7 1st Stage Site survey identify of hazards & Controls Selection Identify the constraints / hazards To seek advise from WAH Equipment Manufacturers & Professional Engineers on Selecting of Appropriate Control Measures WSH Act Manufacturers / Suppliers Ensure that any machinery, equipment or substances you provide is safe for use. You are required to: provide proper information on the safe use of the machinery, equipment or hazardous substance ensure that the machinery, equipment or hazardous substance is safe for use ensure that the machinery, equipment or hazardous substance has been tested and examined so that it is safe for use To check against manufacturer s specification To engage engineering design for control measures that match the manufacturer s specification with the site application Manufacturer & Product code Subject to conformity testing to meet standard specification Sample of a Conformity Test Certificates Material description: Class B connector, in aluminum alloy 7075, with locking gate device by screw, length 114mm, opening 24.5mm Testing conformance to EN

9 2nd Stage Installation of Controls & Testing Its Effectiveness Assess hazards post to Installers To engage Professional Rope Access Personal for the installation To ensure installer are certified Manufacturer s Competent Installers Testing of installed control measures Test the system with full strength in operation Monitor the system functionalities with full operation in progress Adjust & improve the controls to meet operation needs, through :- o o Reviewing of risks; and Evaluating on the rescue plan in an event of a fall 2nd Stage Installation of Controls & Testing Its Effectiveness Assess hazards post to Installers To engage Professional Rope Access Personal for the installation To ensure installer are certified Manufacturer s Competent Installers WSH Act - Equipment Installers Ensure that the person who erects, installs or modifies any machinery or equipment for use at work is safe when properly used. You are required to: erects, installs or modifies any machinery or equipment was in accordance with the information supplied by the designer, manufacturer or supplier it s reasonably for the person to rely on that information 9

10 2nd Stage Installation of Controls & Testing Its Effectiveness Assess hazards post to Installers Testing of installed control measures WSH (WAH) Regulation Reg. 22 Apply for PTW Application be made by the supervisor of the person who is to carry the hazardous WAH. Refer to ACOP WAH 2013, section Person performing supervisory roles must ensure that :- Required safety measures are in place before commencement of work Test the system with full strength in operation Monitor the system functionalities with full operation in progress Adjust & improve the controls to meet operation needs, through :- o Reviewing of risks; and o Evaluating on the rescue plan in an event of a fall 3rd Stage Evaluation of Controls ACOP WAH 2013, section WAH Assessors The appointed work at height assessor must be suitably trained and competent to assess and advise on work at heights safety (eg. Attended relevant WAH courses for Safety Assessors accredited by the MOM. The Assessor could be a WSH professional or any persons responsible to assess the WAH safety. WSH (WAH) Regulation Reg. 23 Evaluation of PTW Application On receipt of the application for a permit-to-work, the workat-height safety assessor shall (a)assess whether all reasonably practicable measures have been taken, and (b) inspect the site (including its surroundings together with the supervisor of the person who is to carry out the work, to ensure that the hazardous work at height can be carried out with due regard to the safety and health of the person and any other person at work in the workplace who may be affected. 10

12 Review the effectiveness of Control Measures 1) Once all the fall arrest equipment are installed, 2) Trial run the work process sequence with actual simulation of job flow. 3) Using Observation to verify the effectiveness of the fall arrest equipment fall prevention, and 4) Try to foreseen any potential hazard that the control measures might have incurred on the work process; examples: obstruction to hoisting 5) Look out for any potential obstruction to emergency rescue. 13 4th Stage Competency Training & Individual Responsibility Brief all personal that are affected by the risks, on the controls Conduct training to ensure correct application and usage of the controls For complex system ensure workmen go on job training with demonstration to clear all doubt Monitor operation to ensure that all workmen perform the job in according to the approved SWPs WSH (WAH) Regulation Reg. 6 Training for Persons at Work The duty of Responsible person is to ensure that No person shall work at height unless he has received adequate training on work at height. To familiarize himself with the hazards and the precautions to be observed. WSH (WAH) Regulation Reg. 10(b) & 11(b) The duty of Responsible person is to ensure that Every person using the control measures is trained in the Safe and Correct use of the System. 14 Operation / Execution of Jobs Duties of Persons at Work Source from OCH Division, MOM 13 th Dec 2012 Presentation Slides Operation / Execution of Jobs Supervision Competent Person appointed to provide proper & adequate supervision Especially important when workers are undergoing training / are new / inexperience & unfamiliar Competent person must ensure safety measures are in place before work commence Only workers who have received appropriate training & instruction in relate to the tasks they are to perform are to carry out the work. WSH (WAH) Regulation Reg. 7 Duty of Responsible Person of any person who carrying work at height to ensure that he shall work at height except under the immediate supervision of a competent person for that work who has sufficient experience and training to perform the work required to work at height. ACOP WAH 2013, Responsible Person, in relation to a person who carries out or is to carry out any work at height, means a) His employer; or b) The principal under whose direction he carries out or is to carry out any such work. 14

15 Operation / Execution of Jobs Review / Monitoring Ensure Control Measures are safe and do not introduce new risks Check on the control measures effectiveness and are in good working condition Do not post expose installers / removers of these controls to a greater risk that the one it is design to control WSH (WAH) Regulation Reg.26 (1) Duty of Authorized Manager for any hazardous work at height to continue review the progress of all hazardous WAH being carry out on or after 1 st May 2014 in the workplace is carry out with due regards to safety (2) Duty of Supervisors to ensure that the measures necessary to ensure safety & health of the person at work are taken and are in place at all time during the validity period of the PTW. 16 Supporting Operations Supporting Operations Inspection & Maintenance Ensure end-users are trained to conduct pre-use check of all fall prevention equipment Ensure that the anchorage point / lines are inspected by competent person before each work cycle. The frequency of inspection will increase with the intensity of use. Proper maintenance is scheduled to ensure the serviceability of equipment WSH (WAH) Regulation Reg. 12 (2) (1) Where a travel restraint system or a fall arrest system is to be used in a workplace, it shall be the duty of the responsible person to appoint a competent person to inspect the anchorage and anchorage line at the start of every work shift to ensure that the anchorage line are in good working conditions and safe for use. (2) To immediately remove from services when is found to be defective 16

The corrective action to prevent re-occurrence of accident ACOP WAH 2013, Section 3.9 Accident / incident Investigation All accidents or incidents relating to falls from height should be investigated and analysed for the following objectives: Determine any underlying deficiencies Identify the need for corrective action Identify the opportunities for preventive actions All findings and outcomes should be properly documented & communicated. Supporting Operations Emergency Preparedness Key Stakeholders are aware of the Emergency response plan Rescue methods / equipment is made available Trained designated rescuers & First aid Know how to apply and instruct the application of the Self Relief Technique to prevent Suspension Trauma from starting. An effective and readily available means of communication WSHA Sect. 12 & 14 (5) To developing and implementing emergency procedures; ACOP WAH 2013, section 3.10 Emergency Response A written emergency response plan must be established. Immediate rescue of a person after an arrest fall can prevent the onset of injuries such as suspension trauma. 17
