3 Dear Team Captain, Thank you for your interest in the 3rd annual UDS All-Star Jam 3-on-3 basketball tournament, a fundraiser for the All-Star Training Club, a program of United Disability Services. The tournament will take place Saturday, July 28, 2018 at St. Vincent - St. Mary High School s LeBron James Arena. This packet of information will give you all the details about the event, the registration process and the game rules. United Disability Services must receive completed registration packets and the team registration fee by Friday, July 20, Teams will be accepted on a first come, first served basis until all divisions are filled. is our preferred method of communicating with teams, so don t forget to include a valid and active address on your registration form. We will be contacting you to confirm your registration, remind you of any outstanding paperwork and provide last minute information on arrival and playing times. Please note the team captain for all of the youth divisions must be an adult. You can keep up-to-date on all of the pre-tournament action by following us at AllStarJam.org. Willie McGee Director of Athletics, St. Vincent - St. Mary High School 2018 All-Star Jam Tournament Chair UDS All-Star Training Club is an inclusive sports program where children and adults of all abilities can play together competitively. The program offers year-round athletic opportunities such as basketball, track, bowling and soccer that are accessible and inviting for children and adults with disabilities. The program is designed to encourage athletes to strive for their highest potential, both on and off the field. Our nearly 1,000 athletes develop social skills, build selfesteem, learn healthy habits, and create meaningful friendships that last well beyond the athletic season. At UDS All-Star Training Club, we celebrate our athletes many abilities not disabilities to create an enjoyable and memorable experience for all.

4 TOURNAMENT AT A GLANCE TOURNAMENT DATE: Saturday, July 28, :00 a.m. Registration Center Opens (the registration center will remain open until all teams have checked in). 9:00 a.m. All-Star Jam Welcome and Opening Remarks 9:15 a.m. MANDATORY COACHES MEETING for Middle School, High School and Inclusive Divisions 9:30 a.m. Basketball Games Begin 10:30 a.m. MANDATORY COACHES MEETING for Adult and Corporate Divisions Awards Ceremonies begin immediately following championship games for each division. St. Vincent - St. Mary High School LeBron James Arena 15 North Maple Street, Akron, OH REGISTRATION: 1. Entry Fee Per Team (3-5 players) Registration: $60 2. Registration Deadline: July 20, 2018 Teams must pre-register by submitting the included Team Registration Form and paying the entry fee prior to deadline. We encourage you to register early some divisions fill quickly. When we have received your team registration form and entry fee, we will send you a Team Roster and a Waiver & Release of Liability Form. Every player must complete the waiver/ release form before being permitted to play. Youth players and players under guardianship must have a parent or guardian sign the waiver. 5 TOURNAMENT FORMAT Team Format Teams must have a minimum of 3 players maximum of 5 players. Registrations will be taken in the order received until the division is filled. Divisions Co-ed and all female teams are welcome in all divisions. Adult Division Players must be 18 or older Corporate Division Teams must have at least 3 players that are employees of the company. High School Division Players entering 9 th grade in fall through 2018 graduates are eligible. Middle School Division Players entering 6 th -8 th grades in fall. Inclusive Division Lineup ratio requirements are based on Special Olympics Unified Sports rules with two players with a disability and one player without a disability on the court during play. Tournament Format: Pool Play There will be a randomly set pool for each division. Adjustments to pool size will be based on number of teams registered for each division. Tournament organizers reserve the right to modify format. Game Format Regular basket (2 points) & 3-point basket (3 points). Each game is 10 minutes maximum or first team to 21 points for middle school and inclusive divisions; first team to 30 points for high school, adult and corporate divisions. Each game will have an official scorekeeper and referee. Coaches are required to check in with the referee 1 minute before the game begins. Please be at your assigned court on time. Continuous clock. There will be at least 5 minutes between games. All games will start at the same time. Fouls: Shooting fouls 1-4 result in 1-shot and change of possession with all other fouls resulting in change of possession only. At the 5th foul, OR with 2 minutes left in the game, ALL fouls result in 1-shot AND retained possession for the fouled team. For the "AND 1" situation: a foul on a made basket results in the and 1- shot, dead ball make or miss, and change of possession.

6 Game Format Continued... Both the coach and referee are expected to review and sign the final score sheet at score table immediately after the game. Signing signals acceptance of the game result. Any concerns should be elevated to the Court Manager at that time. Honor System and Referees Rule Enforcement Referees cannot see everything, and players are expected to play by the honor system. REFEREES DECISIONS ARE FINAL NO EXCEPTIONS. ELIGIBILITY & TEAM CAPTAINS ALL players must have: A current, signed waiver. Be listed on the team roster. All participants must wear shirts. Your team is encouraged to wear similar colored t-shirts the day of the tournament. Teams will not be allowed to participate if a player s shirt is deemed offensive in any way by tournament staff. Referees may request your team wear pinnies at their discretion. Team Captain Reminders: Team captains are responsible for organizing their team, including turning in a completed roster and signed waiver forms for every player by check-in. Earlier submission will expedite your check-in. It is the captain s responsibility to inform team members of the tournament rules and other pertinent information. All communication with tournament officials should be through the team captain. CODE OF CONDUCT Team Responsibilities and Sportsmanlike Conduct Team captains are responsible for their team's actions. Sportsmanlike conduct is expected at all times by all members. United Disability Services encourages players and teams to have an enjoyable and safe experience. The referees will call unsportsmanlike conduct for: Taunting the other team in any manner. Abusive, off-color language. Aggressively challenging the referee or players on other teams. Inappropriate conduct or actions deemed unsportsmanlike by the referees. Referees have discretion in imposing penalties; including but not limited to, ball turn over, called foul, or disqualification. Referees will issue warnings and may eject players for unsportsmanlike behavior. Ejected players are expected to leave the premises or risk a team forfeiture. There will be zero tolerance for technical fouls. Technical fouls will result in ejection for tournament.

7 Detach and return to UDS with payment by Friday, July 20, TEAM REGISTRATION FORM This form must be filled out by the TEAM CAPTAIN and returned with full payment to United Disability Services to reserve a spot in the tournament. Make your reservations early bracket sizes are limited and teams are registered in the order that registration forms are received. Once we have received your registration, you will receive a Team Roster Form and Player Waiver/ Release Forms for each player. Waivers must be signed by each adult player or by a youth player s parent/guardian. All forms must be turned in to UDS by Friday, July 20, Forms are also available at AllStarJam.org. Teams who have not returned all forms by the 7/20/18 deadline may be disqualified from the tournament without refund. Please contact Teri Mino at or tmino@udsakron.org for additional information.
