Page 1 of 5 LOCATION WRITTEN BY APPROVED BY DATE CREATED LAST REVISION All Schools Lorie Carriere Physical Therapists August 9, 2012 April 3, 2017 PERSONAL PROTECTION EQUIPMENT (PPE) OR DEVICES Appropriate footwear must be worn. Shoe must be fully enclosed. No open toed shoes or sandals. Long and loose hair should be tied back to prevent it from being grabbed or coming into contact with the students face. Floor Lift & sling ADDITIONAL REQUIREMENTS Safe Lifting Operation of the NVCI managing reactive / defensive behaviors If the procedure is not followed, the task has the potential for awkward postures and overexertion which can increase the risk of back or upper limb **musculoskeletal injuries. Grabs / strikes from reactive or defensive behavior Pinch points for fingers HAZARDS PRESENT **Signs and symptoms of a musculoskeletal injury (MSI) can include pain, burning, swelling, color changes, numbness/tingling, and loss of movement or strength in a body part. Workers must inform their supervisor if they experience these or other signs and symptoms so the work tasks can be re-assessed by the supervisor or physical therapist. SUPPORTIVE INFORMATION Purpose: To ensure safety of students/staff during transfers with the lift to and from a variety of surfaces including wheelchair, toilet, commode, shower chair, change table, exercise mat, etc. To reduce the potential for injuries to both students and staff. Application: This type of lift is for use with students who are able to weight bear reliably or consistently through upper and lower extremities or can actively participate or risk factors are present which threaten the safety of a manual transfer or use of a sit-stand lift. OT / PT must decide on a case by case basis whether one or more staff members are required for any transfers using the Sabina sit to stand lift.
Page 2 of 5 SAFE WORK PROCEDURE SAFETY PRECAUTIONS: Never exceed the maximum lifting capacity of the lift Active lifting 440 lbs., passive lifting 330 lbs. Never exceed the maximum lifting capacity of the Sabina seat strap 440 lbs. Always plan the lift before starting. Never leave a student unattended during a lifting situation. Always carry out the daily safety checks before using the lift. Do not use a sling unless it is recommended for use with the lift. Ensure the lifting sling is correctly and safely applied to the patient in order to avoid bodily injury. Ensure the slings are correctly applied to the sling bar Ensure that the sling bar latches are intact Ensure the sit to stand vest / sling s straps are properly connected to the sling bar hooks when the straps have been fully extended but before the patient is lifted from the underlying surface. Do not disconnect or bypass a control or safety feature. Do not push a loaded lift at speeds which exceed a slow walking pace. Do not push the lift on uneven or rough ground, especially when loaded. Do not lift students unless you are trained and competent to do so. Do not part / stop a loaded lift on any sloping surface. Never run the batteries completely flat. Charge the lift regularly after use. PRE-USE DAILY INSPECTION: Ensure the lift moves freely on its castors. Inspect the lift and check to ensure there is no external damage. Check the functionality of the locking handles Check the functionality of the latches. Ensure the sling bar is firmly attached to the boom. Examine the sling hooks on the bar and look for excessive wear. If in doubt - do not use. Examine slings for fraying or other damage. DO NOT use any sling if damaged. Operate the hand control or the hydraulic unit to confirm the boom raises and lowers satisfactorily. Check the base width adjustment. Confirm the lift is not giving a low battery alarm when the hand control is operated (electric lifts only). If the batteries indicate charge immediately DO NOT use and place on charge immediately. Check the operation of the emergency stop button and the emergency lowering (both electrical and mechanical).
Page 3 of 5 Sling bar 350 Comfort sling bar Mechanical emergency lowering Lower leg support adjustment Hand control unit Emergency stop Electric emergency lower / raise Height adjustment Wheel locks PROCEDURE: 1. Ensure the Sabina lift is in proper working order, the battery is sufficiently charged and all attachments are available. 2. Ensure that there is adequate space to maneuver the lift in the area, i.e. check for cords on floor, move chairs/bed tables, etc. 3. Communicate to the student and other staff the planned movements in a clear, non-rushed, respectful manner. 4. Choose the appropriate sling according to the logo. Inspect the sling for signs of wear and tear prior to use. 5. Apply the support vest: a. Lock the breaks on the chair and move the foot rests out of the way. Place the Support Vest around the student according to the vest s instruction guide. Place the lift in front of the student and adjust the width of the base. Place the feet in the middle of the foot rest with the lower legs parallel to the lower leg support. Adjust the height and depth of the lower leg support as needed for comfortable support below the kneecap.
Page 4 of 5 b. Connect the vest s straps to the sling bar s hooks. Tighten the calf strap. 6. Raising: a. Raising the sling bar: Raise the sling bar about cm (4-8 in). The student grabs the sling bar. Continue the lifting procedure. If the student leans backwards, the raising will be made easier, preventing the vest from sliding up. The height to which the lift should proceed varies from person to person. Note: Before the student is lifted from the underlying surface, but after the straps have been fully extended, make sure the straps are properly connected to the sling bar. For a more upright position, continue the lifting motion to the uppermost position. The raising motion can be experienced as unpleasant for the person not used to it. For maximum comfort, the lift mast should be affixed to the base in the best possible of the three fastening holes, depending on the height of the student. 7. Back to a sitting position: a. Reverse the process for raising the student. Do not use a seat strap during lowering, the patient is more easily placed into the right position in the wheelchair if the seat strap is not used. 8. Disengage the Student from Lift: a. Once the student is in the seated position, unhook the vest from the lift. Unfasten the front of the vest and cue the student to lean forward as the staff member removes it. b. Ensure that the student is in safe & comfortable & positioned properly. Thank the student for their efforts.
Page 5 of 5 PREVENTATIVE MAINTENANCE: 1. Washing instructions for slings: Machine wash in warm or cold water. Do not use bleach. Do not wash with other colors. Air dry or very low dryer heat. 2. Check the following regularly (monthly): Check the safety belts/vests for signs of wear. Check wheels and brakes for proper functioning, clean as needed. 3. The Sabina list should be periodically inspected at least once / year by an authorized Liko representative. REGULATORY REQUIREMENTS WS&H Act W210, Mb.
