1 Membership Renewal Skaters, Parents, Coaches, Officials and Friends of Skating: It is time to renew your membership for the skating season. The new membership year begins July 1, 2015 and ends on June 30, Please read this letter in its entirety, as some of the information has changed from previous years. Beginning this year our membership will be administered online via Entryeeze. It is quick and easy to renew your membership. To get started, click on the link in the e- mail you received regarding renewal, or the link on the Club s website ( If you have not yet signed into your Entryeeze account for membership, you will need to set up a login and password. Please note, this login and password are different from any login information you may have setup with Entryeeze for a competition account. Once you have your login information, follow the directions below to renew your membership. Memberships must be renewed before July 1, Be sure to complete your renewal by 11:59 pm on June 30, 2015, so your membership does not lapse. Memberships received after June 30, 2015 will be assessed a $25.00 late fee and are not eligible to be considered for competitor funding. Test applications or competition entries will not be approved for memberships that have lapsed. Officials must renew memberships to continue to judge or officiate at competitions or test sessions after July 1, Please note entries for competitions occurring after July 1, 2015, will not be approved until your membership has been renewed. There is an increase to the dues for the membership year. Membership dues and fundraising are the main sources of income for the club and since our fundraising has been down the past few years, it is necessary to increase our dues to help support the Club. Our dues have not been increased since the membership year. Senior, Junior/Senior and Subsequent Year Introductory Junior members are subject to the volunteer policy. The policy is included in the membership packet and must be signed each year in order to activate or renew your membership. The volunteer hours will remain at 13 hours for full members and 6 hours for subsequent introductory junior members. Hours for families with multiple skaters are included in the volunteer policy. For the membership year beginning July 1, 2015, members will be required to enter the volunteer hours they have performed into their Entryeeze account. This can be done by logging into your Entryeeze account and entering the hours on the volunteer tab. The club s volunteer coordinator will then verify the hours. Please note, if you have not yet fulfilled your volunteer requirement for the current membership year, or we do not have a record of your volunteer hours, your membership will be marked as not in good standing in Entryeeze. You will not be able to renew your membership or sign up to test until your membership is in good standing. Your U.S. Figure Skating membership will not be renewed until you are in compliance with the volunteer policy. If you have not yet submitted your volunteer hours, please send them to our volunteer coordinator, Jodi Brinson, at rmfsc.volunteer@yahoo.com as soon as possible so they can be recorded. Please make sure the information listed in your Entryeeze account is correct, and the e- mail in your account is the e- mail you want to receive club communications at. If your e- mail changes during the membership year, please be sure to update it in your Entryeeze account and on your US Figure Skating member account. Your e- mail is used for club communications only and will not be distributed to third parties. If you have questions regarding your membership level please refer to the description for the membership category. If you still have questions, check with your coach or the Membership Chair. The introductory junior membership is available to beginning skaters who have never been a member of USFSA (other than a Basic Skills member), have not taken any USFSA tests (other than Basic Skills), and will not be testing in the upcoming year. Skaters may compete with the introductory junior membership. If you are currently an introductory junior member, you may remain an introductory junior member and compete as a RMFSC member, as long as you are not planning to test in the upcoming year. However, you must pay the subsequent year introductory junior member dues and will be subject to the volunteer policy. Directions for renewing your membership on Entryeeze: Click on the link in the e- mail you received regarding membership renewal, or on the club s website and set up your user ID and password (if you have not already done so). Log into your Entryeeze account Click on the Apply/Renew tab The membership type should be filled in, but can be changed if it is incorrect Click the renew button by each member s name (you must do this for each member you are renewing) Verify and update your personal information and click on SAVE Answer YES/NO to the US citizen question Sign the Waiver and Release of Liability, Assumption of Risk and Indemnity Agreement; Parental Consent and Indemnification Agreement and Consent for Medical Attention or Treatment; the U.S. Figure Skating Parent Code of Conduct (for skaters under the age of 18; Member Code of Conduct and Code of Ethics; and Volunteer Policy. Please be sure to read the all of the waivers and policies included with the renewal as some of them have changed. Your electronic signature acknowledges that you have read the RMFSC policies and agree to abide by them. Choose the areas you are interested in volunteering for Choose your special skills and interests Repeat the above steps for all members in your family Click Cart/Checkout at top right, then Continue and process your payment We accept Visa, MasterCard and Discover. If you are not able to pay with one of these cards, you may pay by check. You will still need to enter all of your information, but DO NOT check out. Please leave your check in the lockbox in the skater s locker room. Please note, entries processed via check, will be charged a $5.00 administrative fee in addition to the 3.5% Entryeeze processing fee. If you are transferring from another club, no transfer forms are necessary if you are joining RMFSC at the beginning of the membership year. Please indicate your previous club in the application. If you are joining as an associate member, please include your home club on your application. Your home club membership must be current in order to be a RMFSC associate member. 5 Rocky Mountain Figure Skating Club Waiver and Release of Liability, Assumption of Risk and Indemnity Agreement ( Agreement ) In consideration of participating in Rocky Mountain Figure Skating Club activities, I represent that I understand the nature of figure skating activities ( activity ) and that I am qualified, in good health and in proper physical condition to participate in such activity. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that this activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the activity, the conditions in which the activity takes place, or the negligence of the releasees named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the activity. I hereby release, discharge, and covenant not to sue the Rocky Mountain Figure Skating Club, United States Figure Skating, it s directors, officers, administrators, sponsors, volunteers, agents, employees, staff, instructors, trainers, other participants and if applicable, owners and lessors of premises on which the activity takes place (each considered one of the Releasees herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the releasees or otherwise, including negligent rescue operations; and I further agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the releasees from any loss, liability, damage, or cost which any may incur as the result of such claim. 7 U.S. Figure Skating Parents Code of Conduct Codes of Conduct give everyone a guide to what is expected of us if we are part of an organization, participating in a sport, or as spectators at our child s events. Preamble: The essential elements of character-building and ethics in sports are embodied in the concept of sportsmanship and six core principles: respect, responsibility, fairness, caring, trustworthiness and good citizenship. The highest potential of sports is achieved when competition reflects these six pillars of character (Arizona Sports Summit Accord) By signing below I hereby agree that: 1. I will encourage good sportsmanship by demonstrating positive support for all skaters, coaches and officials at every practice session, competition and test session. 2. I will place the emotional and physical well being of my child ahead of my personal desire to win. 3. I will encourage my child to skate in a safe and healthy environment. 4. I will inform my child s coach of any physical disability or ailment that may affect the safety of my child or the safety of others. 5. I will teach my child that doing one s best is more important than winning, so that my child will never feel defeated by the results of a competitive event. 6. I will never ridicule or yell at my child or other participant for making a mistake or blame my child s teammates for placement in a competition. 7. I will do my best to make skating fun and will remember that my child participates in sports for his/her own enjoyment and satisfaction not mine. 8. I will ask my child to treat other skaters, coaches, fans, and officials with respect, regardless of race, creed, color, sexual orientation or ability. 9. I will applaud a good effort in both victory and defeat emphasizing the positive accomplishments and learning from the mistakes. 10. I will teach my child to resolve conflicts without resorting to hostility or violence. 11. I will be a positive role model for my child and other skaters. 12. I will demand a figure skating environment for my child that is free of drug or alcohol abuse and agree that I will not use or provide to a third party any drug proscribed by applicable federal, state, or municipal law.

I will not assist or condone any competing athlete s use of a drug banned by the International Olympic Committee, International Skating Union, United States Olympic Committee, or U.S. Figure Skating, or, in case of athletes, to use such drugs or refuse to submit to properly conducted drug tests administered by one of these organizations. 14. I will not provide alcohol to, or condone the use of alcohol by minors, abuse alcohol in the presence of athlete members, or at U.S. Figure Skating activities or, in the case of athletes, consume alcoholic beverages while a minor. 15. I will encourage my child s coach to continue their education and training through programs offered by U.S. Figure Skating, the Professional Skaters Association and other accredited organizations. 16. I will respect my child s coach and refrain from coaching my child or other skaters during competitions and practices because it may conflict with the coach s plan or strategies. 17. I will respect the decisions of officials and their authority during competitions and test sessions and teach my child to do likewise. 18. I will show appreciation and recognize the importance of volunteers and club officials. 19. I will study the rules of the U.S. Figure Skating and teach my child accordingly so that we have an understanding and appreciation of the rules of competition and membership. 20. I will support all the opponents in my child s competition and respect the rights of all skaters to participate. Signature
9 MEMBER CODE OF CONDUCT AND CODE OF ETHICS Adopted July 2014 Amended June 2015 General Guidelines: 1. All members (home club and associate) and their families must adhere to this Code of Conduct and Code of Ethics at all times. 2. All members and their families must exhibit good sportsmanship and be courteous toward their fellow skaters, coaches, family members of skaters, U.S. Figure Skating officials, rink personnel, and guests. U.S. Figure Skating Member Code of Conduct GR 1.02 The following code of conduct applies to all participants in activities hosted, supported, sponsored or engaged in by U.S. Figure Skating and RMFSC, including but not limited to competitions, exhibitions, training camps and local rink activities. I recognize that my participation or my child's in all the activities associated with U.S. Figure Skating and RMFSC is an honor and privilege that carries certain responsibilities. I agree to fully abide by the rules and guidelines set forth by U.S. Figure Skating or its properly designated agents. As a precondition to participation in activities hosted, supported, sponsored or engaged in by U.S. Figure Skating, I will adhere to the following tenets in good faith: A. I will exhibit the highest standards of fairness, ethical behavior and genuine good sportsmanship in all of my relations with others. B. I will not damage public or private property. I understand that I may be held financially responsible for damage deemed to be wantonly or willfully executed on my part, and that I may be subject to disciplinary action by U.S. Figure Skating. C. I will not use or possess illegal drugs, and I will not engage in criminal activity. I understand that, if I am found to use or possess illegal drugs, or if I am found to engage in any criminal activity during any activity hosted, supported, sponsored or engaged in by U.S. Figure Skating, I may be subject to criminal penalties as well as penalties imposed by U.S. Figure Skating. D. I will adhere to the rules of U.S. Figure Skating and the host organization at all activities hosted, supported, sponsored or engaged in by U.S. Figure Skating.
10 E. I will comply with all applicable anti- doping rules including, but not limited to, ISU and USADA anti- doping rules. F. I will conduct myself in a manner not detrimental to the welfare of figure skating. I understand that my actions reflect on U.S. Figure Skating and the sport of figure skating both positively and negatively. I understand that if my acts, statements, or conduct are considered detrimental to the welfare of figure skating by the appropriate authority, I may be subject to penalties imposed by U.S. Figure Skating pursuant to GR G. I understand that the penalties that may be imposed may include, but are not limited to, loss of future international selections, loss of financial support from U.S. Figure Skating and its Memorial Fund, and loss of participation in activities hosted, supported, sponsored or engaged in by U.S. Figure Skating. H. I understand that all disciplinary proceedings will be conducted pursuant to Article XXV, Section 3, of the U.S. Figure Skating bylaws, and that my rights and remedies are derived therefrom. Skaters: Parents: 1. All skaters must comply with ice etiquette and safety rules. 2. All skaters must demonstrate good sportsmanship, courtesy, and respect at all times, ON AND OFF THE ICE. 3. Outbursts of temper, hostile behavior, verbal abuse, physical threats or attacks, poor sportsmanship, profanity, rudeness, yelling, or any other inappropriate behavior ON OR OFF THE ICE will not be tolerated. 4. Skaters must refrain from making or participating in inappropriate, malicious, or disparaging comments regarding other skaters, family members of skaters, clubs, coaches, U.S. Figure Skating Officials, or ice rink personnel. 5. Skaters should avoid any action or conduct that might disrupt the club or interfere with the camaraderie of the club. 1. Parents shall enjoy their child s opportunity to experience the benefits of figure skating. 2. Parents must demonstrate respect, caring, honesty, safety and good sportsmanship at all times. 3. Parents must respect their child s coach and support his/her efforts. This includes but is not limited to refraining from sideline coaching or coaching from the stands. 4. Parents must respect the decisions and authority of officials during any RMFSC event. 5. Parents are responsible for abiding by U.S. Figure Skating rules. 6. Parents must not demonstrate threatening or abusive behavior, or use foul language.
11 Coaches: 7. Parents must refrain from making or participating in inappropriate, malicious, or disparaging comments regarding other skaters, family members of skaters, clubs, coaches, U.S. Figure Skating Officials, or ice rink personnel. 8. Parents are responsible for reporting any perceived misconduct by any member of RMFSC in accordance with the RMFSC SafeSport Reporting Policy. 1. Coaches must obey and abide by all U.S. Figure Skating published rules, regulations and procedures, including the timely completion of all applicable annual coaching member requirements set forth by U.S. Figure Skating Rules and the PSA. 2. Coaches must obey and abide by all RMFSC policies, including the RMFSC SafeSport policies. 3. Coaches shall maintain exemplary standards of personal conduct. 4. Coaches must obey all state, national and international laws. 5. Coaches must maintain a figure skating environment free from alcohol abuse, illegal drugs or substances, which could impair ability or judgment as a coach. 6. Coaches must refrain from using any profane or abusive language. 7. Coaches must not engage in sexual harassment or sexual misconduct with any athlete. 8. Coaches must teach and support the athletes fairly and equitably. 9. Coaches must never discriminate against any athlete. 10. Coaches shall conform to the PSA Code of Ethics and Tenets of Professionalism. Any person who makes groundless allegations or complaints of abuse or harassment may be subject to disciplinary action per Article XXV, Section 3B, of the U.S. Figure Skating bylaws. Violations: Depending on the severity of the offense, discipline may include the following: First offense: club member will be addressed verbally (in private) regarding the violation. Second offense: club member will be removed from the activity and given a written warning. A meeting with the member and/or member s parents may be held.

13 Rocky Mountain Figure Skating Club Volunteer Policy July 2015 Volunteer support is an essential part of our club, as our only source of revenue (other than a portion of the membership dues) is fundraising. Without the help and support of its members through volunteer hours contributed, the Club cannot perform its functions. To that end, members are required to comply with the Club s Volunteer Policy. The Volunteer Policy must be signed by each skater (or skater s parent/guardian) in order to initiate or renew Club membership. Noncompliance with the Volunteer Policy may result in your membership not being in good standing with the Club. In addition, US Figure Skating will be notified that your membership is not in good standing. The volunteer requirement is subject to change each membership year and will be recorded on the membership application annually. The purpose of this policy is to offer our skaters more support and, over time, expand club activities and benefits. There are many volunteer opportunities that can be fulfilled both at the rink and from home. Volunteer opportunities will be sent via e- mail or through VolunteerSpot or other such volunteer organizing tools. Only RMFSC activities, test sessions, and competitions hosted solely by RMFSC or in conjunction with Mile Hi Figure Skating Association are eligible for credit. Volunteer hours for RMFSC s Synchro team will count toward the hour requirement, so long as the hours and activities are verified. Playing of music for contract sessions is done through the Ice Centre and therefore, not eligible for volunteer credit. The above are examples of volunteer activities; they are not intended to be inclusive of approved or non- approved opportunities. Questions regarding eligibility of hours should be approved in advance or you may not receive credit for those hours. Questions should be directed to the Volunteer Coordinator via at: rmfsc.volunteer@yahoo.com. The Volunteer Policy does not apply to Collegiate, Associate, First Year Introductory Junior, Official, Honorary or Coach memberships; but your assistance is welcome and appreciated. Members are responsible for entering their volunteer hours into their Entryeeze account upon completion of their volunteer activity. The hours will then be verified by the volunteer coordinator. To ensure proper credit and verification, volunteer hours must be submitted by the end of the month in which the activity was performed. Hours that cannot be verified may not receive credit. Hours will be tracked in 15- minute increments. Any member of a skater s family may perform volunteer hours on the skater s behalf; however gifting of hours from one skater to another is not allowed. Hours may not be carried over from one membership year to the next. All volunteer hours must be completed by June 30 th of the membership year. If you sign up to volunteer, we will do everything in our power to give you the hours you applied for. However, if you are not needed and/or do not work those hours for some reason, they will not count. If you do not fulfill your volunteer hours, you will be charged a flat fee of $300 for full members and $150 for introductory junior members. The fee is due by June 30 of the membership year end. The fee will not be prorated for partial fulfillment of hours. If you do not pay the fee, you will not remain in good standing with the club and will be flagged as such with USFS. This will result in your ineligibility for competitor funding, testing, competitions etc. If your membership is flagged with US Figure Skating, you will not be able to transfer to another club, and your membership will remain not in good standing until your financial obligations to RMFSC are paid. Members are encouraged to help as much as possible and we appreciate you keeping track of all hours, even after the minimum requirement is met. This not only helps to spread the work load and make fun, successful events, but it helps us to anticipate hours needed in future years.
