1 LARP CARD BULLETIN LARP Card system was introduced in January 2011 with an objective to capture and rectify all unsafe acts and conditions found on board our vessels. This is Synergy s most important tool to achieve the Goal to Zero injury Zero accident Zero Pollution by eradicating unsafe behaviours at the root level itself. This card can also be used to appreciate safe acts & conditions observed on board. LARP stands for Look-Act-Report-Prevent (unsafe acts or conditions) OR Promote (Safe acts conditions).for more explanation; please refer to QHSE Excellence CampaignNo.7. 5 FIRST PRIZE: Name: Mr. Manivannan Vengatesan Rank: AB Vessel: APL Oregon LARP: One of the crew members was found walking under the gantry loading containers. He was immediately stopped. Walking under moving cranes or Gantries can lead to fatal injuries due to falling objects. One has to be vigilant while moving on deck/wharf when alongside performing cargo operations using cranes/gantries. FIRST PRIZE: Name: Mr. Joseph Thomas Antonyraj Rank: Fitter Vessel: Nave Pulsar LARP: A Crew member was using air hose without quick connect coupling. He was immediately stopped and was allowed to work after fixing the coupling. All high pressure hoses should be properly connected by means of a proper coupling. No high pressure hoses should be connected by means of temporary arrangement as the hose may slip due to the high pressure which may lead to serious injury. FIRST PRIZE: Name: Mr. Eleaquim B. Bolando Rank: 3/O Vessel: Anne Mette Bulker LARP: One crew member was found tying the rescue harness to the escape ladder. He was Stopped and was advised not to tie the harness because the rescue harness has to be kept free all the time for emergency purpose so that the harness can be raised to any platform. All rescue equipment s should be kept in a state of readiness at all times and there should not be any obstruction to its access.
6 FIRST PRIZE: Name: Mr. Rinoy Antony Durom Rank: Engine Cadet Vessel: Ruby Express LARP: While entering scavenge space (an enclosed space) an attendant was not posted at the entrance. The entry was stopped and crew members were explained of the procedure Working in enclosed space is a critical task and all the items listed in the enclosed space permit should be strictly adhered while working in enclosed space. SECOND PRIZE: Name: Mr. Raveendran Vishnu Rank: Deck Cadet Vessel: Cala Pinguino LARP: During cargo operation, it was observed some stevedores were walking between cross deck on opened hatch cover without wearing any PPE. They were immediately stopped Prior commencement of cargo operation, a safety meeting shall be conducted with the foreman where all the safety procedures that need to be followed by the stevedores while on board should be explained.
7 SECOND PRIZE: Name: Mr. TEOFIL, JOHN P. Rank: OS Vessel: Occitan Barsac LARP: During cold weather, found a crew member was not wearing safety helmet. Instead he was wearing a monkey cap covered with parka jacket hood. Company PPE Matrix given in Health and Safety manual should be complied with at all times. SECOND PRIZE: Name: Mr. Sliem Fielpan Rank: Bosun Vessel: Jubilant Excellence LARP: While the shore workshop was doing hot work on the hatch cover there were scattered coal cargo and rags near the area. Immediately the job was suspended and the area was cleaned. Prior commencing any Hot work a thorough Risk assessment should be carried out where all the hazards are identified and control measures put in place to prevent such occurrence. SECOND PRIZE: Name: Mr. Praveen Rank: OS Vessel: Star Express LARP: A crew member was found crossing over a mooring rope which was under tension. He was immediately Stopped and advised about adverse effect of crossing on top of mooring rope. As per the COSWP 2015, the entire mooring area is a hazardous zone and extreme caution should be exercised when working in the vicinity.
8 THIRD PRIZE: Name: Mr. Krishna Kumar Kaliveettil Rank: Electrical Officer Vessel: Pacific Dawn LARP: One of the shore de-mucking crew found smoking near bunker tanks. Immediately he was stopped him, advised him about the consequences. It is necessary to ensure that the shore staff are made aware of the hazards associated with the job they are engaged in. THIRD PRIZE: Name: Mr. NISCO, RANDY E. Rank: AB Vessel: Occitan Barsac LARP: While carrying out hot work in open space it was found that all precaution were taken as required and PTW sheet was displayed at the site. All crew members are encouraged to identify the good acts and appreciate the same.
9 THIRD PRIZE: Name: Mr. Hariram Ganapathy Rank: Bosun Vessel: APL Florida LARP: One of the crew members was standing next to the tail rope of the tug line while casting off. Immediately he was advised to move away from the location. The dangers associated with mooring explained to him As per the COSWP 2015, the entire mooring area is a hazardous zone and extreme caution should be exercised when working in the vicinity. THIRD PRIZE: Name: Mr. Sauhardya Bhadra Rank: 3/O Vessel: IS China LARP: One of the crew member found to be walking on hatch coaming while cargo operation was in progress with hatch cover opened. The crew member asked to come down from the coaming and was made to understand the hazards associated with the act. All crew members are advised to strictly adhere to the safety procedures.
