We are delighted that you have chosen to become a part of one of the most prestigious allstar programs in the country. Our mission at The Stingray Allstars is to enrich the lives of the children and their families through the sport of competitive cheerleading. Our goal each day is to teach our athletes the value of commitment, hard work, integrity, leadership, self-confidence, positive attitudes and a love for the sport. Each and every family is important to us and our program. Welcome to the family where you are 1 of a kind! Please be prepared to turn in the Financial Commitment/Credit Card Info page, Tryout Form, Member Information Form, Waiver, Team Rep form and your tryout fee. If you did not turn in a birth certificate during the 2013-2014 season (Full Year or Half Year) you must provide a photocopy of your child s birth certificate. Tryout fee is $50.00 if you turn in your completed packet by April 17 th. Any tryout packet turned in on April 18 th or after will be charged a $75.00 tryout fee. THIS IS A FIRM DATE, NO EXCEPTIONS. INCOMPLETE PACKETS WILL NOT BE ACCEPTED. FAQ S Q: How much does it cost? A: Approximately $3240.00. Not included: uniform, warm-ups, travel costs, Worlds and Summit fee and Coaches Fee (extraneous team shirts, bows, etc. are not included as well). Q: Who are the coaches? A: Our entire coaching staff is certified through USASF. We are privileged to have one of the most experienced and knowledgeable group of coaches in the country. We enjoy a very low turnover rate; many coaches have been on staff since our establishment in 2002. All of our coaches are excited to create the best competition experience for your cheerleader. Q: What competitions do we attend? A: We will release a competition schedule by mid-june. Q: How do you select teams? A: We select teams to be the most competitive in each level. We base these placements on physical ability (jumps, tumble, motions, etc.), as well as past cheer experience. Q: Does everyone make a team? A: We do try our best to place every athlete. However, if you have requested a specific level and we cannot accommodate that, you will not be placed on a team. Q: Are there different expectations and finances for Worlds teams? A: Yes. Your coach will hold an informational meeting about expectations/finances at the beginning of the season. 1

Tuition/Fee Information All payments are made to Stingray Cheer Co. or Stingray Sport Center. May 2014 through January 2015 monthly tuition payments are $310.00. February 2015 through April 2015 monthly tuition payments are $150.00. Monthly Tuition Includes: Monthly Tuition Does Not Include: All team practices and a reserved spot in a one hour weekly tumble class. 2 tumble class drop-ins per week, 1 per day max. Allstar Camp Choreography Fee Music Fee Competition Fees $155.00 Coach s Fee, due within one week of team placement. This fee is per family, not per cheerleader. Uniform, warm-ups, shoes, bows, etc. Worlds Fee Summit Fee Senior Level 5 Stunt Camp Travel Expenses Your monthly fees will be billed to your Stingray account on the 1 st of each month. Fees are due on or before the 7 th of each month. On the 8 th of each month any outstanding balance will be charged to your credit card that was provided in your Tryout Packet. Stingray Cheer Co. and Stingray Sport Center accepts cash, checks, money orders and all major credit cards. You may also pay online at www.stingrayallstars.com. Follow the prompts to your gym location. If you have any questions about your account feel free to contact the office. All accounts must be paid in full in order to tryout for the upcoming season. Please call the office with financial questions, your coach will not be able to advise you on this matter. Cheerleaders may be asked to sit out of team practices if accounts are not current. There will be no refunds made to anyone who is asked to leave the program or that quits a team. Class Policies Class Policy: Every cheerleader is entitled to 1 tumble class per week. You are also allowed up to 2 drop-ins during the week, 1 per day max. Drop-ins are considered a privilege, not a guarantee and you must call the office to schedule. *New Policy*: Accounts must be current in order to do drop-ins, privates, additional tumble, stunt or stretch classes. 2

Stingray Cheer Company, Inc. and Stingray Sport Center, LC PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK In consideration of the services of Stingray Cheer Company, Inc. and Stingray Sport Center, LLC, its owners, agents, officers, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as SA ), I hereby agree to release, discharge, and hold harmless SA, on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate as follows: 1. I understand and acknowledge that the activities that I or my child engage in while on the premises or under the auspices of SA pose known and unknown risks which could result in injury, paralysis, death, emotional distress, or damage to me, my child, to property, or to third parties. The following describes some, but not all, of those risks: Cheerleading and gymnastics, including performances of stunts and use of trampolines, entail certain risks that simply cannot be eliminated without jeopardizing the essential qualities of the activity. Without a certain degree of risk, cheerleading students would not improve their skills and the enjoyment of the sport would be diminished. Cheerleading and gymnastics expose participants to the usual risk of cuts and bruises, and other more serious risks as well. Participants often fall, sprain or break wrists and ankles, and can suffer more serious injuries. Traveling to and from shows, meets and exhibitions raises the possibilities of any manner of transportation accidents. In any event, if you or your child is injured, medical assistance may be required which you must pay for yourself. 2. I expressly agree and promise to accept and assume all of the risks, known and unknown, connected with SArelated activities, including but not limited to performance of stunts and use of trampolines. My participation and that of my child is purely voluntary. No one has forced or coerced me or my child to participate. I elect for myself and my children to participate in such activities in spite of the risks. 3. I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify SA from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with my child s participation in SA-related activities. 4. Should SA be required to incur attorney s fees and costs to enforce this agreement, I agree to indemnify and reimburse them for such fees and costs. 5. I certify that my child has health, accident and liability insurance to cover bodily injury or property damage that may be caused or suffered while participating in this event or activity, or else I agree to bear the costs of such injury or damage to my child. I further certify that I am willing to assume and bear the costs of all risks that may arise or be created, directly or indirectly, through or by any such condition. 6. In the event that I file a lawsuit against SA, I agree to do so solely in the State of Georgia and I further agree that the substantive and procedural laws in that state shall apply in any such action without regard to the conflict of laws rules thereof. I agree that if any portion of this agreement is found void or unenforceable, the remaining portions shall remain in full force and effect. 7. By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation or the participation of any of my children in this activity, I may be found by court of law to have waived my right to maintain a lawsuit against SA on the basis of any claim from which I have released SA by signing this Agreement. Program Rules/Codes of Conduct If you have any questions or concerns that need immediate attention please use the following chain of command: 1. Team Rep 2.Head Coach 3.Allstar Director 4.Owner/President The Stingray Allstars maintain the right to refuse services at any time. Good sportsmanship, polite manners and a good disposition are mandatory at all practices and competitions. It is your responsibility to wear the appropriate practice wear to every practice. Personal items such as cell phones, school bags, or gym bags must be taken into the gym and stored in the cubbies adjacent to your practice floor. NO items are to be left in the gym hallways and cell phones must be turned off when practice starts. NO GOSSIP about any other teams (allstar or school). NO GOSSIP about a child on your team or another team. NO GOSSIP about coaches and staff. It is much better to address a problem than to listen to idle gossip. No profanity or abusive language. Please keep in mind the gym is not a babysitting service. We want our Allstars to use the gym as often as possible, but no child should be dropped off at the gym unsupervised. You must arrive at all practices, competitions or any scheduled event on time. Punctuality is a MUST. During practice your opinion does not count. All squad and routine decisions are left to the discretion of the coaches. Please feel free to talk to your coach about anything; remember to do it at the appropriate time. For example, approaching a coach in the middle of a practice would not be an appropriate time. Never post any negative comments on social media. Practices may be changed or added at any time during the season. Anyone threatening to quit or to pull their child from a team will be dismissed from the program immediately. Only cheerleaders and coaches are allowed in the gym. No one is allowed to yell onto the floor or try to make contact through the parent viewing area window during practices or tumble classes. This is extremely distracting to all involved. The coaches reserve the right to close practices at ANY time for ANY reason. In this event, the team rep will remain at practices. It is the parent s responsibility to know what is going on with your squad. Check your emails and the website regularly. Withholding a child from a practice or a competition should never be used as a form of punishment. Athletes should be able to handle school work and allstar practices, homework is not an acceptable excuse for missing practice. Parents, relatives, friends and cheerleaders are never allowed to speak with competition officials for any reason. Parents are never allowed to represent The Stingray Allstars under any circumstances concerning accommodations, competitions or any other situation. There will be no arguing or questioning of the coaching staff s decisions at competitions. If a problem arises between you and a teammate or another parent, the problem will be addressed with all parties involved at a meeting with your coach and the Allstar director. Anonymous emails/letters/texts/twitters, etc. will be ignored. The Allstar Director may change, add or subtract any rule at any time. 8

Travel All Cheerleaders will use the same accommodations at all out of town competitions. DUE TO CONTRACT AND STAY TO PLAY OBLIGATIONS ALL ATHLETES ARE REQUIRED TO STAY AT TEAM HOTELS. There must be a parent in every room, no exceptions. Each cheerleader must have a chaperone at every competition. It is not your coach, Team Rep or Stingray staff s responsibility to be your child s chaperone. It is your responsibility to pay the hotel directly for your room. Bus service will be provided at NCA in Dallas. The cost of the bus is an additional fee that will be paid by each family. (Note: The buses run between hotels and competition site only.) Proper traveling attire will be discussed with each team prior to an out of town competition. Apparel All Stingray logos and brands are protected. All spirit wear items are to be purchased through Full Out Sports. If a sponsor wants to buy an item for entire team; that order must go through Full Out Sports as well. All spirit items must be approved by Full Out Sports, coaches have sole authority on t-shirt designs (this includes sponsored items also). Bows, t-shirts, bags, blankets, etc. with an identifiable icon (i.e. orange slice, clover, peppermint candy) not purchased through Full Out Sports are not permitted. Teams will not be allowed to wear or use as a group. Anyone that attempts to sell or give away items with Stingray logos or likenesses will be in violation of our policy. Money cannot be asked of parents to buy spirit items, goody bag items or any other nonmandatory items. Team Reps may collect money for coach gifts, but this is strictly voluntary. The Stingray Allstars have worked very hard to establish a brand and we take this policy very seriously. Please help us protect our good name! The Rest of the Story Thank you for choosing to become a part of our program. Our hope is that this is a wonderful experience for your entire family. However, we do know life gets in the way sometimes and we want you to know up front some ways that your spot may be in jeopardy on your team: delinquent accounts, loss of skills/abilities, extensive absences/tardiness, poor attitude or injuries. If an injured athlete is replaced in a routine, they may or may not be worked back in the routine, this will be up to the coach. As always, if you have any questions feel free to contact us.
