1 Grays Harbor EMS BLS OTEP Training Tracking Form Training/Skills -- ANNUAL Training/Skills -- ADDITIONAL ~ Using 2016 senarios from GHEMS ~ Required Once per cert cycle DOCUMENTATION / TRACKING SHEETS ~ Required once per cert cycle Healthcare Provider CPR Renewal (test & skills) ~ MUST maintain a current AHA HCP cert at all times ~ Copy of AHA HCP card required with ReCert Notes: Above, only list dates for training completed in this specific year. Example: training completed in 2014 or 2015 shall not be listed on the 2016 tracking form. If a training topic is completed multiple times in the same year then list all dates on that year's tracking form. ** IVs: A Minimum of 12 successful IV sticks must be completed each year of your certification cycle. The intent is for the sticks to be completed throughout the year. The sticks may not be clustered at the end. Effective January 1, 2014, IV Proficiency training classes are no longer be accepted as an equivelency for sticks.
