then ask them to discuss the talking points quietly in their group. Call on volunteers to open up a group discussion. Scenario Since carbon monoxide is an odorless and colorless gas, sometimes it is difficult to detect. In this lesson, students will learn some general guidelines for detecting carbon monoxide. They ll also learn that exposure to carbon monoxide can make them sick or can even lead to death. The very young, the elderly, people with heart disease, and those under the influence of alcohol, drugs, or medication are particularly sensitive to the effects of carbon monoxide. So, it s very important that we understand our roles. Many gas suppliers immediately refer customer complaints about a carbon monoxide hazard to the local fire department. However, companies may choose to investigate such customer complaints. In this case, employees should always treat the call as a high priority and always...always always refer to their company policy for specific guidelines and procedures on responding to and investigating carbon monoxide calls. Notes for Instructor The talking points provided will help to guide the students as they progress throughout the lesson. Use these talking points to tap into any previous experience or background knowledge for an engaging class discussion. 2
3 Talking Points for Discussion Why do you think it s important to understand your company s policies related to investigating carbon monoxide? Since exposure to carbon monoxide can make you sick or can even lead to death, what precautions might you take when responding to carbon monoxide? Does your company choose to investigate potential carbon monoxide complaints? If so, why do you think they always treat the call as a high priority? MODULE 8, LESSON 3 FILLING CYLINDERS BY WEIGHT Background This lesson focuses on a hypothetical case study based on a character named Jim. While he is a sharp and hard working propane employee, he has a few things to learn about correctly filling cylinders. You will serve as the expert for this case study and will work with your students to examine the areas that Jim needs to correct and improve. Guided answers for the discussion points have been provided for you and may be used as a reference. Directions If you have 8 or more students, divide them into small groups of 4-5. Instruct them to read the entire case study silently. Then ask them to discuss the talking points quietly in their group. Call on volunteers to open up a group discussion. Scenario Jim was a fairly new employee to the propane industry. One workday began when his boss asked him to fill all of the empty forklift cylinders at a dock using the weight method. Since he had a large number of cylinders to fill, Jim went right to work. Jim set the beam scale and weighed the first three cylinders. He found that they all had similar tare weights. He had a large number of cylinders to fill. Jim thought the first several cylinders had almost the exact same tare weights and they looked identical to the others waiting to be filled. Therefore, he thought he'd be more efficient by setting the scale according to the weight of the first three cylinders when filling the rest of the containers. In the afternoon, all of the filled cylinders were transported to the customer location. The customer site happened to be much warmer than the cylinder dock where the containers were filled. The heat of the afternoon caused the propane to expand in several of the cylinders, which activated relief valves and released product. Unfortunately, a fire resulted and caused substantial damage to a storage unit. Worst of all, one of the emergency responders was injured. Talking Points and Guided Answers for Discussion Discuss policies and procedures that Jim should have considered when filling cylinders and what, if anything, would you have done differently? 3
4 This case study shows how the actions of one employee may lead to very dangerous conditions. So, let's talk about what should have happened if the proper cylinder filling procedure had been followed. Before anything, Jim should have put on his personal protective equipment. In this case, gloves and eye protection should have been worn. Jim also should have looked for sources of ignition on the dock, located the nearest fire extinguisher, and checked to see if the scales were accurate. Your company probably has policies related to personal protective equipment. Make sure you know and abide by them. Jim incorrectly assumed that since all the cylinders looked the same and were the same size, the tare weights would also be similar. While that seems like common sense, the mistake could have been avoided if Jim had followed the proper procedures. All of the cylinders that Jim needed to fill were steel forklift cylinders, except for one. Mixed in with all the steel cylinders was an older aluminum cylinder, which has a much lighter weight. So you can begin to see why you need to check the tare weight each cylinder. Jim didn t reset the scale, so when he went to fill the aluminum cylinder, the calculations were off. The difference in weight was made up with additional propane, and the cylinder was overfilled. Discuss the properties and characteristics of propane that should have guided Jim. The cylinders were delivered to the customer on a day that became very warm. The propane expanded on the warm day and since the aluminum cylinder was overfilled it activated the relief valve and caused a release of product. This resulted in a fire and injury. You can see by this example that Jim should not have assumed that the tare weights were the same in all cylinders. Instead, he should have followed the proper procedures for filling cylinders by weight and checked each container for its tare weight. Keep this story in mind as we progress through the rest of the lesson. 4
