1 2015 WASHINGTON COUNTY 4-H HORSE FAIR ENTRY FORM Must be completed in black or blue ink only INSTRUCTIONS PLEASE READ FIRST! 1. Enrolled 4-H members must complete this form and return it, along with a completed copy of current 4-H Horse Record and confirmation by mentor of progress on Horse Advancements for each horse exhibited to their 4-H mentor to register for the 2015 Washington County 4-H Horse Fair. 4-H Members must also have accrued a minimum of 100 participation points and provided documentation as requested by the points program administrator to validate participation in activities reported. 4-H Members who have not met the minimum participation points can include a list of additional activities they plan to do in order to meet the required points to show at Horse Fair (such as Pre-Fair, Leader s Show, volunteer work, etc.) All activities must be completed before the first day of Horse Fair and proof of participation will be required before exhibitor numbers will be released. Mentors must sign off on completion of necessary participation points on the entry form before numbers will be released. 2. Mentors must check the accuracy of the entry form and sign, date, and submit all paperwork to the OSU Extension Office by June 1, 2015 or as requested by the Fair Superintendent after this date. 3. Horse entries with attached copy of 4-H Horse Records, confirmation of points program accrual, Horse Advancements, optional Genny Olmstead award application, and the optional I Did It Myself application are due to the extension office by close of business on JUNE 1, Participants are to read, know and abide by all information in the 2015 Washington County Fair 4-H Exhibitor s Handbook, 4-H Horse Contest Guide and Project Manuals, and the Washington County 4-H Horse Guidelines and Rules. 5. The Washington County Fair Board, OSU Extension Service, Washington County 4-H Leaders Advisory and related persons assume no responsibility for any loss, injury or damage to animals, equipment or exhibits. 6. If your horse is leased, a copy of the lease agreement must be attached to this entry. 7. ONE ENTRY FORM PER 4-H MEMBER. 8. Enter the horse number, exhibitor division and all classes you wish to enter. No additions after June 1, If you are sharing a horse or tack, or have special needs please make a note of it in the comments section. 10. If you enter any Walk/Trot class (other than introductory dressage), you may not enter other equitation classes, because loping and cantering are required in the Jr., Int. and Sr. equitation classes. 11. Mentors and parents must initial 4-H member entries for the member to be eligible to compete in jumping, gaming, and precision driving. 13. Keep a copy of this completed entry along with your original 4-H Horse Records, Advancements and other documentation. 14. Parents, mentors, and the 4-H member must sign this form for the member to show in the 4-H Horse Fair. 15. Exhibitors in Walk/Trot and Novice classes are NOT eligible for Medallion call back classes and cannot qualify for State Fair. 16. Walk Trot and Novice riders are not allowed to compete in jumping classes. Walk Trot exhibitors that compete in gaming events will be required to do walk / trot for these events (no loping/cantering/galloping.) 17. Ground work Showmanship entries (ie foal, yearling, 2 year old, etc.) are not eligible to participate in Master Showmanship competition. 5 Parent and Member signatures are required below for members to be eligible to participate in 4-H Horse Fair Activities MEMBER CODE OF CONDUCT Your participation in 4-H activities carries the responsibility of representing Oregon 4-H to the public. You are expected to conduct yourself in a manner that reflects well on your state, county, club, as well as yourself. Your contribution to the program is as important as what you receive from the program. 1. You are expected to attend all parts of the planned program. Inform those in charge if you are not feeling well or have a schedule conflict. 2. Observe hours established for barn and camp areas. Complete your work and leave the barn by the posted closing hours, and be in the camp area by the curfew time. 3. Dress appropriate to the occasion. At all times be courteous, clean, and display good manners. Language must be appropriate and respectful of others. No swearing. 4. Participants are not to leave the assigned program area at any time without permission of their mentor or fair superintendent, except as part of the planned program. (Examples: Campsite, fairground, etc.) 5. Participants will not use tobacco, alcohol, drugs (except medication prescribed by a doctor), fireworks or firearms or remain in the assigned program area when they are being used. 6. Shoplifting or theft of public or personal property will NOT be tolerated. 7. Avoid roughness and damage to program area and equipment. Participants are responsible for any damage or misconduct H events are to encourage interaction among all members of the group, but not exclusively with another person. Kissing and other sexual displays of personal affection distract from the group and are not appropriate behavior. MENTOR / PARENT CODE OF CONDUCT 1. Mentors/Parents are expected to be courteous and respectful of fair staff, volunteers, and fairgrounds staff. 2. Mentors/Parents will abide by directions of superintendents and staff. 3. If a Mentor/Parent has issues, you will address the superintendent or 4-H agent. 4. At no time is the Mentor/Parent allowed to discuss or argue with a judge. VIOLATORS MAY EXPECT: 1. To have the opportunity to explain actions to staff in charge. 2. Behavior that is disruptive to the event will be noted and a letter describing such may be sent to parents and county 4-H leadership. 3. Violation may result in dismissal and the offender being sent home at parental expense. 4. Violations involving numbers 5 and 6 above will result in the offender being sent home at the earliest convenience at their parent s expenses and can result in criminal charges. 5. Behavior that is disruptive by Mentor/Parent may cause dismissal of member and Mentor/Parent from 4-H event participation. I have read the code of conduct above and agree to all expectations.
