1 GIRLS JUNIOR PGA CONDITIONS OF ENTRY INFORMATION A stroke play competition based on the lowest gross score. Any junior who wishes to advance to the Girls Junior PGA Championship must play from the same tees as the oldest age division. OFFICIAL ENTRY APPLICATION Participants must qualify in the PGA Section in which they reside. Entry must be filed on an official entry application, or on-line for those sections which have on-line entry available. PGA SECTION CHAMPIONS Completed entry applicaitions with the applicable entry fee, must be received by the PGA Section contact as noted on this application prior to the entry deadline. Please make your check payable to and mail it to your local PGA Section office. The Champion from each of the 41 PGA Sections will earn an exemption into the Girls Junior PGA Championship. The Section Champion is identified based on the lowest gross score in the oldest age division. CHAMPIONSHIP INFORMATION 144 Girls who are 18 and under through the final round of the Girls Junior PGA Championship on July 12, Players are not allowed to have caddies at any stage of the Championship, except in practice rounds. Application for entry is subject to acceptance by the PGA Section and the PGA of America, the administrator of the Championship. Applicant agrees that, in the event of acceptance, she may be rejected at any time and stage of the Championship. PGA reserves the right to limit entry, adjust dates, format and other details to fit conditions prevailing at the time of the event. Log on to JuniorPGAChampionship.com for more information.

The National Championship (if I qualifiy) will be held at Kearney Hill Golf Links in Lexington, Kentucky from July 9-12, The applicant and the parent/guardian certify: 1) that the applicant is an amateur player, as defined by the USGA; 2) that the conditions of entry have been read and understood and approval has been granted in submitting his/her entry; and 3) that the information given in this application is true to the best of her knowledge. In consideration of the applicant s participation in The PGA Section Championships, Qualifiers and The Junior PGA Championship: 1) the applicant and the parent/guardian hereby release The PGA of America, The PGA Section, PGA Tournament Corporation, Inc.,Kearney Hill Golf Links, host sites of PGA Section events, sponsors and all of each company s respective officers, directors, employees, agents, subsidaries and affiliates from any and all liability associated either with acts or omissions of such parties, or with accidents or injuries sustained by the applicant or parent/guardian in connection with the tournament; 2) the applicant grants and assigns her individual media/photo rights respecting participation in the competition, without exception to the sponsors, PGA of America, PGA Tournament Corporation, Inc., and the PGA Section. Applicant Information will be used for internal purposes only in conjunction with PGA of America activites and programs. Additionally, The PGA of America will provide event sponsors of the PGA Junior Series and Junior PGA Championship with the names and mailing addresses of its participants. It is a condition of participating in this event that the competitor is not permitted to use a motorized golf car during the event competition. Notwithstanding the foregoing, The PGA of America is committed to conducting golf competitions within the parameters of the Americans With Disabilities Act. Players who request relief under the Americans With Disabilities Act and seek a reasonable accommodation for thier disability by the use of a motorized golf car during competition shall be required to submit their request in writing to the Special Board on or before the entry deadline date for review and determination.
