1 Lithonia Lighting Voluntary Product Recall of Indoor HID Acrylic Light Fixtures To Our Customers: In cooperation with the U.S. Consumer Product Safety Commission, Lithonia Lighting is voluntarily recalling certain fixtures manufactured at its Crawfordsville, Indiana, plant from November 2002 through October These products, listed below, may have used faulty capacitors provided from one of Lithonia Lighting s suppliers. Recall on the following products: TXC and TXD A23; TXS A30 and TXF A30F; TXR PA22C, TXR PA25D, TXR PA25ALE, TXF PA25ALEF, and TXR PA22D; TXR PA22 GLE, THR PA22L, and THR PA22E; and THR PA22N, THR PA22SP and THR PA16. The recalled fixtures are Indoor High Intensity Discharge (HID) light fixtures, with at least one acrylic component, that are used primarily in industrial and commercial locations such as retail spaces, warehouses, and gymnasiums. Only certain models of specific wattage lights are included in the recall. Additional Information Recall Details How to Identify Affected Products Recall Response Form Contact Info More information for Electrical Distributors A component in the light fixture can leak fluid, which can degrade the acrylic lenses and reflectors, causing them to crack and fall. Falling pieces of acrylic can injure someone below the fixture. Lithonia Lighting will provide a replacement fixture or capacitor for every fixture that meets the applicable criteria. In keeping with our high standards of product integrity and safety, we plan to identify, replace and receive back all affected fixtures or their capacitors subject to this recall. We thank you for your anticipated cooperation, and we regret any inconvenience that this recall may cause. This action is necessary to maintain the high level of product quality that you have come to expect from Lithonia Lighting. We value you as a customer and we are committed to a quick resolution. Indoor HID Acrylic Light Fixture Recall Details Product Recall: TXC and TXD A23; TXS A30 and TXF A30F; TXR PA22C, TXR PA25D, TXR PA25ALE, TXF PA25ALEF, and TXR PA22D; TXR PA22 GLE, THR PA22L, and THR PA22E; and THR PA22N, THR PA22SP and THR PA16. Effective immediately, Lithonia Lighting is issuing a recall on certain fixtures manufactured at our plant in Crawfordsville, Indiana from November 2002 October 2003 that may have used faulty capacitors provided from one of our suppliers. The recall applies to: a. TXC and TXD A23, TXS A30 and TXF A30F probe start 175 watt, 250 watt and 400 watt metal halide models and 200 watt high pressure sodium models, exclusive of any KiloWatch dual-level lighting system options. b. TXR PA22C, TXR PA25D, TXR PA25ALE, TXF PA25ALEF, TXR PA22D probe start 175 watt, 250 watt and 400 watt metal halide models and 200 watt high pressure sodium models, exclusive of any KiloWatch dual-level lighting system options.

TXR PA22GLE probe start 175 watt, 250 watt and 400 watt metal halide models and 200 watt high pressure sodium models, exclusive of any KiloWatch dual-level lighting system options; THR PA22L probe start 250 watt and 400 watt metal halide models and 200 watt high pressure sodium models, exclusive of any KiloWatch dual-level lighting system options; and THR PA22E 175 watt and 250 watt metal halide models and 200 watt high pressure sodium models, exclusive of any KiloWatch dual-level lighting system options. d. THR PA22N, THR PA22SP and THR PA 16 probe start 250 watt and 400 watt metal halide models and 200 watt high pressure sodium models, exclusive of any KiloWatch dual-level lighting system options. The capacitor in these products can leak polypropylene glycol fluid. The leaking fluid can drip out of the ballast housing onto the acrylic lens and/or reflector of the fixture, causing it to degrade. We are aware of several instances where this has resulted in lenses or reflectors cracking and pieces of acrylic falling from fixtures. Our commitment to providing high quality fixtures necessitates a product recall. As of February 20, 2004, we have received reports from 17 jobs where the capacitor leakage resulted in lenses cracking, including 8 jobs where pieces of the lenses fell. In addition, we have received reports from 2 jobs where capacitor leakage resulted in reflectors cracking and falling. One of these reported failures involved minor personal injury; no significant property damage has been reported. This recall does not apply to fixtures manufactured in locations other than Crawfordsville, Indiana because those fixtures did not incorporate the potentially leaking capacitors. Fixtures with aluminum reflectors and acrylic lenses (i.e., TXC and TXD A23; TXS A30 and TXF A30F) must meet four criteria to be recalled: They must meet the wattage criteria specified above; They must have been manufactured in Crawfordsville, Indiana; They must have a date of manufacture from November 2002 through October 2003; and If installed at a job site, they must be exhibiting signs of capacitor leakage, i.e., visible oil or cracked lenses, as identified by visual inspection conducted by you (unless this inspection is conducted during the first two months of operation, in which case all the affected fixtures will be recalled). All other fixtures must meet the following four criteria to be recalled: They must meet the wattage criteria specified above; They must have been manufactured in Crawfordsville, Indiana; They must have a date of manufacture from November 2002 through October 2003; and If installed at a job site, they must contain a potentially leaking capacitor, or in the alternative, must have been in operation for two months or less. When you respond to this notice, we will work with you to determine whether your fixture may contain potentially leaking capacitors. If such a determination cannot be made, all the affected fixtures will be recalled. We ask that you follow the steps outlined on the page How to Identify Affected Products to determine whether the fixture is an affected model number, and the date and place of manufacture. We also ask that you fill out the Recall Response Form and promptly submit it to us. We will provide a replacement fixture or capacitor for every fixture that meets the applicable criteria. In keeping with our high standards of product integrity and safety, we plan to identify, replace and receive back all affected fixtures or
3 their capacitors subject to this recall. Once you have identified that your fixtures may be covered by the recall, call us at from 8:00 AM to 5:00 PM Eastern time, Monday through Friday. How to Identify Products That May be Affected Follow these steps to determine if a product is covered by the Indoor HID Acrylic Recall. For A23, A30 or A30F Reflector Lens Combinations If the product uses our A23, A30 or A30F reflector lens combination (pictured below), please follow these steps to determine if the product is covered by the Indoor HID Acrylic Recall: Confirm that the Ballast Housing was manufactured in Crawfordsville, IN. Confirm that it used one of the following lamp wattages: 400M, 250M, 200S, 175M. Confirm that it was manufactured between 11/1/02 and 10/31/03. Determine if any of the lenses exhibit any form of cracking or any oil residue, or in the alternative, the products have been in operation for two months or less. Confirm that the product description does not contain KW, SCWA, RLB, or LLRPSL.

When you respond to this notice, we will work with you to determine whether your fixture may contain a potentially leaking capacitor. If such a determination cannot be made, the fixture will be covered by the recall.
