8 Remarks Preliminary Paragraph: On 11/15/2018 at approximately 2105 hours, I was dispatched a possible property damage accident at the intersection of Hwy 7 and Shady Oak Rd. Hopkins Police Sergeant White stated that she was on scene of the accident and requested assistance. Officer's Actions/Observations: I arrived on scene and noted the accident to have taken place in the right lane of eastbound Hwy 7, just prior to Shady oak Rd. I noted the road conditions were wet due to the ongoing light rain. It was after sunset and the street lights were on. Hopkins Police Sergeant White informed me of the following: Dennis Lee Dickson 07/11/1955 was driving a Ford truck (N 238-RZC, Veh 2). Colton Patrick Fair 03/16/2002 was driving a Ford SUV (N 574-JTV, Veh 1). I spoke with Dennis and confirmed that he was not injured. Dennis informed me of the following: He was stopped in the right lane of eastbound Hwy 7, waiting for the light to turn green at Shady Oak Rd. An SUV slammed into the rear of his truck. I spoke with Colton and learned the following: He was driving eastbound on Hwy 7 approaching Shady Oak Rd. in the right lane. He tried to hit the brakes but slid into the rear end of a truck. Front airbags did deploy. Sebastian Ruizamorano 06/13/2001 ( ) was in the front passenger seat of Colton's vehicle and was hit in the left side of his face with the air bag. I noted swelling on Sebastian's face. HCC paramedics arrived on scene to treat Sebastian with a ice pack and stated that he did not need to be transported (HCC Run# ). The following people were seated in the rear seat of Colton's vehicle: ichael Landis ( ). Ehjay Talatala ( ). att's Auto arrived on scene and collected both vehicles. Dennis was transported home by his wife. Colton and his passengers were transported to Perkins (11300 Hwy 7 innetonka, N) by CSO K. Sisk to wait for a ride. I later completed a N State Accident Report. Disposition: Airo report; information only. See attached State Accident Report. Date of Print: 11/19/ :13 A Page 2 of 2
