3 Application for Entry - (Applications will only be accepted through the online application platform) TELEVISION, RADIO, MOTION PICTURE, PHOTOGRAPHIC, ELECTRONIC, INTERNET AND SIMILAR RIGHTS/DECLARATIONS: The undersigned hereby grants and assigns to PGA TOUR, Inc., without limitation, the undersigned s individual television, radio, motion picture, photographic, electronic, internet Interactive Applications (as defined below) and all other media rights with respect to the undersigned s participation in the qualifying tournament or any portion thereof (i.e., at all times on-site during Tournament week and during any Tournament-related activities), or any other footage of the undersigned that may be captured by PGA TOUR Entertainment or its contractors with the undersigned s permission, with the understanding that PGA TOUR s use thereof shall be limited to the following: (1) advertising, promoting or publicizing the Mackenzie Tour - PGA TOUR Canada, PGA TOUR, PGA TOUR charitable initiatives, tournaments or any broadcasts thereof; (2) live broadcasts during tournaments, (3) broadcasts or exhibitions on a recorded basis as part of a program or motion picture of tournaments or year-end summaries of sports award programs or any news of sports news programs; provided, that in no case shall the rights granted hereunder be used in any program or motion picture designed for instructional purposes without the express written consent of the player(s) involved; and (4) live or taped non-exclusive updates. Interactive Applications shall mean the presentation in any medium in a way that permits the viewer to interact with or manipulate the presentation or access other relevant information during the presentation. The undersigned agrees to refrain from any action which will interfere with PGA TOUR s ownership of the rights hereunder granted and assigned to PGA TOUR or with any authorized use thereof. The undersigned also agrees to abide by and be governed by the PGA TOUR Tournament Regulations currently in effect (and, if a member of PGA TOUR, attest that a copy thereof has been furnished to me) and by the Rules of Golf of the United States Golf Association, subject to any modifications thereof approved by PGA TOUR. The undersigned hereby declares that they have the financial capacity to support themselves on the Mackenzie Tour - PGA TOUR Canada for one full year and that the foregoing information furnished by player in the Application is true and complete; and understands that any material false statements herein furnished by player may result in their expulsion from the Tournament and the suspension or termination of their playing privileges in tournaments cosponsored, approved or endorsed by PGA TOUR. DRUG TESTING: I understand that drug testing may be conducted on players pursuant to the PGA TOUR Anti-Doping Program ( the Program ). As a potential member of the Mackenzie Tour - PGA TOUR Canada, I consent to and agree to be bound by all requirements of the Program specifically including but not limited to my agreement: to provide a blood and/or urine sample to any testing agency authorized by the PGA TOUR at any time or place without advance notice; that refusal to provide a sample, if requested, will constitute an anti-doping rule violation; that any sample collected shall become the property of the PGA TOUR; that the PGA TOUR may disseminate information pertaining to me relevant to the Program, as permitted by the Program; and that the Results Management provisions of the Program shall provide the sole and exclusive method for resolving any dispute related to drug testing. Integrity The PGA TOUR Integrity Program is intended to maintain integrity and prevent betting-related corruption in PGA TOUR competitions. I consent and agree to be bound by all terms of the PGA TOUR Integrity Program Manual (the Integrity Program Manual ) with respect to my participation in the Tournament. The Integrity Program Manual, among other things: prohibits me from betting on the Tournament and providing inside information to third parties for purposes of betting on the Tournament; requires me to report any violations of the Integrity Program Manual to the PGA TOUR; and establishes a process for the monitoring, investigation, sanctioning and disclosure of violations of the Integrity Program Manual. By submission of this entry, I agree to the following: I agree to the regulations on Exhibit A included herewith and I agree that my entry is subject to rejection at any time. I agree that certain risks are inherent to the game of golf and accept personal and sole liability for all such risks. I agree that the responsibility of ensuring this application is received by the Mackenzie Tour - PGA TOUR Canada by no later than the applicable entry deadline as set forth on Exhibit A is mine alone and that this application will be automatically rejected if it is received by the Mackenzie Tour - PGA TOUR Canada after such date and time. Procedures & Information: Format A 72-hole stroke play competition with the low 40 qualifiers and any player tied for 40 th place earning eligibility status for the 2019 Mackenzie Tour - PGA TOUR Canada season. The winner of each site will be exempt for the entire 2019 season. Finishers 2-14 will be exempt through the reshuffle following the completion of the sixth event in In the event of a tie for 1 st or 14 th position, there will be a hole-by-hole playoff immediately following play. Finishers and any player tied for 40 th place will receive conditional status to the Mackenzie Tour - PGA TOUR Canada for the 2019 season. Field Size Maximum of 120 players. All 2018 members of the Mackenzie Tour - PGA TOUR Canada, PGA TOUR Latinoamérica, PGA TOUR Series - China, Web.com Tour Members & 2019 Web.com Members will receive priority with regards to their preferred site choice if their applications are received before 12:00 PM (EST) on January 11 th, Member applications which are not received prior to 12:00 PM (EST) on January 11 th, 2019 will be subject to the timelines set forth by the general entry period. Members missing the priority deadline and all non-member entries will be assigned on a first-come, first-serve basis. The Mackenzie Tour - PGA TOUR Canada reserves the right to balance the field size at qualifying sites by assigning any applicant, including members of the aforementioned tours whose applications are received prior to January 11 th, to other sites as deemed necessary. Should the fields reach the maximum number of players, an alternate list will be created based on the date and time of receipt of application. There is only one (1) alternate list; the Qualifying Tournament is considered one competition (120 players multiplied by 6 sites = 720 players). The 721 st application received is the first alternate and so on. If and when a spot becomes available at any of the six sites, it will be offered to the next alternate. If an alternate chooses not to accept a spot, he will in effect be withdrawing and will receive a full refund. An alternate cannot defer an available spot and remain on the alternate list. Any unfilled spot will be then offered to the next alternate and so on until the spot is filled. If an alternate is not offered a spot (i.e., due to an insufficient number of withdrawals) they would receive a full refund. Note: This process will change when occurring within one week of the first round of competition at the first five sites. If a spot becomes available at a specific site beginning the week prior to and through the final starting time of the first round, the next alternate can decline the spot and still retain his place on the alternate list. However, if a spot opened up at either of the remaining site(s) during that same week before that time or after declining the spot, he would have to accept that spot at the other site or withdraw his entry. Entry Deadline Friday, February 1 12:00 PM (EST). Only online entries will be accepted. Notifications of site assignments will be determined no later than Friday, February 8 th. Information regarding local accommodations will be included in site assignment notifications. Entry Fees Entry fee of $2,750 USD must accompany application and be payable by credit card (Visa or MasterCard only; American Express will not be accepted. Use of a Visa Debit card is not recommended). Entry fee includes: green fee, range usage on the practice and competition days and liability insurance coverage, covering liability which may arise as a result of the applicant s presence at a Qualifying Tournament site. Eligibility Professionals and any Amateurs with a handicap index when applied to the qualifying course slope will generate a course handicap of two (2) or less. The PGA TOUR reserves the right to verify the handicap index of amateurs and to refuse anyone not meeting the requirements.

5 In accordance with the Mackenzie Tour - PGA TOUR Canada Tournament Regulations, a player must be 18 years of age or older to be eligible for membership. A player under the age of 18 may enter the Mackenzie Tour - PGA TOUR Canada Qualifying Tournament, provided the player turns 18 on or before the first scheduled round of the final official money Mackenzie Tour - PGA TOUR Canada tournament of the 2019 season. If a player under the age of 18 qualifies for the Mackenzie Tour - PGA TOUR Canada, the player would not become eligible for the Mackenzie Tour - PGA TOUR Canada membership until his 18 th birthday. Starting Times Starting times will be available two days preceding the Qualifying competition at the particular venue or online at Withdrawals & Refunds Applicants who are placed in Qualifying Tournament fields may withdraw but will be subject to the following processing fees. Should the applicant withdraw from their respective field prior to the dates listed below at 5:00 PM (EST), they will be assessed a processing fee of $250 USD. Should the applicant withdraw from their respective field after the dates listed below, they will be assessed a processing fee of $1,000 USD. Applicants must withdraw by contacting Mackenzie Tour PGA TOUR Canada in writing via (MackenzieTourWD@pgatourhq.com). Contestants who withdraw during the round may be subject to disciplinary action. Full refunds will be offered to those who withdraw while on the alternate list. Practice Rounds There will be one scheduled Practice Round day before each Qualifying Tournament (i.e., the first date listed in the Site Information table above). Additional practice rounds including the charge per green fee are at the discretion of the host facilities. Practice rounds on the official practice days will be awarded on a firstcome, first-served basis. No tee times will be accepted. To practice outside of the official practice day at your assigned site, please call the host venue and book a tee time once you have been assigned to that site and site-specific information has been distributed. Rules of Golf Play shall be governed in accordance with the Rules of Golf of the United States Golf Association/Golf Canada as modified by Mackenzie Tour - PGA TOUR Canada Local Rules and Conditions of Competition card and supplemented by the Notice to Competitors at each Qualifying Competition. The Rules of Golf, as established by the United States Golf Association (USGA), have undergone significant changes that will be in effect as of January 1 st, As such, play will be contested in all Qualifying Competitions and throughout the Tournament Season using the 2019 Rules of Golf. Information pertaining to the new Rules of Golf, Local Rules and Conditions of Competition will be provided to applicants in advance of their Qualifying Competition. Additional information and any questions pertaining to the Rules of Golf or the competition itself should be directed to the Mackenzie Tour - PGA TOUR Canada Rules Committee. In addition, questions or disputes (including equipment rulings) shall be settled by the on-site Rules Committee appointed by the Mackenzie Tour - PGA TOUR Canada, whose decision shall be final. Pace of Play will be in accordance with the Mackenzie Tour - PGA TOUR Canada Pace of Play Policy except that the fine structure will not apply. The condition of competition for Groove Specifications in Elite Competitions will be in effect for the Qualifying Tournament. Distance-measuring devices are permitted for use during the Qualifying Competition. Only a distance measuring feature may be utilized; other features such as slope are prohibited from use during official competition rounds. See Rule 4.3a of the Rules of Golf, effective January 2019, for more information. Spike Policy The use of traditional metal spikes is permitted during the Qualifying Competition. Soft spikes may be required during practice rounds at the discretion of the host facilities. Transportation Players and Caddies shall walk at all times during the Qualifying Tournament, except when permitted to ride by the Rules Committee. Players are not required to have a caddie; they may carry their clubs or use a manual or motorized pull cart. Anyone seeking a golf cart due to a disability defined under the Americans with Disabilities Act (ADA) or other applicable law should contact the PGA TOUR Competition Department at (904) to obtain information on procedures for submitting such requests. All such requests due to temporary injuries or impairments that do not qualify as a disability under the ADA or other applicable law will not be granted. Appearance Players: While on the host facility property, players shall present a neat appearance in both clothing and personal grooming. Blue jeans, shorts, cutoffs, T-shirts or shirts with no collar shall not be worn. Caddies: While on the host facility property, caddies may wear either long pants or tailored solid colored knee length shorts or skorts. Blue jeans, cutoffs, T-shirts or shirts with no collar shall not be worn. Smooth rubber sole shoes are required. Sandals, open toe shoes, and golf shoes with traditional metal or soft spikes are prohibited. Conduct While on host facility property during qualifying rounds; players and caddies shall conduct themselves in a professional manner. PGA TOUR reserves the right to remove a player from the qualifying competition for; non-competitiveness; conduct unbecoming a professional by the player; conduct unbecoming a professional by

6 the player s caddie. Non-Competitive Performance The Mackenzie Tour reserves the right to remove an applicant from the competition if an applicant returns a non-competitive score after any round of the Mackenzie Tour Qualifying Tournament. Refunds will not be granted for any player removed from the competition for returning a non-competitive score. Reduction or Cancellation Any reduction or cancellation in the respective qualifying competition shall be in accordance with Reduction/Cancellation Guidelines as found in the PGA TOUR Tournament Regulations. In the event that the competition is canceled, all entry fees will be refunded. If, in the judgment of the on-site Mackenzie Tour - PGA TOUR Canada Tournament Director or designee, whose decision shall be final, conditions render commencement or continuation of tournament play inadvisable, play shall be postponed until such time as the Mackenzie Tour - PGA TOUR Canada Tournament Director or designee determines that such weather conditions or other occurrences or conditions have improved sufficiently for play to commence or resume. In situations involving adverse weather conditions or any other occurrence or condition, our first priority is the completion of the qualifying competition. Every effort will be made to achieve this objective by the end of the scheduled competition day. In the event the qualifying competition cannot be completed by the end of the scheduled competitive day, play will be extended. Every effort will be made to complete the qualifying competition as soon as possible. The competitions will not continue beyond the end of the day immediately following the last scheduled tournament round of the competitions. The Bad Weather Policy will be posted on site. Travel As stated above, the Bad Weather policy allows for an additional day of competition in order complete play. Historically, at least once per year, the final round and playoff have been completed on the fifth day. Please plan travel accordingly. Membership The winner after the conclusion of play in each Qualifying Tournament will earn exempt membership for the 2019 Mackenzie Tour - PGA TOUR Canada season. Finishers 2-14 will be exempt through the reshuffle following the completion of the sixth event in 2019, unless the Mackenzie Tour - PGA TOUR Canada, in its sole discretion, elects not to award membership based on other factors. Conditional membership will be awarded to players that finished th and ties at each Qualifying Tournament. The deadline to submit an Application for Membership and payment of $ USD for annual dues and liability insurance is May 8, NOTE: Players finishing in the lowest 40 and players tied for 40 th place who are amateurs, are under 18 years of age or who choose not to take out membership following the completion of the qualifying school are still included in the lowest 40 and ties for membership eligibility and ranking purposes. Priority is based on finish position at each site in alternating order beginning with the 15 th place finisher from USA EAST #1 (Mission Inn Resort & ) [i.e., 15 th place finisher from USA EAST #1 (Mission Inn Resort & ), 15 th place finisher from USA CENTRAL (), 15 th place finisher from USA WEST #1 (Carlton Oaks Golf ), 15 th place finisher from CANADA (Crown Isle Resort & Golf ), 15 th place finisher from USA WEST #2 (Wigwam Golf ), 15 th place finisher from USA EAST #2 (TPC Sawgrass), 16 th place finisher from USA EAST #1 (Mission Inn Resort & ), 16 th place finisher from USA CENTRAL (TPC San Antonio), 16 th place finisher from USA WEST #1 (Carlton Oaks Golf ), 16 th place finisher from CANADA (Crown Isle Resort & Golf ), 16 th place finisher from USA WEST #2 (Wigwam Golf ), 16 th place finisher from USA EAST #2 (TPC Sawgrass), etc..]. Ties will be broken at each site for the purpose of ranking on the 2019 eligibility list by matching final round scores. Ties remaining after the match of final round scores will be broken by matching individual hole scores in descending order, beginning with the 18 th hole of the final round. NOTE: Conditional membership does not guarantee players starts in Mackenzie Tour - PGA TOUR Canada events. In 2018, the number of conditional members who earned starts in the first four events (i.e., through the first of two re-shuffles) ranged from fifteen (15) to twenty (20). NOTE: In the event a starting field at any of the sites fails to reach 100 players, the number of players earning status will be reduced. The winner of the site will be exempt through the entire 2019 season. The number of players earning exempt status until the first reshuffle will be the next 12% of the starting field, rounded up in case of a non-whole number. The number of players receiving conditional status will be the next 18% of the field and ties, rounded up in case of a non-whole number. Orientation At the conclusion of each Qualifying Competition, players who are eligible for membership with the Mackenzie Tour - PGA TOUR Canada may attend an orientation session to be held immediately following the final scheduled competition round. Please plan your travel accordingly. Performance Benefits The top five players on the 2019 Mackenzie Tour - PGA TOUR Canada Order of Merit will receive Web.com Tour membership for The player who finishes 1st on the 2019 Mackenzie Tour Order of Merit will receive exempt status on the Web.com Tour for the 2020 season with players 2-5 earning conditional status. Finishers 2-5 will be placed in the Web.com Tour conditional eligibility category that includes finishers from the Web.com Tour Regular Season, Additional Finishers (i.e., beyond 40 th place) from the Web.com Tour Qualifying Tournament Final Stage and finishers 2-5 from PGA TOUR Latinoamérica and PGA TOUR Series - China, in alternating order as follows: #76 WCT Regular Season, #41 WCT Qualifying Tournament, #2 PGA TOUR Latinoamérica, #2 Mackenzie Tour - PGA TOUR Canada, #2 PGA TOUR Series - China, #77 WCT Regular Season, #42 WCT Qualifying Tournament, #3 PGA TOUR Latinoamérica, #3 Mackenzie Tour - PGA TOUR Canada, #3 PGA TOUR Series - China, and so on. Players 2-5 will also be exempt into the final stage of the 2019 Web.com Tour Qualifying Tournament. Players who finish 2 5 on the 2019 Mackenzie Tour - PGA TOUR Canada Order of Merit are not guaranteed Web.Com Tour starts in Players finishing in positions 6-10 on the 2019 Mackenzie Tour - PGA TOUR Canada Order of Merit will be exempt into the Final Stage of the 2019 Web.com Tour Qualifying Tournament. Players finishing in positions on the 2019 Mackenzie Tour - PGA TOUR Canada Order of Merit will be exempt into the Second Stage of the 2019 Web.com Tour Qualifying Tournament.

7 All 2019 Mackenzie Tour - PGA TOUR Canada members will be exempt into the First Stage of the 2019 Web.com Tour Qualifying Tournament. The top 60 players on the 2019 Mackenzie Tour - PGA TOUR Canada Order of Merit will be fully exempt for the 2020 Mackenzie Tour - PGA TOUR Canada season. The winner(s) of the first two events of the 2019 Mackenzie Tour - PGA TOUR Canada season immediately preceding the RBC Canadian Open (Freedom 55 Financial Open Vancouver, BC and Bayview Place DC Bank Open presented by Times Colonist Victoria, BC) will each receive a sponsor invitation to the 2019 RBC Canadian Open, an official PGA TOUR event.
