1 Program No: SP-010 Page PURPOSE AND INTRODUCTION Fall Protection and Elevated Work To establish fall protection safety procedures for Pitt County Schools employees engaged in any of the following work situations: Use of stairway floor openings, railings, guards and toe boards, Work on, or use of, wall openings and holes from which there is a drop of more than 4 feet, Work on, or use of, open sided floors, platforms and runways in which there is a drop of more than 4 feet, Use of portable and/ or fixed ladders, Use of bucket trucks or moveable platforms (lifts), and/or Work in construction workplaces with unprotected sides and edges (i.e. roofs) that are more than 6 feet above a lower level 2.0 SCOPE All Pitt County Schools employees. 3.0 RESPONSIBILITIES 3.1 Program Directors, Site Managers, and Principals Responsible for the oversight and adherence of all employees to the Fall Protection program. 3.2 Safety Coordinator Responsible for evaluating and revising this procedure annually or as regulations change. 3.3 Employees a) Responsible for abiding by this procedure and wearing appropriate PPE in designated areas and conditions.
2 Program No: SP-010 Page 2 b) Ensure that all equipment used is functioning properly. c) Responsible for the care and condition of issued PPE. 4.0 TRAINING AND CERTIFICATION Read and understand this procedure. 5.0 DEFINITIONS PPE Personal Protective Equipment. Examples safety glasses, fall harness 6.0 EQUIPMENT AND MATERIALS Fall protection harness 7.0 INSTRUCTIONS 7.1 Portable Ladders All ladder types should be free of defects and hazards (i.e. sharp edges, etc.) The selected ladder should be of sufficient strength to meet test requirements set by OSHA, and shall be protected against corrosion, unless inherently corrosion resistant. Ladders will also meet specifications for rung spacing, step type, general specifications, minimum width for side rails, and maximum length as set by OSHA regulations Ladders must be maintained in usable condition at all times Ladders should be inspected prior to each use for damage or defects, and should be immediately inspected if they tip over, or are exposed to oil, grease, or other chemicals Any ladder having defects should be marked and taken out of service until it is repaired When ascending or descending, employees must face the ladder When using a stepladder make sure the spreader is in the locked position.
3 Program No: SP-010 Page Ladders must not be tied or fastened together to provide longer sections Ladders must not be used as braces, skids or for any other purpose other than its intended use, unless specifically recommended by the manufacturer Use the correct ladder for the job. If the ladder is not long enough, use a ladder that is the correct height (step ladders should not be longer than twenty feet however) Do not stand on the top four rungs of a ladder or the top two steps of a stepladder Never move, extend, or shift a ladder while someone is still on it Never stand off-center, or reach too far while on the ladder Never let go of a ladder with more than one hand. Use a belt, shoulder strap, or rope hoist to carry tools up or down Never use a metal ladder close to power lines. 7.2 Fixed Ladders If any fixed ladder is damaged or defective, notify your supervisor so that it can be repaired or taken out of service. 7.3 Bucket Trucks and Moveable Lifts Only trained employees shall operate the bucket truck or lift (i.e. genie lift) Employees shall always stand firmly on the floor of the basket or lift, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position A body harness shall be worn and a lanyard attached to the boom, basket, or lift when working from the equipment.
4 Program No: SP-010 Page The brakes shall be set and outriggers shall be positioned on pads or a solid surface. Wheel chocks shall be installed before using a truck or lift on an incline The bucket truck nor the lift shall be moved when the boom is elevated in a working position with employees in the basket, unless the equipment is specifically designed for that use The bucket truck shall have both platform (upper) and lower controls, with the upper controls easily accessible to the operator and the lower controls capable of overriding the upper controls in the case of an emergency The insulated portion of the lifting platform or bucket (if applicable) will not be altered in any manner that might reduce its insulating value. 7.4 Roof Work The provisions of this section do not apply when employees are making an inspection, investigation, or assessment of the workplace conditions prior to the actual start of work at elevations above six (6) feet, or after work is completed. Employees are expected to exercise good judgment and safety practices while inspecting these locations. When the employee begins, the actual work will follow all provisions of this section Personal Fall Arrest Systems Employees will use personal fall arrest systems whenever they are engaged in roof work. Facility Services uses the following as the means of personal fall arrest protection (to prevent a fall): Body harness- system of straps that the employee wears that distributes the forces of a fall over at least the thighs, pelvis, waist, chest and shoulders and is attached to other components of the fall arrest system. Deceleration device - the rope grab that attaches the lanyard to the lifeline and locks, by friction, to the lifeline. Lanyard- a flexible rope that has a connector at each end and allows for attachment of the body harness to a lifeline.
5 Program No: SP-010 Page 5 Lifeline - the flexible rope that anchors the fall arrest system to arrest, or prevent, a fall by the employee Employees will inspect their fall arrest system prior to each use for wear, damage, or deterioration, and defective parts will be removed and replaced. In addition, any fall arrest system that is subject to impact loading will be immediately taken out of service and not used again until inspected by a competent person and determined to be suitable for use Warning Line Systems with Safety Monitors Warning lines with safety monitors may be used in combination with personal fall arrest systems to provide additional protection while the employee is engaged in roofing work for extended periods of time. In addition, there may be times where working conditions prohibit the safe use of personal fall arrest systems. Warning lines with safety monitors will not be used though as a substitute for personal fall arrest systems, unless: The use of the personal fall arrest system increases the danger of performing the job, and The Director of Facility Services or the Safety Coordinator has given permission.
