4 ToeOFF Family Introduction ToeOFF Family Introduction ToeOFF The Quantum Leap in AFO Technology Introduced in 1997, ToeOFF is now recognized as a quantum leap in AFO technology. Today hundreds of thousands of individuals with gait impairment now enjoy a higher quality of life, thanks to the stability and dynamic response provided when wearing one of our patented carbon composite orthoses that are members of the ToeOFF Family of Products. With more experience than any company in the industry and as the world leader in carbon composite AFO technology, our product development team continues to extend the opportunity for improved gait and increased walking capacity to more individuals as they expand the product offering in the ToeOFF Family of Products. 4

5 Anterior Design The ToeOFF family of products comes up from the footplate onto the anterior surface of the leg to avoid pressure on the calf muscles and Achilles tendon. It allows for a floor reaction response to assist with knee extension. Lateral strut The strut is placed on the lateral side of the foot to create stability and function. It also allows for more functional use by bilateral users. Light weight The composition of Carbon fiber, fiberglass and Kevlar produces an extremely lightweight yet structurally superior design, which is of great benefit to the user. Graded Stability To be able to accommodate different users needs, the sizes and products are graded in both stability and dynamic response. Thin The extreme thinness of the product makes it light and almost invisible under slacks or trousers. Comfortable As the shin plate comes on the anterior surface of the leg it provides for a more comfortable design for the user. Dynamic footplate The unique layup and the shape of the footplate contributes to a more dynamic and functional gait pattern. Fits in shoe The thinness of the foot plate and ability to trim it often avoids the need to increase the shoe size. Open heel The open heel design allows the Calcaneus to invert and evert which happens naturally during the gait cycle and allows the biomechanical chain reaction to occur. 6 ToeOFF Family Indications & Contraindications Indications & Contraindications ToeOFF Family of Products Intended Use Ypsilon, ToeOFF, ToeOFF 2.0 and BlueROCKER are intended to support a foot with impaired ability to actively dorsiflex. They are also intended to assist when the ability to perform push off is reduced. KiddieGAIT and KiddieROCKER are intended to support the foot/ankle complex in a more functional posture while allowing more normal ROM during the developmental years. Indications Ypsilon, ToeOFF, ToeOFF 2.0 and BlueROCKER are designed to manage footdrop in conditions such as Stroke, Multiple Sclerosis, Post Polio Syndrome, Muscular Dystrophy, Spinal Cord Injuries, Traumatic Brain Injuries, Guillian-Barre Syndrome, Charcot-Marie-Tooth, Myelomeningocele, Neuropathy or Cerebral Palsy. ToeOFF, ToeOFF 2.0 and BlueROCKER are designed to support gait in conditions such as Posterior Tibialis Tendon Dysfunction (PTTD) and toe amputations. BlueROCKER can also be used for patial foot amputations, most proximal level is Chopart. KiddieGAIT and KiddieROCKER are designed to support footdrop, gait deviations secondary to proprioceptive deficit (either unstable or low-tone gait), toe-walking with no midfoot collapse, low tone crouch gait in conditions such as Spina Bifida, Cerebral Palsy, Muscular Dystrophy and Myelomeningocele. Contraindications Ypsilon, ToeOFF, ToeOFF 2.0 or BlueROCKER should not be used when patients present with: Foot and/or leg ulcers Moderate to severe edema Moderate to severe foot deformities Severe proximal deficits (e.g. quadriceps spasticity, genu valgum or varum, genu recurvatum) Severe spasticity Contraindications KiddieGAIT and KiddieROCKER should not be used when patients present with: Lacking ROM towards dorsiflexion (need at least 5 o dorsiflexion past neutral) Very rigid foot structure Quadriceps spasticity Fixed postural Genu Valgum or Genu Varum Fixed postural Pes Valgus or Pes Varus Limitations When Genu-recurvatum cannot be orthotically managed (such as with insoles or wedges), COMBO (a knee orthosis attachment for ToeOFF and BlueROCKER) may be a suitable solution. 7 Ypsilon, ToeOFF, BlueROCKER, KiddieGAIT and KiddieROCKER are products that look very similar. However, they meet very different user needs. To determine which product is most suited for a specific patient, follow the steps given in this manual very closely. The following scale correlates product rigidity to patient stability, when not wearing an orthosis. Ypsilon For less involved cases of footdrop Allows more medial, lateral and rotational ankle movements. Long strut (leverage arm) Allows orthosis to adapt to and move with the lower leg. Less resistance to ground reaction forces. Strut extends lateral to instep Greater instep clearance. Proximal ends of the Y provide the tibia fixation. Tibia crest clearance. ToeOFF KiddieGAIT For moderate involved cases of footdrop Mild to severe footdrop together with mild to moderate ankle instability. Full coverage, anatomically shaped anterior tibia shell Tibia stabilization and control. Short strut wraps over instep. Medial-lateral and rotation control of the foot and ankle complex. BlueROCKER KiddieROCKER For more involved cases of footdrop Footdrop with multiple involvement, severe ankle instability and/or proximal weakness/deficits. 8 NAVIGAIT / Ypsilon TM NAVIGAIT Navigait is an alternative for users with a mild foot drop. The amount of dorsiflexion is easily adjusted with an elastic coil to accommodate the user s specific needs. Indications: Intended to support a foot when the ability to actively dorsiflex is reduced due to conditions such as Stroke, Multiple Sclerosis, Post Polio Syndrome. Contraindications: Navigait should not be used by patients who present with spasticity or severe oedema. 9 ToeOFF ToeOFF is the original carbon composite dynamic response floor reaction ankle foot orthosis. This is the standard product that is suitable for many different indications and patient groups. It not only keeps the foot up during swing phase, it also gives a soft heel strike, stability in stance and good toeoff. The ToeOFF should always be worn with a soft interface between the leg and the anterior shell. 10 ToeOFF 2.0 ToeOFF 2.0 ToeOFF 2.0 is the updatede version of the Original ToeOFF. This new model have several new fetures that has been asked for from customers over time. The shape is more organic, with shorter side extensions and more rounded shape. Also the surface is smoother and more scratch resistant. The MikroFIX is pre-attached on the inside of the brace to accomodate a faster fitting procedure. It is also available with two different strap systems - With D-ring or without D-ring. Another feature is the increased height on size X-Large with 2 cm more compared to the Original ToeOFF. 13 SoftSHELL TM For individuals that prefer a softer look, SoftSHELL combines the padding system of SoftKIT with a lycra textile front to form a sleeve that slides over the anterior shell. It includes a matching cover for the lower strut. Available in Beige or Brown. 15 BlueROCKER BlueROCKER is identical in shape and design as ToeOFF but offers more rigid orthotic control. Developed primarily for bilateal patients and those with more involved pathologies. The extra stability will improve both balance and posture and give the wearer greater security, especially individuals with weak quadricep muscles. Usually the preferred orthosis to be used in conjunction with a socket and toe filler for management of partial foot amputations. 17 KiddieGAIT Introduction KiddieGAIT should always be combined with an additional foot orthotic, designed to control the position of the foot. It is important that the foot position is as close to neutral as possible when bearing weight. The most common foot related problems like pronation, supination, pes varus, pes valgus should be corrected with this additional foot orthotic. When spasticity is present, it is generally recommended that this foot orthotic include a deep heel cup to further encourage the heel-to-toe gait. The following are guidlines only. When the foot related problems are: Mild = use a Foot Orthotic that is deep and firm Moderate = UCB (Deep Foot Orthotic with trim line below ankle) Severe and/or tight Achilles tendon = SMO (Supra malleolar Orthosis or Molded Inner Boot) For low tone (hypotonia) and pronation, consider a soft SMO, such as SureStep UCB SMO SMO 17

19 KiddieGAIT SoftSHELL TM YOUTH The SoftSHELL for KiddieGAIT comes in two different versions depending on size.
